目的探讨股方肌骨瓣结合空心加压螺纹釘治疗靑壮年股骨颈骨折的疗效。方法行股骨颈股方肌骨瓣植骨结合空心加压螺纹釘内固定。结果本组髋优秀20例,良4例髋,优良率85.7%。结论股方肌骨辬结合空心加压螺纹钉治疗轻壮年股骨颈骨折疗效满意。  相似文献

目的:探讨空心加压螺纹钉治疗中青年股骨颈骨折的效果,并分析其特点。方法:本组18例,平均35.2岁。其中头下型4例,经颈型8例,基底型6例,均用双根空心加压螺纹钉固定。18例中12例囊内骨折联合股方肌骨瓣植入。术后8个月负重活动。平均随访1~5年。结果:本组18例全部愈合,14例发生股骨头坏死。结论:空心加压螺纹钉固定、恰当选择肌骨瓣植入,有利于中青年股骨颈骨折的愈合,是比较理想的股骨颈骨折治疗方法。  相似文献

目的:研究提高股骨颈骨折愈合率、降低股骨头坏死率的治疗手段 方法:采用切开复位、中空加压螺纹钉、骨圆针内固定加带旋股外侧动脉升支和阔筋膜张肌蒂髂骨瓣植骨治疗股骨颈移位骨折 结果:术后随访1~9年共46例,发生骨折不愈合3例、股骨头缺血坏死4例,其他无严重并发症 根据X光片和功能恢复情况分析:优24例(占52.17%)、良15例(占32.61%)、可3例(占6.52%)、差4例(占8.70%),功能恢复满意率为91.30% 结论:带旋股外侧动脉升支血管和阔筋膜张肌蒂髂骨瓣移植给缺血的股骨头引入血供,能促进股骨颈骨折愈合,减少股骨头缺血性坏死和骨折不愈合  相似文献

目的 探讨空心钉内固定术与人工髋关节置换术治疗老年性股骨颈骨折的术式选择。方法 回顾性分析1996年2月~2003年6月收治的60岁以上股骨颈骨折患者42例,其中A组(内固定组)18例,B组(人工髋关节组)24例,年龄、骨折分型、术前全身状况相似。随访11~64个月,平均25.2个月。结果 A组平均住院日数及手术持续时间均较短,且出血量少,费用低,骨折愈合率94.5%(1/18),股骨头坏死率27.7%(5/18)。需再次手术行髋关节置换术6例,再手术率为33.3%;B组需再手术的3例,再手术率为12.5%,关节功能优于A组。结论 GardenI、Ⅱ、Ⅲ型的70岁以下老年性股骨颈骨折患者,在优良复位的前提下可先予坚强的内固定治疗,Ⅰ期髋关节置换术适合于高龄及移位严重的患者。  相似文献

目的探讨和总结青壮年股骨颈骨折的最佳治疗方案.方法取Swith-perster切口,开放整复骨折,C形臂X光机下,用空心加压螺纹钉2枚固定骨折,同时将股骨颈头颈部开槽,取同侧缝匠肌髂骨瓣植骨术治疗青壮年股骨颈各类型骨折64例.结果对64例患者进行6个月至14 a,平均48个月的随访.按张少成等评定标准判定,临床优良率达92%.结论该方法具有骨折固定牢固可靠,同时以重建股骨头血液循环和为股骨头带入有效成骨成分,加速骨折的愈合,适用于青壮年各种类型的股骨颈骨折.是一种较理想的治疗方法.  相似文献

探讨中医辨证施护在空心加压螺钉结合股方肌骨瓣移植术治疗青壮年股骨颈骨折中的应用效果。将60例青壮年股骨颈骨折行空心加压螺钉结合股方肌骨瓣移植手术治疗的患者随机分为2组,观察组30例采取辩证施护,对照组30例采取传统护理。并发症发生率观察组少于对照组(P0.05);Harris髋关节评分观察组优于对照组(P0.05)。对青壮年股骨颈骨折行空心加压螺钉结合股方肌骨瓣移植手术的患者,介入中医辨证施护,可显著降低术后并发症发生率,提高护理质量。  相似文献

评价老年股骨颈骨折空心钉内固定预后的影响因素,内固定治疗老年股骨颈骨折的可行性。回顾性分析空心钉内固定治疗老年股骨颈骨折73例,年龄60~89岁,平均70.8岁,无移位骨折(Garden I型、Ⅱ型)45例;移位骨折(GardenⅢ型、Ⅳ型)28例,随访时间:12~58个月,平均31.4个月。观察内容包括受伤至手术的时间,移位程度,术后早期负重及与术后髋关节功能Harris评分。73例患者术后髋关节功能Harris评分优良率占73.97%。无移位股骨颈骨折,骨折不愈合及股骨头缺血坏死发生率为0.00%,移位骨折,不愈合及股骨头缺血坏死分别占10.71%和17.86%。如能术前掌握好适应证,术后给予正确的康复指导,老年人股骨颈骨折内固定治疗大多数效果良好。  相似文献

理论推导与室内实验相结合,建立了低渗透非均质砂岩油藏启动压力梯度确定方法。首先借助油藏流场与电场相似的原理,推导了非均质砂岩油藏启动压力梯度计算公式。其次基于稳定流实验方法,建立了非均质砂岩油藏启动压力梯度测试方法。结果表明:低渗透非均质砂岩油藏的启动压力梯度确定遵循两个等效原则。平面非均质油藏的启动压力梯度等于各级渗透率段的启动压力梯度关于长度的加权平均;纵向非均质油藏的启动压力梯度等于各渗透率层的启动压力梯度关于渗透率与渗流面积乘积的加权平均。研究成果可用于有效指导低渗透非均质砂岩油藏的合理井距确定,促进该类油藏的高效开发。  相似文献
