手法复位、AF钉内固定治疗胸腰段骨折体会

探讨了手法复位后,AF钉内固定治疗胸腰段骨折的效果.自2003-2009年3月,共收治33例胸腰段骨折的病人,按Frankel分级,A级8例,B级14例,C级9例,D级2例.伤后至手术时间3～10d.笔者在全麻下采用脊柱可调式支架复位、椎弓根螺丝钉内固定、椎板减压、后突骨块复位术治疗脊柱胸腰段骨折并截瘫病人,术后拍片及CT检查显示骨折完全复位,脊髓无压迫,均有不同程度的肌力恢复.本组33例,术后X线片示压缩椎体高度及生理前凸恢复,CT显示突入椎管内骨块复位,椎管体积恢复.无1例脊髓损伤加重者.本方法治疗胸腰段骨折方法简单、疗效可靠.     

钢针撬拨复位多针内固定治疗跟骨关节内骨折25例

笔者自2005年1月～2007年6月,共收治跟骨关节内骨折25例（29足）,采用钢针闭合撬拨手法整复,多针经皮内固定方法治疗,疗效满意,现报道如下。1临床资料本组25例（29足）,男21例,女4例,年龄18～62岁,平均36．5岁。单侧21例,双侧4例。致伤原因：高处坠落22例,交通意外3例。25例皆累及距下关节面。伴有胸腰椎压缩性骨折2例,合并双下肢多发性骨折2例,合并骨盆骨折1例。     

辉力灌肠治疗胸腰段脊柱骨折后腹胀便秘44例报告

目的：观察辉力（磷酸钠灌肠液生产厂家：c.B.Fleet company,Inc.）灌肠治疗胸腰段脊柱骨折后腹胀便秘的临床疗效。方法：将86例胸腰段脊柱骨折患者随机分为2组：观察组以辉力灌肠为基本方治疗,对照组采用口服缓泻剂或清洁灌肠等对症处理。结果：观察组的临床疗效效果优于对照组,观察组显效率和有效率均显著优于对照组（P〈0.01）。结论：使用辉力保留灌肠能够有效地缓解胸腰段骨折后腹胀便秘。     

跨伤椎固定与经伤椎固定治疗胸腰段脊柱骨折的效果对比分析

对比分析跨伤椎固定与经伤椎固定治疗胸腰段脊柱骨折的效果。随机选取兰州市红古区人民医院2014年5月~2015年11月收治的胸腰段脊柱骨折患者84例,并将其分为研究组与对照组,每组各42例。对照组采取跨伤椎固定治疗,研究组实施经伤椎固定治疗。治疗6个月后,对两组患者的临床疗效及VAS、JOA评分进行比较。结果:对比两组患者治疗前及治疗后即刻VAS、JOA评分,无显著差异(P0.05);研究组患者治疗后即刻Cobb角、即刻矫正度、6个月后Cobb角及矫正丢失度均显著优于对照组(P0.05)。对胸腰段脊柱骨折患者实施经伤椎固定治疗,疼痛及脊柱功能改善效果显著,且其远期脊椎矫正效果优于跨伤椎固定,适于临床应用。     

椎弓根钉技术在胸腰段椎管内肿瘤治疗中的应用

目的探讨经后路椎板切除摘除胸腰段椎管内肿瘤,同时行椎弓根螺钉内固定植骨融合后对提高脊柱的稳定性的临床疗效。方法采用后正中入路显微切除椎管肿瘤后应用椎弓根螺钉内固定植骨融合技术进行椎管重建12例,其中,肿瘤位于胸段7例,腰段5例。结果随访9个月~5年观察效果,X线平片显示手术区植骨全部骨性融合,脊柱稳定。结论采用后正中入路显微切除胸腰段椎管肿瘤后应用椎弓根螺钉内固定植骨融合技术,能够维持手术后脊柱的稳定性,防止远期后凸畸形的发生。     

椎弓根钉技术在胸腰椎结核治疗中的应用

目的探讨单纯应用椎弓根钉技术结合横突间或小关节突间植骨治疗胸腰椎结核的疗效。方法2007年7月．2013年2月赣州市第五人民医院（市传染病医院）治疗18例胸腰段脊柱结核患者。受累椎体均在2个或2个以下。且病灶相对局限,无大的流注脓肿。均采用单纯经后路椎弓根钉内固定、横突间或小关节突间植骨术式治疗。术后继续抗结核治疗12～18个月。结果随访9个月～5年观察效果,X线平片显示手术区植骨全部骨性融合,脊柱稳定。植骨融合时间为3-8个月,平均5个月。无结核复发。结论对于病灶较局限的胸腰段脊柱结核．采用单纯经后路椎弓根钉内固定、横突间或小关节突间植骨的手术方式能够维持手术后脊柱的稳定性,促进植骨融合、防止后凸畸形的发生。     

