摘    要:目的:观察有创动脉压监测(IBP)在危重病人中的波形变化和临床意义.方法:选择60例病人,急重症30例(观察组)和择期手术30例(对照组).麻醉方法相同,均采用左桡动脉插管监测动脉压、观察波形变化特点并记录.结果:观察组血压波动大,波形有低平波11例,圆钝波6例,不规则波3例,高尖波10例.对照组血压波动小,波形呈陡直,波幅高22例,高耸规则波8例.结论:急重症病人或低血压病人桡动脉监测,不仅可以动态监测血压变化,同时也可从波形中预测心功能和血容量等的改变,对指导临床治疗具有重要意义.

Abstract:Objective:To observe the changes of waveform of invasive blood pressure(IBP) monitoring in critical patients and its clinical significance.Methods: 60 patients were divided into observation group (30 emergency and severe cases) and control group (30 patients to selective operation).All patients underwent the same anesthesia and their arterial blood pressure was monitored continuously by left radial arterial cannulation,and the changes of waveform of BP was observed and recorded.Results:Patients' BP in observation group fluctuated ob viously.The waveform of BP was low and flat in 11 eases,round and blunt in 6 cases,irregular in 3 cases,and high and sharp in 10 cases.The fluctuation of patients' BP in control group was not evident.The waveform of BP was steep with high amplitude in 22 cases,and topping and regular in 8 cases.Conclusion:Invasive radial arterial blood pressure monitoring can not only monitor the changes of BP of emergency and severe patients or patients with low BP but also forecast the change of heart function and blood volume,which is important in guiding the clinical treatment.
