口腔正畸治疗中RGp检出率变化及对牙龈炎发生的影响

摘    要:目的探讨口腔正畸治疗中牙龈蛋白酶R(RGp)检出率变化及对牙龈炎发生的影响.方法选取接受正畸治疗的患者72例,分别于矫治器戴入前、后60 d取牙龈沟液为研究样本,采用16S r DNA PCR检测样本中牙龈卟啉单胞菌(P.g),取P.g阳性标本进一步检测RGp;同时用牙周探针检测牙龈指数,分析P.g,RGp的检出率与牙龈指数的相关性.结果矫治器戴入后60 d时,P.g,RGp检出率均明显高于戴入前,差异具有统计学意义(P0.05);RGp与P.g检出率之间呈明显正相关,矫治器戴入后60 d牙龈指数Ⅰ级的P.g检出率明显高于戴入前牙龈指数0级的P.g检出率,差异具有显著统计学意义(P0.01);戴入后牙龈指数Ⅰ级的P.g检出率明显高于0级的P.g检出率,差异具有统计学意义(P0.05).P.g阳性患者中,戴入后60 d牙龈指数Ⅰ级的RGp检出率明显高于戴入前牙龈指数0级的RGp检出率,差异具有显著统计学意义(P0.01);戴入后牙龈指数Ⅰ级的RGp检出率明显高于0级的RGp检出率,差异具有统计学意义(P0.05).RGp,P.g检出率与牙龈指数分级之间呈明显正相关.结论 P.g感染和RGp水平提高会引发口腔正畸治疗中牙龈炎性反应,应在矫治器戴入后给予干预性治疗.

Abstract:ObjectiveTo investigate changes in the detection rate of gingipain R (RGp) in orthodontic treatment and its effects on gingivitis.Method72 patients undergoing orthodontic treatment were selected.The patients' gingival crevicular fluid samples were taken on 60 days before and after the application of the appliance as the research samples,16S rDNA PCR was applied to test Porphyromonas gingivalis (P.g) in the samples,and P.g-positive samples were used for the further detection of RGp;at the same time,the gingival indexes were detected with a periodontal probe,and the correlation of gingival index with the change in the detection rate of P.g and RGp was analyzed.ResultsOn 60 days after the application of the appliance,the detection rates of P.g and RGp were significantly higher than those before it,with a statistically significant difference (P<0.05);RGp and P.g showed a positive correlation in the detection rate.P.g detection rate of gingival index (Grade I) on 60 days after the application of the appliance was significantly higher than that before putting into the gingival index (Grade 0),showing a statistically significant difference (P<0.01);The P.g detection rate of gingival index (Grade I) after the application of the appliance was significantly higher than that before putting into the gingival index (Grade 0),showing a statistically significant difference (P<0.05).In P.g-positive patients,the RGp detection rate of gingival index (Grade I) on day 60 after the application of the appliance was significantly higher than that (Grade 0) before it (P<0.01);the RGp detection rate of gingival index (Grade I) after the application of the appliance was significantly higher than that (Grade 0) before it (P<0.05).The detection rate of P.g and RGp was positively correlated with the grading of the gingival index.ConclusionThe P.g infection and the level of RGp can cause gingivitis in the orthodontic treatment,and some interventions should be given to the patients after the application of the appliance.
