小儿急性肾功能衰竭死亡原因分析

目的 :分析小儿急性肾功能衰竭 (ARF)死亡原因。方法 :收集 ARF患儿的临床资料 6 5例 ,其中死亡 13例 ,用统计学方法对与 ARF死亡相关的因素进行分析 ,明确死亡原因。结果 :原发病危重、高危症状持续存在、血尿素氮异常升高、血清钾迅速增加、血清钠持续低值、内生肌酐清除率显著降低以及多种尿酶活性持续并行升高、血清 L PO异常升高、血 SOD持续下降者死亡率高。本组病例死亡率 2 0 % ,较国内外有关文献报告的七、八十年代死亡率为低。结论 :重视小儿 ARF病情特点、完善实验室检测方法 ,对指导治疗、降低死亡率意义重大。

An Analysis of Cause of Death in Children with Acute Renal Failure

Objective: To analyse the cause of death in children with acute renal failure(ARF). Methods: The authors reviewed the clinical cases of 65 children with ARF 13 cases of the series died. To analyse the relevant factors of the death from ARF, and to define the cause of death by satistic methods. Results: The death of ARF was related to serious primary dieases, continuous development of high risk symptoms, persistent high levels of blood urea nitrogen(BUN), serum potassium(Ska), three or four kinds of urinary enzymes and blood lipid peroxides(LPO), as well as markedly low levels of serum sodium(SNa), endogenous crestinine clearance rate(Ccr) and blood superoxide dismutase(SOD). The death rate of this group was 20%. It was lower than that of the seventies or eighties. Conclusion:It is very important to value the clinical characteristies of ARF and perfect the mothods of laboratory for guiding treatment and reducing the death rate.
