经外侧裂小骨窗开颅术与立体定向术治疗高血压脑出血的临床研究

目的：探讨经外侧裂小骨窗开颅血肿清除术（简称小骨窗）与立体定向血肿抽吸术（简称立体定向）治疗高血压脑出血的疗效．方法：分别对80例高血压脑出血患者分别采用经外侧裂小骨窗开颅血肿清除术及立体定向血肿抽吸术治疗．结果：小骨窗组与立体定向组血肿抽吸术患者术后偏瘫开始恢复时间分别为（12±3．2）天、（18±4．5）天，两组比较有显著性差异（P〈0．05）；术后肺部感染和上消化道出血的发生没有明显差别（P〉0．05）；预后（Ac-tivity of daily living，ADL）小骨窗组优于立体定向组（P〈0．05）．结论：与立体定向血肿抽吸术相比，经外侧裂小骨窗开颅血肿清除术能够有效清除血肿，是治疗高血压脑出血的理想方法．

Clinical Study on the Effect of Small Bone Window Craniotomy Via Lateral Fissure and Stereotaxy on Hypertensive Cerebral Hemorrhage

Objective：To compare the curative effect of small bone window craniotomy via lateral fissure to evacuate hematoma with that of aspiration of hematoma by stereotaxy to treat hypertensive cerebral hemorrhage.Methods：80 cases of this disease were treated with above-mentioned methods.Results： Recovery time from hemiparalysis for the patients treated with small bone widow craniotomy via lateral fissure to evacuate hematoma and those treated with aspiration of hematoma by stereotaxy was （12 ±3.2） and （18 ± 4.5） days respectively.There was significant difference between two therapies（P〈0.05） but no significant difference for postoperative pulmonary infection and hemorrhage of upper digestive tract between two therapies （P〉0.05）.The patient＇s prognosis treated with small bone window craniotomy was more favorable than that with aspiration of hematoma.Conclusion：Small bone window craniotomy via lateral fissure can more effectively evacuate hematoma and is an effective therapy to treat hypertensive cerebral hemorrhage compared with aspiration of hematoma by stereotaxy.
