甲亢术中术后气管痉挛17例抢救体会

目的：探讨甲亢行甲状腺次全切除术中术后发生气管痉挛的紧急处理．方法：从1978-03-2004-10内蒙古民族大学附属医院普外科共收治手术治疗甲亢患者2050例，术中术后发生气管痉挛窒息17例，其中气管软化8例，气管痉挛5例，气管软化合并气管痉挛3例，两侧喉返神经损伤1例．结果：本组17例中气管切开10例，气管插管7例，本组抢救成功16例，死亡1例．结论：气管痉挛是甲亢行甲状腺次全切除术术中术后发生的严重并发症，一旦出现气管痉挛术者要迅速判断气管痉挛的原因及时选择正确的紧急处理方法．

Experience with 17 Cases of Spasm of Trachea during and after Subtotal Thyroidectomy

Objective:To find out how to promptly deal with the spasm of trachea during and after the operation for hyperthyroidism.Methods:From March 1978 to October 2004,there were 2050 patients with hyperthyroidism who received operation in the surgical department of the hospital.17 Patients suffered the spasm of trachea during and after the operation,among which 8 patients had tracheomalacia,5 patients had spasm of trachea,3 patients had both tracheomalacia and spasm and 1 patient had injury of recurrent laryngeal nerve on both sides.Results:In 17 cases,we performed 10 cases of incision of trachea and 7 cases of tracheal intubation.16 patients were rescued successfully but one died.Conclusion:Spasm of trachea is the serious complication during and after subtotal thyroidectomy.Once it occurs,the operator has to find the reason quickly and choose correct method to deal with in time.
