经皮肺动脉去交感神经冷冻消融术治疗犬肺动脉高压疗效评价

摘要:目的 通过野百合碱诱导的犬肺动脉高压模型,观察肺动脉去交感神经冷冻消融术后 6 个月的肺动脉平均压力( mPAP) 、肺毛细血管楔压( PCWP)及内皮素受体-1( ET-1)变化,评估冷冻消融去比格犬肺动脉交感神经的安全性和有效性。 方法 将 15 只比格犬随机分为 A 组(冷冻消融组) 、B 组(假手术组) 及 C 组( 对照组) ,对比观察比格犬手术前后 mPAP、PCWP 及 ET-1 水平变化。 结果 A、B 和 C 各组术后 6 个月,mPAP、PCWP 和 ET-1 值均低于术前水平,差异有统计学意义( P<0. 05) 。 结论 犬肺动脉高压经皮肺动脉去交感神经冷冻消融术安全且有效,术后表现出延续性降肺动脉压力效果。     

慢性肺心病急性发作期治疗前后血浆内皮素等血管活性物质的变化

本文应用放免法,观察慢性肺心病急性发作期呼吸衰竭患者应用持续低流量吸氧,止咳祛痰,平喘解痉,抗感染等常规综合治疗,20天前后其动脉血氧分压(PaO_2)与血浆内皮素(ET)、血浆心纳素(ANF)与血浆肾素—血管紧张素—醛固酮系统(RAA系统)在治疗前后的变化。结果治疗前与正常组相比,各项指标均有显著差异P<0.001,肺心病治疗前后比较则无明显差异,我们认为可能与慢性肺心病长期缺氧,肺血管平滑肌增生,肺动脉高压而致肺组织生理功能改变有关。     

血液灌流联合血液透析对尿毒症难治性高血压患者的疗效

目的:研究血液灌流联合血液透析对尿毒症难治性高血压患者的疗效.方法:本次研究选取的研究对象为2015年9月1日~2016年8月31日期间在我院进行治疗的尿毒症难治性高血压患者,将42例患者计算机随机分为2组,21例/组.单一实施血液透析(对照组),采用血液灌流联合血液透析(观察组).对比两组尿毒症难治性高血压患者的血压、肾素、血管紧张素Ⅱ、醛固酮浓度、同型半胱氨酸水平、血浆内皮素-1以及血清瘦素浓度.结果:观察组尿毒症难治性高血压患者治疗后的舒张压78.94±3.41 mmHg、收缩压140.25±12.37 mmHg以及平均动脉压97.96±9.88mmHg均比对照组低(P0.05);观察组患者治疗后的肾素0.90±0.27ng/mL、血管紧张素Ⅱ82.04±12.25pg/mL、醛固酮浓度1.21±0.24ng/mL以及同型半胱氨酸水平16.14±6.62μmol/L均优于对照组(P0.05);治疗后观察组患者的血浆内皮素-1 104.36±11.53pg/mL以及血清瘦素浓度10.58±2.29ng/mL与对照组存在高度差异(P0.05).结论:对尿毒症难治性高血压患者联合采用血液透析和血液灌流治疗的效果优越,具有较高的临床推广价值.     

LHI注射液抗野百合碱致大鼠肺动脉高压的实验研究

目的 :观察LHI注射液抗野百合碱 (MCT)致大鼠肺动脉高压 (PH)和右心室肥大的影响。方法 :给SD大鼠一次腹腔注射MCT(60mg.kg-1)复制PH模型 ,2wk后用LHI注射液进行治疗 ,连续8d,然后用孙波法测定肺动脉压。结果 :MCT所致大鼠肺动脉高压和右心室肥大模型效果确切 ,平均肺动脉压升高、右心室/心脏重量比、肺湿/干重比值增大 ,与对照组相比 ,P<0.001(18.47±1.32VS11.02±1.21) ;使用LHI治疗后 ,肺动脉压有所降低 ,与模型组相比P<0.05(13.31±1.12VS18.47±1.32)。结论 :MCT造成肺动脉高压模型效果确切 ,LHI注射液对MCT所致的肺动脉高压有一定的对抗作用。     