Luque手术在胸腰椎骨折的运用(附29例报告)

目的：探讨Luque手术治疗胸腰椎骨折。方法：对29例胸腰椎骨折患运用Luque手术治疗。结果：29例脊柱均恢复生理性弯曲度，frankel分级均有不同程度下降。结论：Luque手术可治疗胸腰椎骨折，只要正确掌握方法，注意钢棒预弯定型，注意推板下穿钢丝，术中同时减压，注意脊柱骨折复位，植骨融合，固定稳定可达到较佳疗效。     

一例犬脊柱骨折的诊断治疗

脊柱骨折或脱位是由脊柱椎骨和支持性软组织的外伤或病理性破裂而引起的，并导致脊髓和神经根受压。其中，胸腰段脊椎骨折或脱位在犬猫中最为常见，临床上可以采用保守治疗或手术治疗或综合治疗。分析一例犬T3腰椎骨折的手术治疗过程，通过采用骨水泥联合螺钉的手术方法来固定脊柱矫正患犬脊柱骨折处，术后配合药物治疗，使患犬患处愈合良好，并通过患犬后续恢复情况来评估脊柱神经受损的恢复情况。     

以手法为主治疗胸腰椎压缩性骨折125例临床小结

本文是作者们以中医理论为指导于1989~1991年间以手法为主治疗胸腰椎压缩性骨折125例的临床总结。本法结合内服中西药,外敷伤药,合理固定,动静结合,疗效快,功能恢复早,后遗症少。近期(2周),远期(1年以上)治疗有效率均达98%以上。     

胸腰椎爆裂性骨折的手术治疗

目的：探讨胸腰椎爆裂性骨折手术治疗方法的选择及疗效。方法回顾性分析我院2009年9月～2011年7月收治的27例胸腰椎爆裂性骨折病例临床资料,TLISS评分均大于4分,脊髓功能依Frankel分级：A级2例,B级4例,C级15例,D级6例。其中6例行后路椎弓根内固定＋注射型硫酸钙椎体成形,15例行后路椎板切除减压＋椎弓根内固定＋后外侧植骨融合,6例行后路经椎弓根半椎体切除减压、钛网和椎弓根内固定。观察术后症状改善及骨折复位稳定情况。结果：所有病例均顺利完成手术,术中术后禾出现严重并发症。经过4～24个月随访所有患者脊柱稳定性均得到满意的重建,未发生螺钉断裂或内固定松动。脊髓神经功能A级1例无改善,其余均有不同程度提高。结论：手术治疗胸腰椎爆裂性骨折安全有效,合理治疗方案的选择有赖于骨折的分型、骨折的稳定程度和脊髓神经损伤程度。’     

Markedness and its Interpretation in Language Use

Language markedness is a common phenomenon in languages, and is reflected from hearing, vision and sense, i.e. the variation in the three aspects such as phonology, morphology and semantics. This paper focuses on the interpretation of markedness in language use following the three perspectives, i.e. pragmatic interpretation, psychological interpretation and cognitive interpretation, with an aim to define the function of markedness.     

Characterization of Ordovician carbonate reservoirs, southeastern Saskatchewan, Canada

The discovery of the prolific Ordovician Red River reservoirs in 1995 in southeastern Saskatchewan was the catalyst for extensive exploration activity which resulted in the discovery of more than 15 new Red River pools. The best yields of Red River production to date have been from dolomite reservoirs. Understanding the processes of dolomitization is, therefore, crucial for the prediction of the connectivity, spatial distribution and heterogeneity of dolomite reservoirs.The Red River reservoirs in the Midale area consist of 3～4 thin dolomitized zones, with a total thickness of about 20 m, which occur at the top of the Yeoman Formation. Two types of replacement dolomite were recognized in the Red River reservoir: dolomitized burrow infills and dolomitized host matrix. The spatial distribution of dolomite suggests that burrowing organisms played an important role in facilitating the fluid flow in the backfilled sediments. This resulted in penecontemporaneous dolomitization of burrow infills by normal seawater. The dolomite in the host matrix is interpreted as having occurred at shallow burial by evaporitic seawater during precipitation of Lake Almar anhydrite that immediately overlies the Yeoman Formation. However, the low δ18O values of dolomited burrow infills (-5.9‰～ -7.8‰, PDB) and matrix dolomites (-6.6‰～ -8.1‰, avg. -7.4‰ PDB) compared to the estimated values for the late Ordovician marine dolomite could be attributed to modification and alteration of dolomite at higher temperatures during deeper burial, which could also be responsible for its 87Sr/86Sr ratios (0.7084～0.7088) that are higher than suggested for the late Ordovician seawaters (0.7078～0.7080). The trace amounts of saddle dolomite cement in the Red River carbonates are probably related to "cannibalization" of earlier replacement dolomite during the chemical compaction.

