乙酰肝素酶在膀胱移行细胞癌中的表达及意义

目的:探讨乙酰肝素酶(HPSE)在膀胱移行细胞癌(BTCC)中的表达及其与肿瘤临床病理特征的关系。方法:采用免疫组织化学方法(S-P法)检测62例BTCC及12例正常膀胱组织中HPSE的表达情况。结果:HPSE在62例BTCC中的阳性表达率为53.23%(33/62例),在正常膀胱黏膜不表达。HPSE在G1,G2和G3级BTCC中阳性表达率分别为23.53%(4/17例),57.14%(12/21例)和70.83%(17/24例),各组间差异有显著性(P<0.05)。HPSE在浅表性和浸润性BTCC中的阳性表达率为31.82%(7/22例)和65.00%(26/40例),有显著性差异(P<0.05)。HPSE在复发组中表达率为76%(19/25例),显著高于初发组(41.18%),P<0.05。HPSE阳性表达率与BTCC分级、分期及复发呈正相关。结论:HPSE在BTCC中的表达增高,与肿瘤分级、分期、复发相关,检测HPSE的表达可作为判断BTCC生物学行为和预后的客观指标。

Expression and significance of heparanase in bladder transitional cell cancer

Aim:To evaluate the expression of heparanase(HPSE) protein in bladder transitional cell carcinoma(BTCC) and analyze the relationship of HPSEand their histopathological grade,clinical stage and recurrence. Methods: Sixty-two randomly selected BTCC and twelve normal bladder tissue specimens were used as experimental and controlled group.HPSE protein were detected with immunohistochemical technique.Results: HPSE protein was detected in 53.23%(33 of 62) of BTCC specimens,while normal bladder tissue did not express HPSE protein.HPSE expression rate in G1,G2 and G3 group was 29.41%(5 of 17),57.14%(12 of 21) and 70.83%(17 of 24).In muscle-invasive and superficial tumor,HPSE expression rate was 65%(26 of 40) and 31.82%(7 of 22).76%(19 of 25) of recurrence BTCC expressed HPSE protein,while in non-recurrence group,the number was 41.18%.The difference of HPSE protein expression in different grade and stage is significant(P<0.05),the same in recurrence and non-recurrence group.Relatively analysis revealed that a correlation between HPSE protein expression and patient's clinical and pathological characteristics.Conclusion:HPSE is overexpressed in BTCC.There is positive correlation between heparanase protein expression and patient's clinical and pathological characteristics.HPSE can be used as an important marker for the invasion of BTCC.
