目的:探讨经外侧裂小骨窗开颅血肿清除术(简称小骨窗)与立体定向血肿抽吸术(简称立体定向)治疗高血压脑出血的疗效.方法:分别对80例高血压脑出血患者分别采用经外侧裂小骨窗开颅血肿清除术及立体定向血肿抽吸术治疗.结果:小骨窗组与立体定向组血肿抽吸术患者术后偏瘫开始恢复时间分别为(12±3.2)天、(18±4.5)天,两组比较有显著性差异(P〈0.05);术后肺部感染和上消化道出血的发生没有明显差别(P〉0.05);预后(Ac-tivity of daily living,ADL)小骨窗组优于立体定向组(P〈0.05).结论:与立体定向血肿抽吸术相比,经外侧裂小骨窗开颅血肿清除术能够有效清除血肿,是治疗高血压脑出血的理想方法.  相似文献

观察立体定向穿刺引流术用于高血压基底节区脑出血的效果。将华亭市第一人民医院2018年3月~2019年5月收治的84例高血压基底节区脑出血患者按照随机数字表法划分成对照组(n=42例)和治疗组(n=42例),分别采取传统开颅手术、立体定向穿刺引流术治疗。比较两组临床疗效、神经功能改善情况、术后并发症发生率。治疗组总有效率92.86%较对照组的73.81%高;两组术后美国国立卫生研究院卒中量表(NIHSS)与术前相比均下降(P0.05),且治疗组较对照组低(P0.05);治疗组术后并发症发生率9.52%较对照组的30.95%低(P0.05)。对高血压基底节区脑出血患者实施立体定向穿刺引流术治疗,其疗效优于传统开颅手术治疗,可最大程度恢复患者神经功能,并能减低术后并发症的发生。  相似文献

研究分析对高血压脑出血患者分别采用小骨窗开颅以及常规骨瓣开颅的临床治疗效果。选取医院2014年12月~2016年12月接收的84例高血压脑出血患者,通过计算机产生随机数字,分为观察组和参照组,其中参照组为42例,实施常规骨瓣开颅治疗手段,观察组为42例,予以小骨窗开颅手段,比较两组并发症发生情况以及手术各项指标变化情况。比较分析,观察组并发症发生率为(4.76%),参照组并发症发生率为(19.06%),同时观察组手术时间、术中出血量等各手术指标,均优于参照组,差异有统计学意义(P0.05)。高血压脑出血患者采用小骨窗开颅治疗手段,效果显著,值得推广。  相似文献

目的:探讨高血压脑出血(Hypertensive intracerebral hemorrhage,HICH)开颅血肿清除术后的护理要点.方法:对54例高血压脑出血患者行开颅血肿清除术的术前护理、术后护理及并发症的护理进行总结.结果:54例患者死亡4例,自动出院3例,植物人2例,重度残疾8例,中度残疾34例,好转3例.结论:高血压脑出血开颅血肿清除术的手术期护理尤其重要,术后应控制血压在正常水平,防止再出血和脑缺血,预防感染,加强功能锻炼以促进患者的恢复.  相似文献

目的探讨影响微创法清除颅内血肿术治疗高血压脑出血的治疗效果及预后因素.方法对126例高血压脑出血微创清除颅内血肿术的临床资料进行统计学处理.结果126例患者术后存活120例(95.3%),本组死亡6例(4.8%).生存患者术后3个月随访,用ADL(日常生活能力)评定疗效.ADL1:恢复日常生活34例(26.9%);ADL2:生活自理35例(27.7%);ADL3:部分生活自理33例(26.1%);ADL4:卧床不起但有意识21例(16.6%);ADL5:植物状态3例(2.4%).结论出血至手术时间、意识与脑疝征、瞳孔改变、中线移位4项因素对微创清除颅内血肿术,治疗高血压脑出血患者具有显著的预后价值,超早期手术是治疗成功的关键.  相似文献

目的探讨不同术式治疗高血压脑出血的疗效。方法根据人院时出血部位、血肿量、GCS评分等选择不同术式对高血压脑出血116例进行治疗,并分析不同术式的疗效。结果116例脑出血中死亡27例,存活89例。与骨瓣开颅血肿清除术组、微创穿刺血肿清除术组相比,小骨窗开颅血肿清除术组生活完全自理率最高,生活完全不能自理率最低。结论小骨窗开颅血肿清除术能够明显改善高血压脑出血患者的预后,提高生存质量。  相似文献

