目的探讨急性ST段抬高型心肌梗死(STEMI)紧急血运重建的临床疗效.方法 308例AMI患者分为紧急血运重建治疗组157例和药物治疗组151例.对所有患者随访4~88个月,平均随访(45±26)个月.观察住院期间和随访期间的心脏不良事件和超声心动图的变化,并利用QRS计分评估梗死范围的变化.结果 1)紧急血运重建组平均住院时间为(13.8±9.7)d,显著少于内科药物治疗组(19.8±8.9)d,P0.05;2)延迟血运重建组的复合终点事件发生率显著少于药物治疗组,P0.05,差异有统计学意义;3)随访期间,延迟血运重建组的心力衰竭及复合终点事件的发生率显著低于药物治疗组,P0.05;4)超声心动图随访结果示:紧急血运重建组的LVEF显著高于药物治疗组,差异有统计学意义,P0.05,紧急血运重建组的LVDd显著小于药物治疗组,P0.05;5)紧急血运重建组的心肌梗死范围回缩率(0.386±0.120)显著大于药物治疗组(0.273±0.096),P0.05.结论紧急血运重建能减少住院期间和随访期间的心脏事件发生率,改善左室功能,使心肌梗死面积缩小,并改善AMI患者的预后.  相似文献

目的探讨延迟PCI对急性ST段抬高型心肌梗死(STEMI)预后的影响.方法 563例STEMI患者分为直接PCI组157例、延迟PCI组124例、溶栓组131例和药物组151例.随访4~88个月,平均随访(45±27)个月;观察主要心脏不良事件(MACE)、超声心动图指标变化及QRS计分评估梗死范围的变化.结果延迟PCI组MACE发生率显著少于溶栓组和药物治疗组(P0.05),与直接PCI组比较无统计学意义(P0.05);延迟PCI组的LVEF显著高于非PCI治疗组(P0.05),而延迟PCI组的LAD和LVDd均显著小于非PCI组;延迟PCI组的心肌梗死范围回缩率显著大于药物治疗组(P0.05).结论延迟PCI可减少MACE发生率,并改善左室功能,使心肌梗死面积缩小,改善STEMI患者的预后.  相似文献

目的:探讨急性心肌梗死患者接受不同再灌注治疗的特点及近期疗效.方法:回顾分析44例ST段抬高急性心肌梗死患者分别接受静脉药物溶栓、直接冠状动脉成形术、补救性冠状动脉成形术、常规治疗,比较接受不同治疗患者的临床特征,再灌注治疗时间,冠状动脉病变特点,住院期间不良心血管事件发生情况.结果:四组患者死亡率有显著差异,再发心绞痛无显著差异,直接和补救性PCI组血管开通率明显高于药物溶栓治疗.结论:对急性心绞痛患者实施不同再灌注治疗是安全有效的,直接冠脉成形术血运重建率高,改善患者预后.  相似文献

目的 分析非ST段抬高急性心肌梗死(NSTEMI)患者行经皮冠状动脉介入术(PCI)的疗效.方法观察92例接受PCI的NSTEMI患者冠状动脉造影结果、介入治疗情况、住院期及随访期的临床事件.结果92例患者PCI成功83例,失败4例.住院期间,1例患者术后死亡,无其他不良心血管事件;随访期间无不良事件发生.结论 非ST段抬高急性心肌梗死患者行PCI安全可行.  相似文献

