缩窄性心包炎心包剥脱术的麻醉处理体会

目的：探讨缩窄性心包炎心包剥脱术的麻醉及术中处理,进一步提高手术麻醉安全性。方法：回顾性分析总结了12例缩窄性心包炎在非体外循环下行心包剥脱术的麻醉经过。结果：全组病例均在全麻非体外循环下完成手术,无大出血、严重心律失常、低心排、心衰、心搏骤停等并发症发生。结论：缩窄性心包炎可在全麻非体外循环下完成心包剥脱术,但由于患者全身情况及心功能差,术前应做好充分准备,术中选择对心肌抑制轻、对循环影响小的药物,注意容量控制并适当强心利尿,术中、术后加强监护治疗。

Reflection on anaesthesia for exfoliate surgery treating contraction pericarditis

Objective: Explore the way to handle anaesthesia for exfoliate surgery treating contraction pericarditis and improve anaesthesia security.Methods: Make an introspective analysis of 12 cases of contraction pericarditis that were given anaesthesia in surgical operation of pericarditis exfoliate under the condition of non-outside-body circulation.Results:The whole group underwent the operation under complete anaesthesia and non-outside-body circulation,with no complications arising,such as big bleeding,severe irregular heartbeat,low heart discharge,heart failure,etc.Conclusion: Contraction pericarditis can be operated on for exfoliate under the condition of complete anaesthesia and non-outside-body circulation,but as patients vary in physical fitness and heart capacity,a full preparation is needed beforehand,and in operation it is advisable to choose the medicine that less suppress cardiac muscle and less affect circulation;take care to control the dosage,and reinforce the heart to facilitate urine flow;and in operation or after operation,strengthen supervision and treatment.