前列地尔联合疏血通治疗肺心病肺动脉高压疗效观察

目的:观察前列地尔联合疏血通治疗肺心病肺动脉高压患者的疗效。方法:将106例肺心病患者随机分为对照组和3个试验组:对照组31例,常规治疗;疏血通组25例,给予疏血通6ml静脉注射,1次/d,共10d;前列地尔组25例,给予前列地尔注射液20μg静脉注射,1次/d,共10d;联合治疗组25例,同时给予疏血通6ml静脉注射,1次/d及前列地尔20μg静脉注射1次/d,共10d。观察治疗前后各组及组间肺动脉压力的变化。结果:3个试验组治疗后较治疗前相比肺动脉压力均明显降低(P<0.05),联合治疗组效果更明显,差异有显著性(P<0.01)。结论:前列地尔联合疏血通治疗肺心病肺动脉高压患者能显著降低肺动脉压力。     

RP-HPLC法测定大鼠血浆中胡黄连苷-I浓度及药动学研究

建立反相液相色谱法测定大鼠血浆中胡黄连苷Ⅰ(HD-Ⅰ)的含量,并对胡黄连苷-Ⅰ在大鼠体内动力学特征进行分析研究。以芍药苷为内标物,血浆样品经Vc抗氧化,乙腈沉蛋白后,采用AgilentXDBC18色谱柱(250mm×4.6mm,5μm)分离,流动相:乙腈-水,梯度洗脱程序[(0-1)min]20:80,[(1-2)min]24:76,[(2-6)min]24:76,[(6-8)min]20:80,([(8-14)]min20:80;检测波长:277nm。结果:HD-Ⅰ线性范围(0.1-150)μg/mL(r=0.9994),最低检测浓度25ng,回收率分别为(94.70±3.27)％、(95.09±0.78)％、(95.91±0.83)％,日内、日问精密度准确度均小于10％。大鼠单次静脉注射HD-Ⅰ7mg/kg、14mg/kg、40mg/kg后,血药浓度-时间曲线均呈二室开放模型,半衰期T1/2β分别为(27.07±6.56)min、(29.78±6.24)min、(31.41±4.75)min,清除速率CL(s)值分别为(0.050±0.017),(0.049±0.012),(0.046±0.025)mg·kg-1·min-1(μg·mL-1),平均滞留时间MRT平均为(15.86±0.29)min,曲线下面积AUC和AUC(so)与剂量呈正相关。证实方法准确、灵敏度高,可用于HD-Ⅰ体内过程的研究。HD-Ⅰ在大鼠体内符台线性动力学二室开放模型,代谢快,分布广,清除速率快。     

血浆NT-proBNP水平对房颤患者心功能的评估

目的:观察血浆B型脑钠利尿肽前体N端脑利钠肽(NT-proBNP)水平对不同房颤患者预后情况分析,以判断NT-proBNP对房颤患者预后评估的价值。方法:取无房颤及心衰的患者(A组)23例作为对照组,72例心房纤颤的患者,依据临床症状将其分为左室功能正常者(B1组)22例,左室功能不正常者共50例,分别分为(B2组)为NYHAⅠ级26例,(B3组)为NYHAⅡ~Ⅳ级24例,进行血浆NT-proBNP测定及超声心动图LVEF的检测。结果:A组血浆NT-proBNP(7.40±8.8)pg/ml;B1组血浆NT-proBNP(56±9.8)pg/ml;B2组血浆NT-proBNP(335±12.8)pg/ml;B3组血浆NT-proBNP(956±8.8)pg/ml;组间两两比较A组与B1组差异有统计学意义(p0.05);A组与其他组差异均有显著统计学意义(P0.01)超声心动图检测,A组LVEF值(55.6±7.2)%,B1组LVEF值(52.7±8.5)%,B2组LVEF值(43.2±8.4)%,B3组LVEF值(34.2±6.1)%,组间两两比较A组与B1组差异无统计学意义(p0.05);A组与其他组差异均有统计学意义(P0.05)。结论:房颤合并心衰患者血浆NT-proBNP水平明显升高,血浆NT-proBNP水平是判断房颤合并心衰患者的独立危险因素。     

