早期神经、血管松解术治疗Volkmann挛缩疗效分析

目的探讨Volkmann挛缩临床分期及早期神经、血管松解术治疗Volkmann挛缩的疗效.方法将行神经、血管松解术的38例Volkmann挛缩(挛缩期)病例分为挛缩进行期A组(19例)和挛缩恢复期B组(19例),术后随访6个月~15 a.结果术后总优良率为76.32%.A组和B组病例术后功能评价优良率分别为94.74%和57.89%,A组的功能评定优良率较B组有显著性差异(P0.05),A组术后功能恢复优良率明显高于B组.结论Volkmann挛缩(挛缩期)越早行神经、血管松解术疗效越好.

Clinical Analysis on the Curative Effective of Early Nerve and Vascular Decompression on Volkmann Ischemic Contracture in Forearm

Objective To discuss the clinical stage of Volkmann ischemic contracture in forearm,and to explore the clinical curative effective of nerve and vascular decompression on Volkmann ischemic contracture at the contracture stage. Method A total of 38 patients who underwent never and vascular decompression were divided into two groups:group A with 19 cases ( contracture stage:24 hours ~1 month ) and group B with 19 cases ( restoration stage:1 month~6 months) . All the patients were followed up 6 months to 15 years. Results After operation,the total excellent and good rate was 76. 32%,the excellent and good rate of group A was 94. 74% and the excellent and good rate of group B was 57 . 89%. The difference between the two groups was significant (P<0. 05),which indicated that the functional recoveries of the patients in group A were much better than those in group B. Conclusion In the Volkmann contracture stage,the earlier the nerve and vascular decompression is carried out,the curative effective is better.
