定制化铰链内滚动式双动人工半膝关节假体的设计与研究

摘    要:针对半膝关节股骨髁假体对胫骨平台的磨损,提出一种基于计算机辅助设计(CAD)和先进机械制造技术的新型定制化双动半膝关节假体,并对假体进行离体实验研究.以Mimics/Geomagic/Pro-E等工程软件为核心的技术路线,借鉴双极人工股骨头的设计原理,在定制化半膝关节股骨髁假体内部设计双动结构.通过先进的机械制造技术得到金属假体实体;分别对植入假体前、植入双动半膝假体及全膝关节假体的三组离体膝关节进行动态X线透视观察,对三组离体膝关节的运动模式进行对比研究.结果假体内部的双动结构能有效的降低假体与胫骨的摩擦频率,且双动半膝关节假体比全膝关节假体更接近于正常膝关节的运动模式.说明该假体为非限制型半膝关节假体,在保留膝关节一侧骨骺生长能力的同时,降低了假体与胫骨平台之间的磨损,为半膝关节假体置换的概念注入新的活力.假体只需定制具有个体化关节面的下部件,其余部件选用预制的标准部件,这样有利于缩短定制周期,保证质量和降低费用.
