U型交锁缝合式胰肠吻合在胰十二指肠切除术中的应用

目的对胰十二指肠切除术中胰肠吻合口缝合技术进行改进,以提高预防胰瘘的发生几率。方法对20例胰十二指肠切除术患者采用胰空肠端端套入及U型交锁缝合式胰肠吻合,对照分析采用传统胰肠吻合的胰十二指肠切除患者18例。结果胰空肠端端套入并U型交锁缝合式胰肠吻合时间平均20 min,手术时间大大缩短,与没有进行胰肠吻合口瘘患者相比,两组在平均手术时间、住院天数、胰瘘、腹腔出血、胃肠吻合口出血的差异有统计学意义(P0.05)。结论胰空肠端端套入并U型交锁缝合式胰肠吻合术,操作简单、减少了手术时间及术后并发症,明显改善了胰肠吻合缝合技术。

APPLICATION OF U INTERLOCKING SUTURE PANCREATICOJEJUNOSTOMY IN PANCREATICODUODENECTOMY

Objective: Pancreaticoduodenectomy in pancreatic anastomotic suture technique is improved to increase the probability of the occurrence of pancreatic fistula prevention. Methods:20 cases of pancreaticoduodenectomy patients were treated with pancreatic jejunum end to end and set into the U-shaped interlocking suture pancreaticojejunostomy, comparative analysis with traditional pancreaticoduodenectomy 18 patients. Results: The time of pancreaticojejunostomy end to end and setting into a U-shaped interlocking suture pancreaticojejunostomy was 20 minutes, the operation time is greatly reduced, no pancreatic anastomotic fistula patients, two groups in mean operative time, length of stay, pancreatic fistula, abdominal bleeding, anastomotic gastrointestinal bleeding difference was statistically significant (P < 0.05). Conclusion: Pancreas and jejunum end to end and setting into the U-shaped interlocking suture pancreaticojejunostomy, is easy to operate, reduces surgery time and postoperative complications and significantly improves the pancreaticojejunostomy suture technique.
