28例颅脑嗜酸性肉芽肿的临床分析

目的:探讨嗜酸性肉芽肿的临床表现、影像学特点、诊断及手术治疗。方法:对我科近5年经手术和病理证实的28例颅脑嗜酸性肉芽肿的临床资料进行回顾性分析。结果:26例为颅骨嗜酸性肉芽肿,2例为原发于下丘脑垂体柄的嗜酸性肉芽肿。仅1例为手术活检,其余病例均行病灶全切,并将受累硬膜恰当处理。随访3个月至4年,1例多发颅骨嗜酸性肉芽肿术后4个月出现新病灶,其余病例未见复发。结论:嗜酸性肉芽肿常出现硬膜浸润,彻底清除病灶及对硬膜的恰当处理是手术治疗的关键。

Clinical analysis of 28 cases of eosinophilic granuloma in skull and brain

Aim: To discuss the clinical symptoms,the eikonic characters,the diagnosis and the treatment of the eosinophilic granuloma.Methods: A retrospective analysis was conducted in 28 cases which were confirmed by operation and pathology during the late 5 years in our department.Results: The results showed 26 cases were skull eosinophilic granuloma and 2 cases were intracranial eosinophilic granuloma involve hypothalamus and stalk hypophysial.Biopsy was taken in 1 case,totally removed were treated in the others,and the affected dura matter was appropriately dealt with. Twenty-one cases were followed up for 3 months to 4 years,except for 1 case which was polyostotic eosinophilic granuloma was found recurrence in other place after operation in 4 months.Conclusion: eosinophilic granuloma frequently diffusely infiltrating dura matter,completely remove the lesion and appropriately deal with the dura matter are the key points of the operation.