IL-23刺激对溃疡性结肠炎和感染性腹泻患者外周血单个核细胞分泌IL-17的影响

观察人白介素-23(Interlerleukin,IL-23)刺激对溃疡性结肠炎(ulcerative colitis,UC)、感染性腹泻患者及健康对照者的外周血单个核细胞(peripheral blood mononuclear cells,PBMCs)分泌人白介素-17(Interlerleukin,IL-17)的影响。分离、培养17例UC患者、10例感染性腹泻、13例健康对照者的PBMCs;采用ELISA方法检测入组者PBMCs经IL-23刺激前、后的培养上清液中IL-17水平。未经IL-23刺激的PBMCs培养上清液中IL-17水平在UC组为(29.49±6.63)pg/mL、感染性腹泻组(18.18±3.11)pg/mL、健康对照组(16.85±3.90)pg/mL,UC组高于其他两组(P<0.05)。经IL-23刺激的PBMCs培养上清液中IL-17水平在UC组为(63.63±11.79)pg/mL,较感染性腹泻组(7.87±8.31)pg/mL和健康对照组(26.53±6.42)pg/mL的升高幅度更为明显(P<0.05)。结果提示IL-23可能具有促进PBMCs分泌IL-17的作用。IL-23可能通过诱导IL-17分泌而使后者在UC的慢性病程中发挥重要作用,IL-23/IL-17免疫通路失调而产生的对机体免疫耐受机制的破坏作用,使其成为导致UC发生的原因之一。     

螺旋CT在胰腺癌诊断中的应用

目的　探讨螺旋CT双期扫描在胰腺癌诊断中的应用价值.方法　螺旋CT双期扫描胰腺癌30例,先平扫,再以3mL/s速度注射对比剂100mL,分别行胰腺期(注射后30～35s)和肝脏期(注射后65～70s)扫描,并测量各扫描期肿瘤和正常胰腺组织的CT值,计算出肿瘤 胰腺的密度差.结果　正常胰腺组织的平均CT值,在胰腺期和肝脏期分别为(119±17)Hu,(99±16)Hu,而肿瘤组织的平均CT值分别为(51±18)Hu,(61±12)Hu,胰腺 病灶增强差值分别为(66±28)Hu,(37±24)Hu,二者之间有显著差别.结论　螺旋CT双期扫描在胰腺癌诊断中具有重要的临床价值.     

不同强度远程缺血预适应对稳定型心绞痛患者运动耐量、血清一氧化氮及内皮素的影响

目的对比不同强度远程缺血预适应对稳定型心绞痛患者运动耐量、血清一氧化氮及内皮素的影响。方法将我院2016年1月至2017年6月76例稳定型心绞痛患者随机分为强度I组、Ⅱ组、Ⅲ组、IV组;以上各组均在冠心病标准治疗基础上,采用远程缺血预适应训练仪进行远程缺血预适应训练,其中强度I组一次做3个循环,共30min;强度Ⅱ组一次做4个循环,共40min;强度Ⅲ组一次做5个循环,共50min;强度IV组一次做6个循环,共60 min;以上各组每天上午、下午均各做1次,共持续1周。测定并对比四组患者试验前及试验结束时6 min的步行距离、血清一氧化氮(NO)水平及内皮素水平。结果 (1)6 min的步行距离:较试验前四组均明显改善,试验后Ⅱ组、Ⅲ组及IV组分别平均为:584、611、615 m,与I组比均有明显提高(P 0.05),且IV组提高最明显。(2)NO:四组均较试验前明显提高,试验后Ⅱ组、Ⅲ组及IV组分别平均为:60.23、67.84、68.99μmol/L,较I组均有明显提高(P 0.05),且Ⅲ和IV组提高最明显,两者无明显差异,(P0.05)。(3)内皮素:较试验前四组均明显降低,试验后Ⅱ组、Ⅲ组及IV组平均值分别为:30.17、25.59、24.85pg/mL,与I组比均有明显降低(P0.05),且Ⅲ和IV组降低最明显,两者无明显差异(P0.05)。结论不同强度的远程缺血预适应对稳定型心绞痛患者的作用保护存在差异,且远程缺血预适应强度IV组的保护作用最强。     

