目的:探讨腹壁与会阴伤口疤痕子宫内膜异位症(EMT)的诊断和处理.方法:对我院1998年-2005年收治的14例腹壁与会阴伤口疤痕子宫内膜异位症进行回顾性分析.结果:剖宫术切口、阴道分娩侧切及会阴裂伤后形成的子宫内膜异位病灶多为子宫内膜种植所致.异位子宫内膜受卵巢激素影响,药物治疗效果差,应早期首选手术治疗.结论:避免子宫内膜组织脱落、遗留在伤口是预防和减少医源性EMT发生的关键.  相似文献

目的:探讨会阴、腹壁子宫内膜异位症的发生原因及治疗。方法:回顾性分析13例会阴、腹壁子宫内膜异位症的临床资料进行总结。结果:11例会阴子宫内膜异位症(占84.61%) ,1例为腹壁剖宫产切口子宫内膜异位症(占7.69%) ,1例为无创伤史的左侧腹直肌前鞘内膜异位症(占7.69%)。最早发病者为产后7a,最晚为产后10a,平均6.2a。12例在切口处有痛性结节 ,大小与月经周期有关 ,仅有1例无任何临床表现。结论:会阴、腹壁伤口子宫内膜异位症的发生重在预防 ,应尽量避免宫腔操作引起的子宫内膜医原性播散。治疗以手术切除异位病灶为主  相似文献

目的:观察围绝经期子宫内膜息肉(EP)行宫腔镜电切术(TCRP术)后口服小剂量米非司酮预防其复发、诱导绝经的临床疗效.方法:选择80例围绝经期(年龄45~55岁之间)EP患者,均接受宫腔镜下子宫内膜息肉电切术,术后病理诊断均为子宫内膜息肉者.将病例随机分为两组,观察组40例,术后1周口服米非司酮每日12.5 mg治疗,1日1次,共3个月.对照组40例,接受宫腔治疗但未服药.随访12个月,观察患者子宫内膜息肉的复发情况.结果:观察组40例患者有2例(2/40)内膜息肉复发,复发率为5%,而对照组则有10例(10/40)复发内膜息肉,复发率达到25%,两组之间的比较差异存在统计学意义(p<0.01);且观察组40例患者治疗期间均闭经,停药后29例绝经(72.5%).结论:小剂量米非司酮对围绝经期子宫内膜息肉行宫腔镜电切术后在预防复发、诱导绝经方面有显著疗效.  相似文献

剖宫产疤痕子宫内膜异位症是一种少见的医源性疾病,本研究旨在探讨其临床特征和预防方法.回顾分析本院2007年1月至2016年12月剖宫产疤痕子宫内膜异位症患者的临床资料,共有169例剖宫产疤痕子宫内膜异位症患者被纳入研究.患者平均年龄(32.0±3.9)岁,产次1~2次.潜伏期平均为(31.4±23.7)月,与患者年龄、产次等基本特征无明显相关性.80.5%的患者既往剖宫产为Pfannenstiel切口,19.5%的患者为正中纵形切口.统计学分析发现,Pfannenstiel切口发生子宫内膜异位症的潜伏期显著低于正中纵形切口(24.0 vs 36.0,P=0.005),并且多发性子宫内膜腺瘤患者潜伏期显著低于单个腺瘤患者(12.0 vs 24.0,P=0.034).子宫内膜腺瘤在剖宫产疤痕的两端更常见,在Pfannenstiel切口,83.4%的腺瘤位于两端;在正中纵形切口,84.8%的腺瘤位于两端.本研究提示剖宫产Pfannenstiel切口发生子宫内膜异位症的风险可能高于正中纵形切口,但仍需大样本前瞻性研究的验证.在剖宫产结束时彻底冲洗切口,尤其是脂肪层和筋膜层的两端对于预防剖宫产疤痕子宫内膜异位症十分重要.  相似文献

目的:探讨子宫内膜息肉的宫腔镜手术治疗及术后应用孕激素预防复发的临床效果.方法:选取入院治疗的520例子宫内膜息肉患者做为研究对象,按照治疗方法的差异分为研究组258例与对照组262例,两组患者均给予子宫内膜息肉的宫腔镜手术治疗,研究组患者术后应用孕激素药物干预治疗,对照组患者术后无药物干预治疗.比较两组患者治疗前后组织雌激素受体(ER)、孕激素受体(PR)、血管内皮生长因子(VEGF)表达变化情况.术后6个月、1年随访,统计患者复发情况,子宫内膜厚度及血红蛋白指标.结果:研究组患者治疗后ER、PR及VEGF表达数显著少于对照组患者,研究组患者的复发率低于对照组患者,具统计学差异(P0.05).两组患者术后6个月子宫内膜厚度及血红蛋白指标变化接近,无统计学差异(P0.05);研究组患者术后1年子宫内膜厚度及血红蛋白指标变化显著优于对照组患者,具统计学差异(P0.05).结论:临床应用宫腔镜手术治疗子宫内膜息肉,效果确切,术后给予孕激素治疗可以有效预防术后复发,值得临床推广应用.  相似文献

