精神分裂症患者攻击行为与事件相关电位P300的相关性研究

摘    要:目的探讨P300作为精神分裂症患者攻击行为的脑电生理学预测因子的可行性。方法采用病例对照研究方法,收集30对有或无攻击行为的精神分裂症男性患者。对两组患者的人口学资料、简明精神病量表(BPRs)各因子分、修改版外显攻击行为量表(MOAS)总分和各因子分、冲动量表(BIS-11)总分、敌意量表(BDHI)总分和各因子分、P300潜伏期及P300振幅进行比较和分析。结果①两组在婚姻状况、文化程度、职业、精神分裂症首次发病年龄、病期、治疗情况、自杀史、早年不良家庭环境方面的分布差异均无统计学意义(P〉0.05)。②研究组MOAS总分、MOAS自身及对他人攻击因子分、BIS-11总分、BDHI总分、BDHI躯体攻击因子分及易激惹因子分均高于对照组,差异均有统计学意义(P〈0.01)。③研究组中央顶部(Cz)事件相关电位P300振幅比对照组降低,P300潜伏期比对照组延长,差异有统计学意义(P〉0.05)。结论有攻击行为的男性精神分裂症患者比非攻击对照组事件相关电位P300(CZ)潜伏期延长,振幅降低,提示男性精神分裂症攻击个体可能存在更严重的认知功能障碍。

To Study the Relationship between Aggressive Behavior of Schizophrenie Patients and the P300 Component of Evenet Related Potential

Abstract:Objective To study the relationship between aggressive behavior of schizophrenic patients and the P300 component of Event -Related Potential, and try to identify the cerebral electrophysiology predictor of aggressive behavior in schizophrenics. Methods A case - control study was carried out in 30 pairs of schizophrenics with and without aggressive behavior. The demographic data, the clinical data, the total scores of BPRS, MOAS, BIS - 11, BDHI, the subscale scores of BPRS, MOAS, BDHI, P300 latency and P300 amplitude were used to assess in these two groups. Results There were no significant difference between two groups in age, the state of marriage, the degree of education, occupation, age at onset of schizophrenia, duration of the disease, the state of treatment, suicide attempt, early deviant family environment, BPRS total score and all BPRS subscale scores. The MOAS total score, MOAS property aggression, auto- aggression, physical aggression subscale scores, BIS- 11 total score, BDH1 total score, BDH1 assault, iritability subscale scores of the study group were significantly higher than that of the control lgroup. The P300 latency of the sutdy group (320.69 ± 45.16)ms was significantly longer and the p300 amplitude of the former (3.37± 1.59)try was significantly lower as compared to the control group (320.69 ±45.16 ms, 4.81 ± 0.54uv). Conclusion The results indicated that aggressive male schizophrenics had more severe deficit in cognitive function and P300 latency may be the main predictor of aggressive behaviorin patients with schizophrenia.
