摘    要:目的]探讨MDCTA对脊柱侧弯矫形手术的临床应用价值。材料与方法]对42例脊柱侧弯患者术前行全脊柱螺旋CT平扫及CTA检查,利用螺旋CT血管造影三维重建技术(VR法)对脊柱侧弯矫形手术相关部位进行重建,所得图像与MIP、MPR/CPR、SSD、3-D骨重建等影像后处理方式对比,以评价其优越性,并分析不同后处理技术的应用价值。结果]各种图像后处理技术的联合应用可较全面地评价脊柱侧弯的相关表现。结论]MDCT全脊柱螺旋CT扫描及CTA检查,其多种后处理技术的联合应用可对脊柱侧弯做出全面的评价,可为脊柱侧弯矫形手术制定手术计划,保证手术安全提供影像学参考。

Abstract:Purpose] To evaluate the clinical applications of imaging technique combined spiral CT 3-Dreconstruction with CTA for the correction operation of scoliosis. Materials and Methods] 42 patients with scoliosis had been performed both plain and enhanced CT scanning .Using CTA and 3-D reconstructions combined technique to reconstruct the imaging of the correlative part of the correction operation of scoliosis of the 42 patients, then evaluating its advantage by means of contrasting to MIP, MPR/CPR, SSD, 3-D bone reconstructions. Attempt to simulate the implantation of the vertebrae screw used for anterior instrumentation surgery in the workstation and measure the length of the concomitant aorta of the spinal column which needs to be corrected. Results] Using CTA and 3-D reconstructions combined technique to successfully reconstruct the imaging which can synchronously and three-di- mensionally display the malformed spinal column, aorta and other concomitant organ of the correlative part of the correction operation of scoliosis of the 42 patients. This imaging has taken on distinct predominance in synchronously and three-dimensionally displaying the malformed spinal column and its circum]acent soft tissue. Applying this tech- nique can triumphantly simulate the screw implant of anterior instrumentation surgery and can measure and work out the length of the aorta abutting upon the malformed spinal column. Conclusion] Imaging achieved from the tech- nique combined spiral CT 3D-reconstruction with CTA can synchronously and three-dimensionally display mal- formed spinal column, aorta and other concomitant organ and can provide imagology reference for surgical planning to avoid aorta damage and to ensure surgical security.
