左肝管定向穿刺胆管造影术

目的:报告左肝管定向穿刺胆管造影术的方法。方法:本组70例行左肝管定向穿刺胆管造影术(其中22例肝门胆管癌),61例穿刺成功,成功率87.14％,手术证实无出血及胆漏并发症。结果:总成功率98.57％,全组无住院死亡病例。结论:左肝管定向穿刺胆管造影术可作为PTC的首选方法,能有效防止出血和胆漏,但不能完全替代传统的右侧径路PTC,左肝管定向穿刺PTC有助于肝门胆管癌的诊断和分型诊断,应作为首选方法。

Performing the Percutaneous Transhepatic Cholangiography (PTC) by Puncturing the Left Hepatic Duct

To study the value of the percutaneous transhepatic cholangiography (PTC) by puncturing the left hepatic duct. Methods: 70 cases were treated with PTC by puncturing the left duct (22 cases were the hepatic hilar cholangiocarcinoma ) , 6 1 cases were successfully punctured (a successful ratio is 87.14%). Results: The complications, such as bleeding and bile leaking, were not found. No death cases had occurred in this study. Conclusion: The percutaneous transhepatic cholangiography performed by puncturing the left hepatic duct should be choiced primarily because there is not bleeding and bile leaking complications, but it cannot completely replace traditional PTC. This method should be recommended as the first choice method for the diagnosis and Bismuth type of hepatic hilar cholangiocarcinoma.