探讨电视腹腔镜术在诊治子宫内膜异位症的作用,使用电视腹腔镜配合内凝固术诊断及治疗子内膜异位症32例,术后随访2-30个月,疼痛缓解有效率95.6%,妊娠率57.9%,囊肿治疗有效率88.9%。电视腹腔镜术配合内凝固术在诊治盆腔子宫内膜异位症,尤其是盆腔腹膜内异症微小病灶中,有其独到之处,是一种好手段。  相似文献

目的:利用大鼠子宫内膜异位症动物模型,观察卵巢切除术对大鼠子宫内膜异位症的影响。方法:47只大鼠在建模后3周,第2次剖腹,观察移植物的生长情况和体积。将已成模大鼠随机分为两组:对照组(n=23)和卵巢切除术组(n=24),对卵巢切除术组大鼠行双侧卵巢切除,分别观察两组动物成模后2、4、6和8周移植物的生长情况和体积。结果:与建模后3周时相比,成模后2、4、6和8周对照组大鼠移植物的生长情况和体积基本保持一致,而卵巢切除术组大鼠异位子宫内膜出现萎缩退化并且体积明显缩小,术后4、6和8周与术后2周相比,作用更明显,百术后4、6和8周之间无明显差异,术后8周仍可见异位内膜组织存在。组织学检查,对照组大鼠异位子宫内膜呈增生状态,而卵巢切除术组呈萎缩状态。结论:卵巢切除术可使子宫内膜异位症大鼠异位子宫内膜逐渐萎缩退化,但未能使其消失。  相似文献

目的 分析小切口行甲状腺切除术的临产特点.方法 对采用小切口甲状腺切除术治疗的36例良性甲状腺疾病患者的临床资料进行回顾性分析.结果 36例患者手术均获得成功,术后1例出现饮水呛咳,在3天内恢复.随访2~35个月,未见复发.术后患者对颈部切口的愈合均较满意,手术效果理想.结论 小切口甲状腺切除术安全可行,创伤小,并发症少,不需要特殊器械,可以在基层医院推广应用.  相似文献

目的比较不同移植部位建立大鼠子宫内膜异位症模型效果的差别,以期建立重复性高的子宫内膜异位症造模方法。方法 SPF级成年雌性SD大鼠11只,10%水合氯醛麻醉后,手术取出带有内膜、肌层、浆膜子宫组织,用可吸收线分别将其缝于左右腹壁各1块,左右肠系膜靠近肠袢处各1块。术后常规饲养4周后检测移植内膜存活情况及生长情况。结果开腹后见移植内膜体积明显增大,呈暗红色或透亮的小囊泡视为造模成功。术后大鼠死亡1只,存活大鼠腹壁移植造模成功率为65%,肠系膜移植造模成功率为95%,χ~2检验P<0.05,两者差异有统计学意义。移植至腹壁的子宫内膜形成的囊肿体积(35.53±22.58)mm~3较肠系膜处(90.29±74.93)mm~3小,但两者比较差异无统计学意义。结论内膜移植至肠系膜更利于存活,且易形成较稳定的生长病灶,更利于子宫内膜异位症动物模型相关研究。  相似文献

目的探讨脂肪抽吸联合腹壁整形的临床疗效及安全性.方法 42例患者采用肿胀麻醉、负压吸引器行腹壁脂肪抽吸;在耻骨联合区域标记"W"形切口,沿腹肌筋膜浅层剥离,折叠缝合腹直肌前鞘,根据实际情况完成脐成形及切除多余皮瓣术,分层缝合腹壁.结果 42例患者切口均Ⅰ期愈合,无并发症发生.随访6~12个月,腹部对称柔软,切口瘢痕隐蔽,塑形效果满意.结论脂肪抽吸与腹壁整形联合应用临床疗效佳,安全可靠,值得推广.  相似文献

The Williston Basin is a significant petroleum province, containing oil production zones that include the Middle Cambrian to Lower Ordovician, Upper Ordovician, Middle Devonian, Upper Devonian and Mississippian and within the Jurassic and Cretaceous. The oils of the Williston Basin exhibit a wide range of geochemical characteristics defined as "oil families", although the geochemical signature of the Cambrian Deadwood Formation and Lower Ordovician Winnipeg reservoired oils does not match any "oil family". Despite their close stratigraphic proximity, it is evident that the oils of the Lower Palaeozoic within the Williston Basin are distinct. This suggests the presence of a new "oil family" within the Williston Basin. Diagnostic geochemical signatures occur in the gasoline range chromatograms, within saturate fraction gas chromatograms and biomarker fingerprints. However, some of the established criteria and cross-plots that are currently used to segregate oils into distinct genetic families within the basin do not always meet with success, particularly when applied to the Lower Palaeozoic oils of the Deadwood and Winnipeg Formation.  相似文献

