| 摘 要： | 研究医疗体制改革中的医疗支付费制度，将按人预付费制与实报实销付费制相结合设计出一种混合付费制，并利用委托代理理论讨论了在单阶段和多阶段情况下最优合同机制所应满足的条件．在单阶段情况下，混合付费体系在保证医疗供给方利益的条件下实现了对医疗费用的制约．在多阶段情况下，混合付费体系中的医疗质量信号机制可以在医疗质量与制约医疗费用之间取得折衷．

| Abstract: | The payment system is the center content of medical system. The capitation system was combined with the fee\|for\|service system to found a mixture payment system. By way of principal\|agent theory the conditions for the optimal contract mechanism to be fulfilled in single and multi period were discussed. In single period, the mixture payment system can hurdle the expenditure under the condition of keeping revenues of the providers, while in multi period, the mixture payment system provides a signal of medical quality in multi period, which courses a trade off between medical expenditure and quality. |
