胎膜早破对新生儿脐血免疫球蛋白及补体的影响

目的:研究胎膜早破时新生儿脐血免疫球蛋白、补体的变化规律。方法:应用速率透射比浊法检测79例新生儿脐血IgG、IgA、IgM和补体C3、C4水平,追踪新生儿出生1周内感染发生的情况,其中有胎膜早破史的研究组38例,无胎膜早破史的对照组41例。结果:胎膜早破组脐血IgA(0.16±0.12)g/L,对照组(0.06±0.03)g/L,(P<0.01);胎膜早破组IgM(0.24±0.15)g/L,对照组(0.16±0.03)g/L,(P<0.01),差异有统计学意义;脐血IgG水平无明显变化;脐血IgA浓度在胎膜早破72h为0.234±0.156)g/L,较胎膜早破<12h的0.091±0.047g/L明显增加,差异有统计学意义(P<0.05);脐血补体C3(0.76±0.35)g/L,对照组(0.93±0.21)g/L,(P<0.05),C4(0.16±0.09)g/L,对照组0.20±0.05g/L,(P<0.05),研究组明显低于对照组,差异有统计学意义。结论:有胎膜早破史的新生儿,脐血免疫球蛋白IgA、IgM浓度升高,IgG不受胎膜早破的影响,脐血补体C3、C4水平下降。脐血IgA、IgM及补体C3、C4均能反映胎膜早破个体合并宫内感染。     

妊娠期糖尿病对新生儿并发症及神经行为发育的影响

目的:观察妊娠期糖尿病对新生儿病理状况及神经行为发育的影响.方法:收集210例妊娠期糖尿病母亲婴儿的临床资料,并从同期健康母亲婴儿按1∶1比例随机抽取210例作为对照组,进行前瞻性观察研究.观察两组新生儿的并发症及病理状况,同时对比两组新生儿出生7日龄的神经行为能力评分和2月龄、6月龄的盖泽尔智能评分.结果:妊娠期糖尿病组新生儿发生低血糖、红细胞增多症、早产、大于胎龄儿、心脑畸形、呼吸窘迫综合征、高胆红素血症、合并感染发生率均高于对照组,有统计学差异(P0.05);妊娠期糖尿病组新生儿出生后7日龄神经行为能力评分为(36.77±1.33)分,显著低于对照组的(37.46±1.08)分(P0.01);GDM组婴儿2月龄、6月龄盖泽尔智能评分均显著低于对照组(P0.01,P0.05).结论:妊娠期糖尿病增加新生儿发生低血糖等病理状况的风险,对新生儿神经行为发育可能有长期不良影响.     

中医护理对新生儿缺氧缺血性脑病患儿智能发育的影响

观察早期干预联合抚触按摩对中、重度新生儿缺氧缺血性脑病的智能发育的影响。将庆阳市人民医院新生儿科住院的50例新生儿缺氧缺血性脑病患儿随机分为对照组18例;干预组25例。对照组采用三项对症三项支持常规治疗;干预组在常规治疗的基础上采用早期干预,在抚触治疗同时给予全身按摩,尤其进行足部按摩。治疗前后做新生儿行为神经测定(NBNA)及随访。NBNA评分显示干预组与对照组比较有统计学意义(P<0.01)。随访结果显示:干预组发育商(DQ)评分两组比较差异有显著性(P<0.05)干预组智能发育明显优于对照组。新生儿缺氧缺血性脑病进行早期干预联合抚触按摩治疗后智能发育明显提高,脑瘫等致残率明显降低。     

血清S-100蛋白水平与新生儿脑损伤的关系

目的探讨新生儿缺氧缺血性脑损伤血液中S-100蛋白的变化规律及其与预后的关系.方法新生儿出生后24h内采集血标本1份,检测S-100蛋白含量,1周左右行头颅CT检查.结果影像学检查有阳性与阴性两组血清S100蛋白水平分别为1.24±0.57ug/L、0.51±0.12ug/L,两组差异有统计学意义(P＜0.01),血清S-100水平与出生后1分钟Apgar评分呈负相关.结论S-100蛋白可作为临床早期评价新生儿脑损伤严重程度及预后的敏感指标.     

慢性阻塞性肺疾病患者低氧血症对血清肌钙蛋白Ⅰ的影响

目的探讨慢性阻塞性肺疾病(COPD)患者低氧血症对血清肌钙蛋白I(CTnI)的影响.方法对137例急性发作期的COPD患者的血氧分压p(O2)和血清CTnI及心肌酶谱进行检测,按氧分压的水平分成3组,观察CTnI及心肌酶谱的变化,并分析COPD患者p(O2)与CTnI及各种心肌酶的相关关系.结果轻、中、重度低氧血症组的CTnI分别为(0.15±0.08)μg/L,(0.87±0.92)μg/L和(2.24±0.97)μg/L,3组间均有显著差异(P＜0.05).而3组p(O2)水平与相对应的CTnI水平呈显著的负相关关系(r=0.968 1,P＜0.05).死亡患者8例,平均CTnI为(3.62±1.08)μg/L,与存活病例比较显著升高(P＜0.05).结论低氧血症可导致COPD患者血清CTnI显著升高,且与氧分压p(O2)呈显著负相关关系.当CTnI明显升高时,提示患者病情危重.     

