妊娠合并血小板减少的临床分析

目的探讨妊娠合并血小板减少的病因及围产期的处理方法。方法分析200901-201101期间大同市第三人民医院92例妊娠合并血小板减少患者的临床资料。结果 92例患者中妊娠相关性血小板减少(PAT)45例,妊娠期高血压疾病(HCDP)所致血小板减少22例,妊娠合并巨幼红细胞贫血(MACP 11例,特发性血小板减少性紫癜(ITP)9例,再生障碍性贫血(AA)3例,系统性红斑狼疮1例。阴道分娩34例,剖宫产52例,产后出血8例,经处理后效果良好。结论该病是由多种内科合并症及妊娠并发症引起的一种围产期常见病,处理不当,将危及母婴生命,应针对病因进行个体化处理。分娩方式视血小板情况及有无产科手术指征而定。

Clincal Analysis on Pregnancy with Thrombopenia

Objective To explore the etiopathogenesis and optimal management of pregnant women with thrombocytopenia.Methods 92 cases of pregnant women with gynecological thrombocytopenia with the clinical data were stuied retrospectively from 2009 to 2011 in the -associated thrombocytopenia(PAT) 45 cases;gestational hypertension induced thrombocytopenia in 22 cases;megaloblastic anemia complicating pregnancy(MACPn 11 cases;idiopathic thrombocytopenic purpura(ITP) in 9cases;aregeneratory anemia in 3 cases;systematialupus erythematatosus in 1 cases.34 cases of vaginal delivery;52 cases of cesarean section.postpartum hemorrhage 8 cases;treated well.Conclusions Pregnancy complicated by the variety of platelet reduce medical complications arising from pregnancy complications and perinatal is acommon disease,handled not properly,will threaten maternal-fetal life,the individual should be a cause for treatment.Delivery way is depended on platelets and surgical indications.
