超声造影在肝动脉化疗栓塞术 联合微波消融治疗肝癌效果评估中的应用

目的通过对比研究超声造影(CEUS)与多层螺旋CT(MDCT)在肝癌行肝动脉化疗栓塞术(TACE)联合微波消融后的影像,探讨超声造影在肝癌TACE后效果评估中临床价值.方法对82例肝癌患者行TACE联合微波消融术进行治疗,术后4周及24周分别行CEUS及MDCT检查,将检查结果与病理确诊结果进行比较分析.结果术后4周CEUS的评价敏感性为100%,准确率为69.5%;MDCT敏感性为78.1%,准确率为91.5%.术后24周CEUS的评价敏感性为100%,准确率为97.7%;MDCT敏感性为100%,准确率为97.1%.结论与MDCT相比,CEUS可在术后较短时间内及时发现复发病灶,有利于及时进行针对性治疗.

Application of Contrast-Enhanced Ultrasonography Combined with Microwave Ablation in the Evaluation of Hepatocellular Carcinoma Effect of Transcatheter Arterial Chemoembolization

Objective The study of contrast-enhanced ultrasound and multi-detector computed tomography ( MDCT) in the image of hepatocellular carcinoma after the therapy of TACE combined with microwave ablation was compared,the clinical value of contrast-enhanced ultrasound in the evaluation of hepatocellular carcinoma after TACE was investigated. Method 82 Cases with hepatocellular carcinoma undergoing TACE combined with microwave ablation therapy were examined by contrast-enhanced ultrasound and MDCT 4 weeks and 24 weeks after the therapy. The two examination results and pathologic findings were compared and analyzed. Results The sensitivity and accuracy of CEUS 4 weeks after therapy were 100% and 69. 5%,the sensitivity and accuracy of CEUS after 4 weeks therapy were 78. 1% and 91. 5%,respectively. The sensitivity and accuracy of both CEUS and MDCT after 24 weeks therapy were 100% and 97. 1%,respectively. Conclusion Compared with MDCT, CEUS can detect the recurrence of tumors earlier,within a short time after the therapy,which is applied to the evaluation of hepatocellular carcinoma.