哮喘患者血清基质蛋白酶的测定及临床意义

探讨了发作期哮喘患者血清基质金属蛋白酶-9(MMP-9)的水平及其在哮喘病机制中的作用.使用ELISA法分别测定了发作期门诊哮喘患者40名、健康对照20名的血清MMP-9的浓度并测定了外周血嗜酸粒细胞计数(Eos).哮喘患者血清MMP-9的浓度(160.57士76.17ng/mL)明显高于健康对照组(107.51±70.85ng/mL)(t=2.523,p＜0.05),哮喘患者的血清MMP一9水平与Eos(r=0.253,0.10＞p＞0.05)成较弱的相关关系.哮喘发作期血清MMP-9水平明显增高,MMP-9参与了哮喘的气道炎症及气道重构.     

水体中铅暴露对南方鲇Silurus meridionalis胚胎发育和仔鱼存活的影响

采用人工受精获得的南方鲇Silurus meridionalis Chen胚胎和仔鱼为实验对象,以Pb(NO3)2为毒物源,在水温22.5℃条件下,观察了水体中不同质量浓度铅(Pb)暴露对南方鲇胚胎孵化率、畸形率、自主运动、孵化历时的影响,并进行了初孵仔鱼的Pb暴露毒理实验.实验结果表明,在水体中Pb2+质量浓度分别为0(对照),4,8,12和16mg/L的条件下,南方鲇胚胎孵化率随着Pb质量浓度的上升呈下降趋势,分别为84.94%,71.23%,69.75%,66.57%和64.69%,各Pb质量浓度处理组的孵化率显著低于对照组(p0.05);且孵出的仔鱼出现心包水肿,脊柱弯曲以及尾巴弯曲退化等畸形特征,畸形率随着Pb质量浓度的升高呈上升趋势,分别为10.27%,15.28%,22.28%和36.67%,各Pb暴露组均显著高于对照组(2.37%)(p0.05);各Pb质量浓度暴露组胚胎在出膜前期的自主运动频率分别为63.86,52.71,48.43和47.14次/min均显著低于对照组(78.00次/min)(p0.05);各Pb质量浓度暴露组的胚胎孵化历时均显著长于对照组(p0.05).孵化后在净水(未加入Pb(NO3)2的实验水体)中养殖的卵黄囊期仔鱼72h死亡率随着其胚胎期Pb暴露质量浓度的上升呈上升趋势,胚胎期Pb暴露质量浓度分别为0,4,8,12和16mg/L的条件下,仔鱼期的净水死亡率分别为9.45%,18.22%,20.95%,24.05%和26.29%,胚胎期各Pb质量浓度暴露组所获仔鱼的死亡率均显著高于对照组(p0.05).根据水体Pb暴露对南方鲇初孵仔鱼急性中毒致死的观测结果推算,其96h半致死质量浓度(96hLC50)为0.32mg/L.本研究结果表明,南方鲇胚胎对水体Pb暴露的耐受性高于其初孵仔鱼,可能与南方鲇的受精卵卵膜较厚,对水体中的Pb2+进入胚胎具有较大的阻滞作用有关.     

SLE患者淋巴细胞凋亡及血清新蝶呤、γ-干扰素水平研究

探讨SLE患者体内淋巴细胞凋亡率、巨噬细胞功能相关的细胞因子(新蝶呤、γ-干扰素)水平及两者间相互关系,并同时与抗ds-DNA抗体滴度、疾病活动性评分(SLAM评分)进行相关性分析.应用Annexin V凋亡检测试剂盒及流式细胞仪检测淋巴细胞凋亡率;采用ELISA法检测血清γ-干扰素、新蝶呤水平.结果发现:(1)SLE活动期患者外周血淋巴细胞凋亡率((13.07±7.39),n=30)明显高于非活动期患者((4.08±3.55),n=8,P<0.001)及正常人((5.13±3.37),n=11,P<0.001).(2)抗ds-DNA抗体阳性组患者淋巴细胞凋亡率((12.98±9.25),n=20)与阴性组((9.35±4.76),n=18)相比无差异(P=0.14).淋巴细胞凋亡率与抗ds-DNA抗体滴度无相关性(r=0.112,P=0.77).(3)SLE患者血清新蝶呤水平((1.39±1.10)μg/L,n=20)极显著高于正常人((0.36±0.19)μg/L,n=20,P<0.01).(4)SLE活动期患者血清γ-干扰素水平((58.97±34.52)ng/L,n=15)显著高于正常人((28.06±2.35)ng/L,n=16,P<0.05).(5)SLE患者淋巴细胞凋亡率与血清新蝶呤水平呈正相关(r=0.446,P<0.05,n=22),与SLAM评分呈正相关(r=0.533,P<0.001,n=38),血清新蝶呤水平与SLAM评分亦呈正相关(r=0.485,P<0.05,n=22).未发现SLE患者淋巴细胞凋亡与抗ds-DNA抗体产生相关,未发现明显升高的细胞凋亡率与血清新蝶呤     

