托珠单抗与依那西普治疗类风湿关节炎疗效观察

目的:回顾性比较托珠单抗与依那西普在治疗类风湿关节炎患者的疗效差异性.方法:收集给予托珠单抗或者依那西普联合改善病情抗风湿药(DMARDs)治疗的重度活动类风湿关节炎住院患者30例,分别记录两组患者治疗前、治疗后第4周、第12周时患者临床症状及实验室指标的变化情况,并进行对比分析.结果:两组患者治疗前、治疗后第4周、第12周时临床症状及实验室指标均有一定改善,且随治疗时间的延长,各项实验指标呈进行性下降趋势.治疗4周后托珠单抗组类风湿关节炎疾病活动度评分(DAS28评分)显著低于依那西普组,有统计学差异(2.5 vs 3.5,P=0.03);治疗12周后托珠单抗组类风湿因子水平显著低于依那西普组,有统计学差异(45.3 vs 105.8,P=0.04);治疗4周及12周后两组达到美国风湿病学会20(ACR20)标准、美国风湿病学会50(ACR50)标准的比例相当,差异无统计学意义.结论:托珠单抗治疗RA疗效确切,更快降低DAS 28水平,更显著降低类风湿因子水平.

概述了核因子-κB的生物学特性、核因子-κB的活化与在类风湿关节炎发病中的关系,以及目前临床治疗类风湿关节炎的药物,包括非甾体类抗炎药、糖皮质激素、慢作用抗风湿药、中药等调控核因子-κB活性的研究现状,为深入探讨类风湿关节炎的发病机制,研发新的抗类风湿关节炎药物提供研究依据.

<正>我校生命科学学院宋新强博士2018年获批国家自然科学基金项目:基于点击化学和蛋白质组学筛选和鉴定雷公藤治疗类风湿关节炎的靶点蛋白,项目批准号:U1804179.类风湿关节炎是一类常见的自身免疫性关节炎.对类风湿关节炎疗效最为突出的中药首推雷公藤,雷公藤甲素和雷公藤红素是雷公藤中最重要的两个活性成分.     

几种类风湿关节炎吸附剂的制备及吸附性能比较

制备了5种不同的类风湿关节炎吸附剂.用它们与类风湿关节炎病人血清作静态吸附实验,EL ISA检测吸附前后血清中RF s的浓度变化.实验发现,以纤维素球为载体,环氧氯丙烷活化后固定苯丙氨酸为配基的吸附剂综合吸附效果最好.该吸附剂对病人血清中IgGRF,IgM RF,IgARF的吸附中分别为39.3%,56.0%,28.1%.血液相容性实验结果显示,该吸附剂的血液相容性良好,能够满足临床应用要求.本文还研究了手臂对吸附剂吸附性能的影响,静态吸附实验结果显示PEG 4000为手臂的吸附剂吸附性能要高于以1,4-丁二醇二缩水甘油醚为手臂的吸附剂.     

一次性护理垫在类风湿关节炎患者蜡疗中的应用效果探析

探讨一次性护理垫在类风湿关节炎患者蜡疗中的应用效果。选择90例行蜡疗的类风湿关节炎患者,随机分为对照组和观察组各45例。对照组患者在行蜡疗时将制成的蜡饼直接敷于患者的疼痛部位;观察组患者在蜡饼外面给予一次性护理垫包裹。对照组和观察组总有效率分别为80.00%和95.56%;对照组和观察组的患者满意率分别为86.67%和100.00%;治疗后观察组的关节疼痛指数、关节压痛数、关节肿胀数、关节晨僵时间明显低于对照组。类风湿关节炎患者行蜡疗时使用一次性护理垫包裹,可显著增强蜡疗的治疗效果和提高患者满意度。该项技术具有较高的临床应用价值,值得推广应用。     

桂枝芍药知母汤加减治疗类风湿性关节炎60例

目的 :探讨桂枝芍药知母汤加减治疗类风湿关节炎的临床效果。方法 :运用桂枝芍药知母汤加减辨证分型治疗类风湿性关节炎患者60例 ,并进行临床观察和疗效分析。结果 :服药1疗程显效48例 ,好转9例 ,无效3例。结论 :类风湿关节炎为虚实寒热夹杂之证 ,解决好清热与温阳的矛盾 ,治疗上以通为要 ,则能达到理想的治疗效果

高通量筛选高特异性的检测类风湿关节炎的生物探针分子

从靶蛋白-配体特异性结合角度,采用蛋白折叠码(PFSC)技术在PDB数据库中高通量地筛选出与TNFA_HUMAN靶点区结合灵敏度高的分子作为检测类风湿关节炎的生物探针分子.结果显示,小分子307具有相对较高的灵敏度和较低的检出限,可以作为生物探针来检测类风湿关节炎.结果显示,蛋白指纹检索技术是一种快速、有效的蛋白质结构检索工具,通过对数据库中海量数据的筛选,可为重大疾病的检测试剂开发提供先导信息.     

来氟米特治疗类风湿关节炎的疗效评价

探讨来氟米特治疗类风湿关节炎的疗效和不良反应.选择80例类风湿关节炎患者,口服来氟米特20mg·d-1,疗程为12周,比较6周和12周的疗效.治疗6周时病情明显进步10例,进步24例,改善23例,总有效率74%.12周时明显进步28例,进步29例,改善16例,总有效率91.5%.13例发生不良反应(16.3%),表现为皮疹、转氨酶升高、白细胞下降、消化道不适,3例停药(撤药率3.8%).无严重不良反应.来氟米特治疗类风湿关节炎有效且相对安全.     