观察小骨窗开颅血肿清除术及钻孔引流术用于高血压脑出血患者的实际疗效,探讨不同治疗方案的临床应用价值及选择。选择2016年1月至2016年12月临夏州人民医院收治的78例高血压脑出血患者,按照随机原则分为例数相等的观察组(n=39)及对照组(n=39),其中观察组采用小骨窗开颅血肿清除术治疗,对照组采用钻孔引流术治疗。观察指标包括两组患者血肿清除率、手术时间及肺部感染发生率。观察组患者血肿清除率高于对照组,但手术时间较对照组长,且肺部感染率高于对照组,各项观察指标组间差异均具有统计学意义(P0.05)。小骨窗开颅血肿清除术用于高血压脑出血的临床治疗,血肿清除率高于钻孔引流术,但手术时间较长,且会增加肺部感染的风险,在临床工作中应考虑患者的年龄、病情及免疫状态,综合判断选择手术方式。  相似文献

目的:探讨经侧裂入路显微手术治疗壳核区高血压脑出血的效果,总结临床经验.方法:回顾我院于2001年1月-2007年2月收治的壳核或以壳核为主的高血压脑出血患者246例,均采用小骨窗经外侧裂入路在显微镜下清除血肿.结果:全部病例血肿清除满意.血肿完全清除231例(93.9%),近全清除15例(6.1%).术后无一例血肿复发.无手术死亡病例.结论:本手术方法不受出血时间的限制;不受血肿量大小的限制;能充分利用脑组织正常间隙及非重要功能区进行操作,脑组织损伤小,减少了手术的创伤;能够清晰看清出血点,更准确有效地止血,降低了手术后再出血的风险;血肿彻底清除.  相似文献

目的:对微创手术治疗中少量高血压脑出血的疗效进行探讨。方法:选取自2012年5月到2013年8月在我院接受中少量高血压脑出血治疗的48例患者,通过随机抽样的方法分为对照组与观察组,每组患者各24例。对照组患者采用保守方法进行治疗,而观察组患者采用微创手术进行治疗,对两组患者的临床治效果进行比较。结果:观察组患者接受治疗后,血肿全部消退,有3例患者出现并发症;对照组有7例患者在接受保守治疗无效后,改行手术治疗;对照组患者所表现出的日常生活能力要明显弱于观察组,两组比较差异具有统计学意义(P<0.05)。结论:采用微创手术对中少量高血压脑出血患者进行治疗,能够取得非常理想的治疗效果,值得在临床中广为推广。  相似文献

The discovery of the prolific Ordovician Red River reservoirs in 1995 in southeastern Saskatchewan was the catalyst for extensive exploration activity which resulted in the discovery of more than 15 new Red River pools. The best yields of Red River production to date have been from dolomite reservoirs. Understanding the processes of dolomitization is, therefore, crucial for the prediction of the connectivity, spatial distribution and heterogeneity of dolomite reservoirs.The Red River reservoirs in the Midale area consist of 3~4 thin dolomitized zones, with a total thickness of about 20 m, which occur at the top of the Yeoman Formation. Two types of replacement dolomite were recognized in the Red River reservoir: dolomitized burrow infills and dolomitized host matrix. The spatial distribution of dolomite suggests that burrowing organisms played an important role in facilitating the fluid flow in the backfilled sediments. This resulted in penecontemporaneous dolomitization of burrow infills by normal seawater. The dolomite in the host matrix is interpreted as having occurred at shallow burial by evaporitic seawater during precipitation of Lake Almar anhydrite that immediately overlies the Yeoman Formation. However, the low δ18O values of dolomited burrow infills (-5.9‰~ -7.8‰, PDB) and matrix dolomites (-6.6‰~ -8.1‰, avg. -7.4‰ PDB) compared to the estimated values for the late Ordovician marine dolomite could be attributed to modification and alteration of dolomite at higher temperatures during deeper burial, which could also be responsible for its 87Sr/86Sr ratios (0.7084~0.7088) that are higher than suggested for the late Ordovician seawaters (0.7078~0.7080). The trace amounts of saddle dolomite cement in the Red River carbonates are probably related to "cannibalization" of earlier replacement dolomite during the chemical compaction.  相似文献