Language is a means of verbal communication. People use language to communicate with each other. In the society, no two speakers are exactly alike in the way of speaking. Some differences are due to age, gender, statue and personality. Above all, gender is one of the obvious reasons. The writer of this paper tries to describe the features of women＇s language from these perspectives： pronunciation, intonation, diction, subjects, grammar and discourse. From the discussion of the features of women＇s language, more attention should be paid to language use in social context. What＇s more, the linguistic phenomena in a speaking community can be understood more thoroughly.

低渗透非均质砂岩油藏启动压力梯度研究

理论推导与室内实验相结合,建立了低渗透非均质砂岩油藏启动压力梯度确定方法。首先借助油藏流场与电场相似的原理,推导了非均质砂岩油藏启动压力梯度计算公式。其次基于稳定流实验方法,建立了非均质砂岩油藏启动压力梯度测试方法。结果表明:低渗透非均质砂岩油藏的启动压力梯度确定遵循两个等效原则。平面非均质油藏的启动压力梯度等于各级渗透率段的启动压力梯度关于长度的加权平均;纵向非均质油藏的启动压力梯度等于各渗透率层的启动压力梯度关于渗透率与渗流面积乘积的加权平均。研究成果可用于有效指导低渗透非均质砂岩油藏的合理井距确定,促进该类油藏的高效开发。

海南岛地体及其毗邻陆缘晚中生代—新生代古地磁研究和构造演化

本文叙述了对海南岛及其毗邻大陆边缘白垩纪到第四纪地层岩石进行古地磁研究的全部工作过程。通过分析岩石中剩余磁矢量的磁偏角及磁倾角的变化,提出海南岛白垩纪以来经历的构造演化模式如下:早期伴随顺时针旋转而向南迁移,后期伴随逆时针转动并向北运移。联系该地区及邻区的地质、地球物理资料,对海南岛上述的构造地体运动提出以下认识:北部湾内早期有一拉张作用,主要是该作用使湾内地壳显著伸长减薄,形成北部湾盆地。从而导致了海南岛的早期构造运动,而海南岛后期的构造运动则主要是受南海海底扩张的影响。海南地体运动规律的阐明对于了解北部湾油气盆地的形成演化有重要的理论和实际意义。     

An Index mechanism for multi-scale view in spatial databases

There are numerous geometric objects stored in the spatial databases. An importance function in a spatial database is that users can browse the geometric objects as a map efficiently. Thus the spatial database should display the geometric objects users concern about swiftly onto the display window. This process includes two operations:retrieve data from database and then draw them onto screen. Accordingly, to improve the efficiency, we should try to reduce time of both retrieving object and displaying them. The former can be achieved with the aid of spatial index such as R-tree, the latter require to simplify the objects. Simplification means that objects are shown with sufficient but not with unnecessary detail which depend on the scale of browse. So the major problem is how to retrieve data at different detail level efficiently. This paper introduces the implementation of a multi-scale index in the spatial database SISP (Spatial Information Shared Platform) which is generalized from R-tree. The difference between the generalization and the R-tree lies on two facets: One is that every node and geometric object in the generalization is assigned with a importance value which denote the importance of them, and every vertex in the objects are assigned with a importance value,too. The importance value can be use to decide which data should be retrieve from disk in a query. The other difference is that geometric objects in the generalization are divided into one or more sub-blocks, and vertexes are total ordered by their importance value. With the help of the generalized R-tree, one can easily retrieve data at different detail levels.Some experiments are performed on real-life data to evaluate the performance of solutions that separately use normal spatial index and multi-scale spatial index. The results show that the solution using multi-scale index in SISP is satisfying.
