摘    要:目的:探讨胆总管囊肿切除后改良胆道重建术。方法 :分析8例胆总管囊肿切除术后 ,将左、右肝管汇合处纵向切开外翻 ,行Y形空肠臂或间置空肠套入吻合 ,再行肠肠吻合活瓣成形术。结果 :采用本手术方法治疗胆总管囊肿8例 ,手术经过顺利7例 ,并发肺炎1例 ,经药物治愈 ,8例均经随访 ,平均随访时间7.8a。全部病例均无胆管炎发生。经B超检查未见肝内胆管扩张、结石、肝硬变及癌肿等并发症。经钡餐检查8例均未见消化性溃疡 ,亦无钡剂返流入胆道。结论 :胆总管囊肿切除术后采用本术式 ,只要仔细操作 ,术后效果较满意

Abstract:Objective:To study cystectomy and modified biliary tract rebuilding operation.Methods:Analysis the curative effect of 8 cases cystectomy cyst that were operated with cystectomy and Rouxy- Y hepaticojejunostomy plus antireflux valvoplasty or hepaticojejunoduodenostomy plus antireflux valvoplasty.Results:8 cases cystectomy cyst were operated with this operative procedures.7 cases were operated easily.1 case occured complicated pulm onitis but was cured by medicine.8 cases all were followed- up and the average time was 7.8 years.All cases didn't complicate cholangitis.No dilatation or duct stone of intrahepatic bile duct,cirrhosis and cancer were discovered in ultrasonography.No peptic ulcer in barium meal examination and no contrast- medium backflow to bile duct.Conclusions:The curative effect is quite satisfactory with this method when choledochal cyst was operated.