Markedness and its Interpretation in Language Use

Language markedness is a common phenomenon in languages, and is reflected from hearing, vision and sense, i.e. the variation in the three aspects such as phonology, morphology and semantics. This paper focuses on the interpretation of markedness in language use following the three perspectives, i.e. pragmatic interpretation, psychological interpretation and cognitive interpretation, with an aim to define the function of markedness.

Some differences are due to age, gender, statue and personality. Above all, gender is one of the obvious reasons. Lower Paleozoic oil relationships within Williston Basin, Canada

The Williston Basin is a significant petroleum province, containing oil production zones that include the Middle Cambrian to Lower Ordovician, Upper Ordovician, Middle Devonian, Upper Devonian and Mississippian and within the Jurassic and Cretaceous. The oils of the Williston Basin exhibit a wide range of geochemical characteristics defined as "oil families", although the geochemical signature of the Cambrian Deadwood Formation and Lower Ordovician Winnipeg reservoired oils does not match any "oil family". Despite their close stratigraphic proximity, it is evident that the oils of the Lower Palaeozoic within the Williston Basin are distinct. This suggests the presence of a new "oil family" within the Williston Basin. Diagnostic geochemical signatures occur in the gasoline range chromatograms, within saturate fraction gas chromatograms and biomarker fingerprints. However, some of the established criteria and cross-plots that are currently used to segregate oils into distinct genetic families within the basin do not always meet with success, particularly when applied to the Lower Palaeozoic oils of the Deadwood and Winnipeg Formation.     

低渗透非均质砂岩油藏启动压力梯度研究

理论推导与室内实验相结合,建立了低渗透非均质砂岩油藏启动压力梯度确定方法。首先借助油藏流场与电场相似的原理,推导了非均质砂岩油藏启动压力梯度计算公式。其次基于稳定流实验方法,建立了非均质砂岩油藏启动压力梯度测试方法。结果表明:低渗透非均质砂岩油藏的启动压力梯度确定遵循两个等效原则。平面非均质油藏的启动压力梯度等于各级渗透率段的启动压力梯度关于长度的加权平均;纵向非均质油藏的启动压力梯度等于各渗透率层的启动压力梯度关于渗透率与渗流面积乘积的加权平均。研究成果可用于有效指导低渗透非均质砂岩油藏的合理井距确定,促进该类油藏的高效开发。

橡胶颗粒沥青混合料低温抗裂性试验研究

以AC-13级配为基础,将橡胶颗粒代替部分集料掺入混合料中,以低温弯曲试验为评价方法对不同橡胶颗粒掺量下沥青混合料的低温抗裂性进行研究,并引入应变能密度值对混合料的低温抗裂性进行综合评价.试验结果表明:橡胶颗粒沥青混合料试件的破坏微应变均超过2 300,满足冬寒区的技术指标;无论是否掺加橡胶颗粒,随着温度的下降,沥青混合料破坏时的最大弯拉强度增大,弯拉应变降低,劲度模量增大;弯曲应变能密度在胶粒掺量为1%左右时具有较大的弯曲应变能密度值,此时橡胶颗粒沥青混合料具有较好的低温抗裂性.
