动态血压变异性与高血压伴冠心病的关系探讨

摘    要:目的 :探讨动态血压变异性(ABPV)与高血压伴冠心病的关系。方法 :采用24h动态血压监测(24hABPM)仪 ,对46例原发性高血压(EH)伴冠心病(CHD)和60例EH无并发症患者及45例正常人进行24hABPM ,以24h、白天、夜间血压均值 ,血压变异性为参数指标。结果:三组24h、白天、夜间血压均值相互比较有显著的统计学意义(P<0 01)。血压变异性 ,EH伴CHD与高血压无并发症组及正常对照组比较有显著的统计学意义(P<0 01) ,而高血压无并发症组与正常对照组比较除白天收缩压变异性有明显差异性外 ,其余无统计学意义。结论:高血压伴冠心病与动态血压变异性及动态血压均值有密切的关系。血压变异性可作为一项有别于平均血压的独立的预测心血管病变及自主神经受损程度的敏感指标。

Abstract:Objective:To study on the relationship of hypertension with c oronary heart disease and ambulatory blood pressure variability.Methods:46cases of essential hypertension with coronary heart disease,60cases of essential hypertension without complication and45cases of normal blood pressure were performed24-hour ABPMby using24-hour ambulatory blood pressure monitoring.Blood pressure mean values of24-hour,day,night and blood pressure variability were as the parameter index.Results:To compare the three groups'blood pressure mean values of24-hour,day and night,they had striking statistcally meaning(P<0.01).The blood pressure variability of essential hypertension with coronary heart disease group had striking statistically meaning as compared with essential hypertension without complicationgroup and normal blood pressure group(P<0.01).Essential hypertension without complication group had no significant different as compared with normal blood pressure group except day SSD.Conclusion:They have a close relationship among hypertension with coronary heart disease,ambulatory blood pressure variability and ambulatory blood pressure mean value.Blood pressure variability can be used as a sensitive marker to prognosticate independently cardiovascular disease and the damage degree of parasympathetic system.It is distinguish from mean blood pressure.
