探讨腹腔镜下附件手术中放置腹腔引流的临床意义

目的：探讨腹腔镜下附件手术中放置腹腔引流的临床意义。方法：分析总结我院自2011年6月至2012年6月216例患者行腹腔镜下附件手术（包括卵巢囊肿剥除、输卵管积脓附件切除、异位妊娠手术）的术后恢复情况。按是否放置腹腔引流与否分为引流组和非引流组,比较两组患者在术后切口（引流口）疼痛、肩部和（或）上腹部胀痛、、肛门排气时间、术后体温情况、术后白细胞计数、术后使用抗生素时间、术后住院时间等。结果：引流组切口疼痛感高于非引流组,引流组术后肩部和（或）上腹部胀痛、、肛门排气时间、术后体温情况、术后白细胞计数、术后使用抗生素时间、术后住院时间均低于非引流组（P〈0.05）。结论：腹腔镜下附件手术中放置腹腔引流可以减少术后并发症,有利于患者恢复,值得临床应用。

The clinical significance of Abdominal Cavity Drainage in Anexa Laparoscopic Surgeries

Objective：To explore the clinical value of abdominal cavity drainage in anexa laparoscopic surgeries. Methods：216 patients in our hospital who had laparoscopic surgery （including oophorocystecctomy,adnexectomy and ectopic pregnancy surgery） were analysised and their postoperative recovey was summaried.These patients were divided into two groups according to with or without drainaget, namely AUD（surgery of adnexa uteri drainage）gurop and AUND（surgery of adnexa uteri without drainage）group.The results of comparison including the pain score of incision,the pain score of shoulder and diaphragm,the mean time of passing gas,the condition of postoperative temperature and the white blood cell count,the antibiotic use time,the mean time of hospital stay were compared among two groups. Results：The patients in AUD group incison pain score was higher than that of AUND group（P〈0.05）.Ithe pain score of shoulder and diaphragm,the highest temperature during 48 hours after operation and the white blood cell count in AUD group were lower than those of AUND group（P〈0.05）.The mean time of passing gas,hospital stay and the antibiotic use time in AUD were shorter than those of AUND gronp（P〈0.05）,Conclusions：The proper expansion of the indication of putting cavity drainage during laparoscopic adnexa uteri can reduce postoperative complications,and is conducive to the postoperative recovery.
