应用全结肠术中灌洗治疗低位大肠梗阻

目的评价术中封闭式结肠灌洗术在低位大肠癌性梗阻Ⅰ期根治性切除的运用效果。方法1999年1月至2002年12月收治的计17例低位癌性肠梗阻的患者,进行Ⅰ期根治手术时术中封闭式全结肠灌洗,观察术后有无并发肠瘘、切口感染等并发症。结果达到了清洁肠道、减少污染、改善肠壁血运效果,保证了17例手术全部Ⅰ期愈合。本组病例无手术死亡,无并发肠瘘,切口感染3例(17.6%)。结论术中结肠灌洗术对降低低位肠梗阻根治性切除术的并发症有效;只要术中结肠灌洗彻底,低位大肠癌性肠梗阻Ⅰ期根治性切除术是安全可行的。

On-table colonic lavage appilcation to primary resection and an astomasis of carcinous obstruction of the large bowel

Objective: To investigate the clinical value of On-table colonic lavage in primary resection and an anastomasis for carcinous obstruction of the the large bowel.Methods: A retrospective analysis was made that is s series of 17 patients with colonic obstruction is presented in which a primary anastomosis was done after an on-table-lavage during the past 4 years.Results: All patients were recovered and there was no perioperative mortality in this series.There was no clinical anastomotic leak in patients undergoing primary anastomosis.Only three patients underwent incisions` infections(17.6%).Conclusion: The primary resection and an anastomosis with on-table colonic lavage in the large bowel carcinous obstruction is a safe procedure.