两种浓度左布比卡因混合芬太尼硬膜外自控分娩镇痛的观察

目的 评价硬膜外自控分娩镇痛左布比卡因的最低有效浓度.方法 左布比卡因混合芬太尼1 mg/L用于每组100例的初产妇,对照组100例.从VAS和Bromage评分、产程时间、剖宫产率、新生儿Apgar评分进行对比.结果 Ⅰ,Ⅱ组VAS评分于分娩结束后差异无统计学意义;与对照组比较,Ⅰ,Ⅱ组第一产程时间缩短、剖宫产率降低(P＜0.01),Ⅱ组第二产程时间延长(P＜0.05),新生儿Apgar评分比较差异无统计学意义(P＞0.05).结论 0.1%和0.15%左布比卡因混合芬太尼1 mg/L应用产妇硬膜外自控分娩镇痛可达到满意的效果.     

两种强度健身跑运动改善中年男子血脂代谢的研究

探讨两种强度健身跑运动对中年男性血脂异常患者血脂代谢的影响.方法:将45名中年男性血脂异常患者随机分成乳酸阈强度组(L4组)、阈下强度组(L2组)和对照组(C组)三组.L2组和L4组分别以2mmol/L和4mmol/L乳酸对应强度,进行为期16周健身跑运动,C组不参加任何运动.结果:16周运动干预后,L2组和L4组血脂指标均有改善,而且16周运动后L4组TG、TC/HDL-c、LDL-c/ HDL-c和血脂综合指数(LCI=TC·TG-LDL-c/HDL-c)的降低量明显高于L2组(p＜0.05).同时体脂百分比、体重及体重指数亦明显降低(p＜0.01).16周前后,C组血脂、身体成分各指标亦无显著性差异(P＞0.05).运动干预后,体脂百分比的降低与TG、HDL-c与LDL-c/HDL-c密切相关(p＜0.05).结论:16周健身跑运动对中年血脂异常患者的血脂、身体成分有良好的改善作用,且以4mmol/L乳酸对应强度运动效果最为明显;但对于血脂与体脂百分比改变的相关性有待于进一步的研究.     

Markedness and its Interpretation in Language Use

Language markedness is a common phenomenon in languages, and is reflected from hearing, vision and sense, i.e. the variation in the three aspects such as phonology, morphology and semantics. This paper focuses on the interpretation of markedness in language use following the three perspectives, i.e. pragmatic interpretation, psychological interpretation and cognitive interpretation, with an aim to define the function of markedness.

Lower Paleozoic oil relationships within Williston Basin, Canada

The Williston Basin is a significant petroleum province, containing oil production zones that include the Middle Cambrian to Lower Ordovician, Upper Ordovician, Middle Devonian, Upper Devonian and Mississippian and within the Jurassic and Cretaceous. The oils of the Williston Basin exhibit a wide range of geochemical characteristics defined as "oil families", although the geochemical signature of the Cambrian Deadwood Formation and Lower Ordovician Winnipeg reservoired oils does not match any "oil family". Despite their close stratigraphic proximity, it is evident that the oils of the Lower Palaeozoic within the Williston Basin are distinct. This suggests the presence of a new "oil family" within the Williston Basin. Diagnostic geochemical signatures occur in the gasoline range chromatograms, within saturate fraction gas chromatograms and biomarker fingerprints. However, some of the established criteria and cross-plots that are currently used to segregate oils into distinct genetic families within the basin do not always meet with success, particularly when applied to the Lower Palaeozoic oils of the Deadwood and Winnipeg Formation.     

低渗透非均质砂岩油藏启动压力梯度研究

理论推导与室内实验相结合,建立了低渗透非均质砂岩油藏启动压力梯度确定方法。首先借助油藏流场与电场相似的原理,推导了非均质砂岩油藏启动压力梯度计算公式。其次基于稳定流实验方法,建立了非均质砂岩油藏启动压力梯度测试方法。结果表明:低渗透非均质砂岩油藏的启动压力梯度确定遵循两个等效原则。平面非均质油藏的启动压力梯度等于各级渗透率段的启动压力梯度关于长度的加权平均;纵向非均质油藏的启动压力梯度等于各渗透率层的启动压力梯度关于渗透率与渗流面积乘积的加权平均。研究成果可用于有效指导低渗透非均质砂岩油藏的合理井距确定,促进该类油藏的高效开发。
