54例中耳炎性疾患的CT表现与手术病理对照

目的:加深对不同性质中耳炎的高分辨率CT表现的认识。方法:搜集经手术病理证实的54例(55侧)中耳炎的资料,详细分析其CT表现并与手术病理结果对照。结果:54例(55侧)中耳炎中,单纯胆脂瘤者15例(16侧),胆脂瘤伴肉芽者24例,单纯肉芽肿者14例,前两者在CT表现上相似;而单纯胆脂瘤和胆脂瘤伴肉芽者,在听小骨、鼓窦入口和外耳道破坏方面均显著多于单纯肉芽肿者(P<0.05)。中耳炎患者患侧乳突呈硬化型或板障型者53耳,占96.36%,而对侧乳突呈硬化型或板障型者32耳,占58.18%。结论:CT发现听小骨和鼓窦入口破坏多提示胆脂瘤;双侧乳突发育可不一致。

Contrast observation between CT findings and operation pathology in chronic otitis

Aim: To improve the cognition of the findings in media otitis with high resolution CT scan.Methods: CT findings in 54 cases(55 ears) of the media otitis were analyzed retrospectively,and a comparison with operation and pathological findings was made.Results: In 54 cases(55 ear) of media otitis,there were 15 cases(16 ears) of cholesteatoma,24 cases of cholesteatoma mixed with granuloma and 14 cases of pure granuloma.The CT findings of the first two type were similar,while there was statistical difference in ditus ad antrum enlargement,ossicular chain erosion and external auditory canal destruction between the first two type and the latter one(P<0.05).Fifty-three ears(96.36%) were constrictive or diploetic type mastoid in the ailing side while 32 ears in the opposite sides.Conclusion:The destruction of ditus ad antrum and ossicles is the due to the diagnosis of cholesteatoma.The development of the two side mastoid can be imcompatible