Markedness and its Interpretation in Language Use

Language markedness is a common phenomenon in languages, and is reflected from hearing, vision and sense, i.e. the variation in the three aspects such as phonology, morphology and semantics. This paper focuses on the interpretation of markedness in language use following the three perspectives, i.e. pragmatic interpretation, psychological interpretation and cognitive interpretation, with an aim to define the function of markedness.     

Lower Paleozoic oil relationships within Williston Basin, Canada

The Williston Basin is a significant petroleum province, containing oil production zones that include the Middle Cambrian to Lower Ordovician, Upper Ordovician, Middle Devonian, Upper Devonian and Mississippian and within the Jurassic and Cretaceous. The oils of the Williston Basin exhibit a wide range of geochemical characteristics defined as "oil families", although the geochemical signature of the Cambrian Deadwood Formation and Lower Ordovician Winnipeg reservoired oils does not match any "oil family". Despite their close stratigraphic proximity, it is evident that the oils of the Lower Palaeozoic within the Williston Basin are distinct. This suggests the presence of a new "oil family" within the Williston Basin. Diagnostic geochemical signatures occur in the gasoline range chromatograms, within saturate fraction gas chromatograms and biomarker fingerprints. However, some of the established criteria and cross-plots that are currently used to segregate oils into distinct genetic families within the basin do not always meet with success, particularly when applied to the Lower Palaeozoic oils of the Deadwood and Winnipeg Formation.

Characterization of Ordovician carbonate reservoirs, southeastern Saskatchewan, Canada

The discovery of the prolific Ordovician Red River reservoirs in 1995 in southeastern Saskatchewan was the catalyst for extensive exploration activity which resulted in the discovery of more than 15 new Red River pools. The best yields of Red River production to date have been from dolomite reservoirs. Understanding the processes of dolomitization is, therefore, crucial for the prediction of the connectivity, spatial distribution and heterogeneity of dolomite reservoirs.The Red River reservoirs in the Midale area consist of 3～4 thin dolomitized zones, with a total thickness of about 20 m, which occur at the top of the Yeoman Formation. Two types of replacement dolomite were recognized in the Red River reservoir: dolomitized burrow infills and dolomitized host matrix. The spatial distribution of dolomite suggests that burrowing organisms played an important role in facilitating the fluid flow in the backfilled sediments. This resulted in penecontemporaneous dolomitization of burrow infills by normal seawater. The dolomite in the host matrix is interpreted as having occurred at shallow burial by evaporitic seawater during precipitation of Lake Almar anhydrite that immediately overlies the Yeoman Formation. However, the low δ18O values of dolomited burrow infills (-5.9‰～ -7.8‰, PDB) and matrix dolomites (-6.6‰～ -8.1‰, avg. -7.4‰ PDB) compared to the estimated values for the late Ordovician marine dolomite could be attributed to modification and alteration of dolomite at higher temperatures during deeper burial, which could also be responsible for its 87Sr/86Sr ratios (0.7084～0.7088) that are higher than suggested for the late Ordovician seawaters (0.7078～0.7080). The trace amounts of saddle dolomite cement in the Red River carbonates are probably related to "cannibalization" of earlier replacement dolomite during the chemical compaction.

Some differences are due to age, gender, statue and personality. Above all, gender is one of the obvious reasons. The writer of this paper tries to describe the features of women＇s language from these perspectives： pronunciation, intonation, diction, subjects, grammar and discourse. From the discussion of the features of women＇s language, more attention should be paid to language use in social context. What＇s more, the linguistic phenomena in a speaking community can be understood more thoroughly.     

Design and analysis of generic LBS application developing platform

Location based services is promising due to its novel working style and contents.A software platform is proposed to provide application programs of typical location based services and support new applications developing efficiently. The analysis shows that this scheme is easy implemented, low cost and adapt to all kinds of mobile nework system.     

橡胶颗粒沥青混合料低温抗裂性试验研究

以AC-13级配为基础,将橡胶颗粒代替部分集料掺入混合料中,以低温弯曲试验为评价方法对不同橡胶颗粒掺量下沥青混合料的低温抗裂性进行研究,并引入应变能密度值对混合料的低温抗裂性进行综合评价.试验结果表明:橡胶颗粒沥青混合料试件的破坏微应变均超过2 300,满足冬寒区的技术指标;无论是否掺加橡胶颗粒,随着温度的下降,沥青混合料破坏时的最大弯拉强度增大,弯拉应变降低,劲度模量增大;弯曲应变能密度在胶粒掺量为1%左右时具有较大的弯曲应变能密度值,此时橡胶颗粒沥青混合料具有较好的低温抗裂性.

低渗透非均质砂岩油藏启动压力梯度研究

理论推导与室内实验相结合,建立了低渗透非均质砂岩油藏启动压力梯度确定方法。首先借助油藏流场与电场相似的原理,推导了非均质砂岩油藏启动压力梯度计算公式。其次基于稳定流实验方法,建立了非均质砂岩油藏启动压力梯度测试方法。结果表明:低渗透非均质砂岩油藏的启动压力梯度确定遵循两个等效原则。平面非均质油藏的启动压力梯度等于各级渗透率段的启动压力梯度关于长度的加权平均;纵向非均质油藏的启动压力梯度等于各渗透率层的启动压力梯度关于渗透率与渗流面积乘积的加权平均。研究成果可用于有效指导低渗透非均质砂岩油藏的合理井距确定,促进该类油藏的高效开发。
