老年股骨粗隆间骨折手术固定方法的探讨

目的:探讨用不同手术固定方法治疗老年股骨粗隆间骨折的疗效.方法:采用动力髋螺针、单臂外固定架固定、Gamma钉固定老年股骨粗隆间骨折89例,早期功能练习.结果:随访83例 (93.2%),随访时间6～30个月.骨折均愈合,骨愈合3～7个月.无髋内翻、肢体短缩畸形.优:81例(9 2%),良6例(6.7%),可:2例(2.2%).结论:根据老年股骨粗隆间骨折稳定程度,患者一般状态和有无合并内科疾病,选择不同固定方法,可降低髋内翻等并发症及病残死率.

Discussion on Operative Fixation Methods Treating Senile Intertrochanter Fracture of Femur

Objective:To discuss the curative effect of different operative fixation methods treating senile intertrochanter fracture of femur.Methods:89 cases of senile intertrochanter fracture of femur were treated with dynamic hip bone screw,Gamma nail and single-arm external fixation bracket respectively.All patients did dirigation early.Results:83 cases were followed up,whose follow-up duration ranged from 6m to 30m.All patients' fracture healed,whose duration of fracture healing ranged from 3m to 7m.Coxa vara and shortening deformity of extremity didn't happen to any patient.Curative effect of 81 cases was excellent,that of 6 cases effective and that of 2 cases passable.Conclusion:Different fixation methods should be applied according to the stability of fracture,general state and medical complications of senile patients,which can reduce the incidence of complications,invalidity and fatality,such as coxa vara and so on.
