尿路感染病原菌及耐药性分析

摘    要:目的:探讨尿路感染的病原菌构成及耐药性,指导临床合理使用抗菌药物。方法:收集我院2006年1月至2007年12月尿路感染患者清洁中段尿培养阳性588株细菌进行鉴定,并用K—B纸片法作药敏分析。结果:尿路感染病原菌以革兰阴性杆菌为主(63.8%),前4位分别是大肠埃希菌(45.7%)、凝固酶阴性葡萄球菌(11.6%)、假丝酵母菌(11.1%)、肠球菌(7.8%)。大肠埃希菌和克雷伯菌超广谱β-内酰胺胺酶(ESBLS)检出率分别为29%和32.1%,甲氧西林耐药凝固酶阴性葡萄球菌(MRCNS)检出率为41.2%。结论:革兰阴性杆菌是尿路感染的主要病原菌,对常规抗菌药物的耐药性呈上升趋势,细菌分离培养鉴定及药敏试验对指导临床合理使用抗菌药物具有重要意义。

Abstract:Objective: To investigate the composition and drug resistance characteristic of pathogenic bacteria from urinary tract infection and guide clinical doctors in selecting antibiotics correctly. Methods: Patients' clearing urine was collected from January 2006 to December 2007 and cultivated. Antibiotic sensitivity was tested by Kirby-Bauer method. Results: Among 588 strains from urinary tract infection, G- bacilli rated the top one (63.8%) and the most common pathogenic were Escherichia coli (45.7%) , eoagulase negative Staphylococcus (11.6%), Candida (11.1%), and Enterococcus (7.8%). ESBLs were 29% of E. coli and 32.1% of Klebsiella respectively. The constituent ratio of meticillin resistance coagulase negative Staphylococcus was 42.1%. Conclusions: G- bacilli were the main infectious pathogenic in urinary tract infection and antibiotic resistance rates continue to evolve. Therefore bacterial culture and antibiotic sensitivity test were very important in selecting antibiotics correctly for clinical treatment of the infection.