Language is a means of verbal communication. People use language to communicate with each other. In the society, no two speakers are exactly alike in the way of speaking. Some differences are due to age, gender, statue and personality. Above all, gender is one of the obvious reasons. The writer of this paper tries to describe the features of women's language from these perspectives: pronunciation, intonation, diction, subjects, grammar and discourse. From the discussion of the features of women's language, more attention should be paid to language use in social context. The discovery of the prolific Ordovician Red River reservoirs in 1995 in southeastern Saskatchewan was the catalyst for extensive exploration activity which resulted in the discovery of more than 15 new Red River pools. The best yields of Red River production to date have been from dolomite reservoirs. Understanding the processes of dolomitization is, therefore, crucial for the prediction of the connectivity, spatial distribution and heterogeneity of dolomite reservoirs.The Red River reservoirs in the Midale area consist of 3~4 thin dolomitized zones, with a total thickness of about 20 m, which occur at the top of the Yeoman Formation. Two types of replacement dolomite were recognized in the Red River reservoir: dolomitized burrow infills and dolomitized host matrix. The spatial distribution of dolomite suggests that burrowing organisms played an important role in facilitating the fluid flow in the backfilled sediments. This resulted in penecontemporaneous dolomitization of burrow infills by normal seawater. The dolomite in the host matrix is interpreted as having occurred at shallow burial by evaporitic seawater during precipitation of Lake Almar anhydrite that immediately overlies the Yeoman Formation. However, the low δ18O values of dolomited burrow infills (-5.9‰~ -7.8‰, PDB) and matrix dolomites (-6.6‰~ -8.1‰, avg. -7.4‰ PDB) compared to the estimated values for the late Ordovician marine dolomite could be attributed to modification and alteration of dolomite at higher temperatures during deeper burial, which could also be responsible for its 87Sr/86Sr ratios (0.7084~0.7088) that are higher than suggested for the late Ordovician seawaters (0.7078~0.7080). The trace amounts of saddle dolomite cement in the Red River carbonates are probably related to "cannibalization" of earlier replacement dolomite during the chemical compaction.  相似文献

以AC-13级配为基础,将橡胶颗粒代替部分集料掺入混合料中,以低温弯曲试验为评价方法对不同橡胶颗粒掺量下沥青混合料的低温抗裂性进行研究,并引入应变能密度值对混合料的低温抗裂性进行综合评价.试验结果表明:橡胶颗粒沥青混合料试件的破坏微应变均超过2 300,满足冬寒区的技术指标;无论是否掺加橡胶颗粒,随着温度的下降,沥青混合料破坏时的最大弯拉强度增大,弯拉应变降低,劲度模量增大;弯曲应变能密度在胶粒掺量为1%左右时具有较大的弯曲应变能密度值,此时橡胶颗粒沥青混合料具有较好的低温抗裂性.  相似文献

理论推导与室内实验相结合,建立了低渗透非均质砂岩油藏启动压力梯度确定方法。首先借助油藏流场与电场相似的原理,推导了非均质砂岩油藏启动压力梯度计算公式。其次基于稳定流实验方法,建立了非均质砂岩油藏启动压力梯度测试方法。结果表明:低渗透非均质砂岩油藏的启动压力梯度确定遵循两个等效原则。平面非均质油藏的启动压力梯度等于各级渗透率段的启动压力梯度关于长度的加权平均;纵向非均质油藏的启动压力梯度等于各渗透率层的启动压力梯度关于渗透率与渗流面积乘积的加权平均。研究成果可用于有效指导低渗透非均质砂岩油藏的合理井距确定,促进该类油藏的高效开发。  相似文献

Quality traits in wheat (Triticum aestirum L.) were studied by quantitative trait locus (QTL) analysis in a recombinant inbred line (RIL) population, a set of 131 lines derived from Chuan 35050 × Shannong 483 cross (ChSh). Grains from RILs were assayed for 21 quality traits related to protein and starch. A total of 35 putative QTLs for 19 traits with a single QTL explaining 7.99-40.52% of phenotypic variations were detected on 10 chromosomes, 1D, 2A, 2D, 3B, 3D, 5A, 6A, 6B, 6D, and 7B. The additive effects of 30 QTLs were positive, contributed by Chuan 35050, the remaining 5 QTLs were negative with the additive effect contributed by Shannong 483. For protein traits, 15 QTLs were obtained and most of them were located on chromosomes 1 D, 3B and 6D, while 20 QTLs for starch traits were detected and most of them were located on chromosomes 3D, 6B and 7B. Only 7 QTLs for protein and starch traits were co-located in three regions on chromosomes 1D, 2A and 2D. These protein and starch trait QTLs showed a distinct distribution pattern in certain regions and chromosomes. Twenty-two QTLs were clustered in 6 regions of 5 chromosomes. Two QTL clusters for protein traits were located on chromosomes 1D and 3B, respectively, three clusters for starch traits on chromosomes 3D, 6B and 7B, and one cluster including protein and starch traits on chromosome 1D.  相似文献
