经直肠上动脉化疗栓塞治疗直肠癌的初步观察

目的：探讨直肠上动脉化疗栓塞术治疗直肠癌的安全性,减少化疗药物剂量后的疗效与副反应,以及对手术切除的影响。方法：采用经皮股动脉入路作直肠上动脉化疗栓塞术,用表阿霉素30mg,羟基喜树碱20mg（或顺铂30mg）,5-Fu500mg化疗,明胶海棉条（大小1.0cm×0.2cm）1条栓塞。并于化疗栓塞术后第二周或择期行肿瘤切除术。结果：16例患者临床症状减轻,CT复查10例中9例肿瘤有缩小,1例无变化为恶性间质瘤,16例中15例顺利完成根治术,尤其术前评估难以切除肿瘤的9例中有8例顺利完成肿瘤切除。结论：直肠上动脉化疗栓塞术治疗直肠癌是一种安全有效、简便的方法,可提高手术切除率,可作为直肠癌综合治疗的组成部分。

Preliminary observation of super rectal artery chemoembolization in curing rectal carcinoma

Objective： To probe into the security of super rectal artery chemoembolization curing rectal carcinoma, the curative effect and side reaction after reducing the dosage of chemotherapy drugs, and the influence on surgical resection, Method： Super rectal artery ehemoembolization was performed via percutaneous femoral artery, with 30mg of epirubicin, 20mg of hydroxy amptothecine （or 30mg of cisplatin）, 500mg of 5-Fu, and a piece of gelfoam（1.0×0.2cm）. Results： Clinical symptoms of all the observed patients were lessened. Checked by CT, the volume of tumor shrank in 9 cases out of 10, except the one who contracted malignant mesothelioma; the block resection was complete in 15 cases out of 16, and 8 out of 9 patients went through tumor excision successfully. Conclusion： Super rectal artery chemoembolization via pereutaneous femoral artery was a safe and simple way to improve resection ratio, and thus became a component of combined treatment of rectal carcinoma