There are numerous geometric objects stored in the spatial databases. An importance function in a spatial database is that users can browse the geometric objects as a map efficiently. Thus the spatial database should display the geometric objects users concern about swiftly onto the display window. This process includes two operations:retrieve data from database and then draw them onto screen. Accordingly, to improve the efficiency, we should try to reduce time of both retrieving object and displaying them. The former can be achieved with the aid of spatial index such as R-tree, the latter require to simplify the objects. Simplification means that objects are shown with sufficient but not with unnecessary detail which depend on the scale of browse. So the major problem is how to retrieve data at different detail level efficiently. This paper introduces the implementation of a multi-scale index in the spatial database SISP (Spatial Information Shared Platform) which is generalized from R-tree. The difference between the generalization and the R-tree lies on two facets: One is that every node and geometric object in the generalization is assigned with a importance value which denote the importance of them, and every vertex in the objects are assigned with a importance value,too. The importance value can be use to decide which data should be retrieve from disk in a query. The other difference is that geometric objects in the generalization are divided into one or more sub-blocks, and vertexes are total ordered by their importance value. With the help of the generalized R-tree, one can easily retrieve data at different detail levels.Some experiments are performed on real-life data to evaluate the performance of solutions that separately use normal spatial index and multi-scale spatial index. 本文叙述了对海南岛及其毗邻大陆边缘白垩纪到第四纪地层岩石进行古地磁研究的全部工作过程。通过分析岩石中剩余磁矢量的磁偏角及磁倾角的变化,提出海南岛白垩纪以来经历的构造演化模式如下:早期伴随顺时针旋转而向南迁移,后期伴随逆时针转动并向北运移。联系该地区及邻区的地质、地球物理资料,对海南岛上述的构造地体运动提出以下认识:北部湾内早期有一拉张作用,主要是该作用使湾内地壳显著伸长减薄,形成北部湾盆地。从而导致了海南岛的早期构造运动,而海南岛后期的构造运动则主要是受南海海底扩张的影响。海南地体运动规律的阐明对于了解北部湾油气盆地的形成演化有重要的理论和实际意义。  相似文献

The explosive growth of the Internet and database applications has driven database to be more scalable and available, and able to support on-line scaling without interrupting service. To support more client's queries without downtime and degrading the response time, more nodes have to be scaled up while the database is running. This paper presents the overview of scalable and available database that satisfies the above characteristics. And we propose a novel on-line scaling method. Our method improves the existing on-line scaling method for fast response time and higher throughputs. Our proposed method reduces unnecessary network use, i.e. , we decrease the number of data copy by reusing the backup data. Also, our on-line scaling operation can be processed parallel by selecting adequate nodes as new node. Our performance study shows that our method results in significant reduction in data copy time.  相似文献

R-Tree is a good structure for spatial searching. But in this indexing structure,either the sequence of nodes in the same level or sequence of traveling these nodes when queries are made is random. Since the possibility that the object appears in different MBR which have the same parents node is different, if we make the subnode who has the most possibility be traveled first, the time cost will be decreased in most of the cases. In some case, the possibility of a point belong to a rectangle will shows direct proportion with the size of the rectangle. But this conclusion is based on an assumption that the objects are symmetrically distributing in the area and this assumption is not always coming into existence. Now we found a more direct parameter to scale the possibility and made a little change on the structure of R-tree, to increase the possibility of founding the satisfying answer in the front sub trees. The geographic information service is enabled by the advancements in general Web service technology and the focused efforts of the OGC in defining XML-based Web GIS service. Based on these models, this paper addresses the issue of services chaining,the process of combining or pipelining results from several interoperable GIS Web Services to create a customized solution. This paper presents a mediated chaining architecture in which a specific service takes responsibility for performing the process that describes a service chain. We designed the Spatial Information Process Language (SIPL) for dynamic modeling and describing the service chain, also a prototype of the Spatial Information Process Execution Engine (SIPEE) is implemented for executing processes written in SIPL.