目的观察和评价丁苯酞注射液在超早期缺血性脑卒中静脉溶栓后24 h内序贯治疗的有效性和安全性.方法选取100例发病时间在4.5 h内的脑卒中患者,根据患者随机及自愿的原则,将其分为溶栓治疗组(常规治疗+rt-PA静脉溶栓)、丁苯酞序贯治疗组(常规治疗+rt-PA静脉溶栓+丁苯酞注射液序贯治疗),每组50例.两组患者常规基础治疗相同,丁苯酞注射液于溶栓后24 h内应用,剂量为100 m L,2次/d静点,时间间隔6 h,连续治疗14 d,随访观察90 d,比较两组的疗效及安全性.结果溶栓后2周,溶栓治疗组和丁苯酞序贯治疗组患者的NIHSS评分分别为(6.28±2.69)分和(5.16±2.00)分,总有效率分别为80.0%和94.0%,差异具有统计学意义(P0.05);治疗后随访90 d,溶栓治疗组和丁苯酞序贯治疗组患者的Bl评分分别为(79.20±8.41)分和(88.10±6.37)分,mRS评分分别为(1.94±0.93)分和(1.02±0.68)分,差异具有统计学意义(P0.05).结论静脉溶栓后24 h内应用丁苯酞注射液可以显著改善患者的预后.  相似文献

[目的]比较冠状动脉三支病变患者经皮冠状动脉介入治疗(percutaneous corollary intervention,PCI)和冠状动脉旁路移植术(coronary artery bypass grafting,CABG)的预后.[方法]回顾性分析了首次行血运重建治疗的冠状动脉三支病变患者233例,其中PCI组160例,CABG组73例,比较两组患者2年的不良心脑血管事件(包括全因死亡;全因死亡和非致死性卒中/心肌梗死联合终点;再次血运重建术).[结果]CABG组的2年再次血运重建率低于PCI组,但由于术后院内死亡率较高导致CABG组2年全因死亡发生率高于PCI组(P<0.05),2年主要不良心脑血管事件发生率(major adverse cardiac and cerebrovascular events,MACCE)、2年全因死亡和非致死性卒中/心肌梗死联合终点发生率两组间差异无统计学意义(P>0.05).[结论]冠状动脉三支病变患者2年联合终点事件发生率两组间并无差异,但PCI组的2年再次血运重建率仍高于CABG组.  相似文献

急性ST段抬高型心肌梗死(STEMI)的发生和发展伴随着血栓形成并致冠脉血管闭塞,STEMI发病主要机制是冠脉血管壁上存在着不稳定性粥样硬化斑块(易损斑块),易损斑块破裂触发了凝血瀑布,导致血栓的形成和进展.高血栓负荷STEMI是经皮冠状动脉介入(PCI)治疗中的挑战性问题,PCI在开通冠脉血管的过程中,血栓因受到挤压而导致脱落,脱落的血栓可导致远端血管栓塞,发生慢血流或无复流现象,从而危及患者生命,使PCI失败.冠脉内血栓抽吸可有效清除血栓,减少慢血流和无复流事件的发生,并提高心肌组织的灌注水平,有利于保护心功能和改善预后.延迟支架术对于高血栓负荷和伴有血流动力学不稳定的患者有益,血管内低温治疗及使用抗栓溶栓药物可作PCI的辅助治疗,对于血栓负荷较少的患者行直接支架术治疗是合理的.  相似文献

探究低分子肝素与尿激酶静脉溶栓联合治疗急性心肌梗死的疗效。选取2013年11月-2016年2月甘肃省成县人民医院收治的急性心肌梗死患者152例,随机分为观察组和对照组,对照组给予尿激酶静脉溶栓治疗,观察组在对照组的基础上加用低分子肝素治疗,对比两组患者冠状动脉再通情况、心脏超声指标、心肌酶及病死率。观察组患者冠状动脉再通率72.37%显著高于对照组52.63%(P0.05);两组患者治疗后LVEF分数均明显升高,观察组患者LVEF分数升高程度显著大于对照组(P0.05),两组患者治疗后LVEDd、LVEsd及CK-MB水平均明显降低,观察组患者降低程度显著大于对照组(P0.05);观察组患者病死率3.96%显著低于对照组17.11%且差异有统计学意义(P0.05)。低分子肝素与尿激酶静脉溶栓联合可显著提高急性心肌梗死患者冠状动脉再通率,改善心脏超声指标及心肌酶水平,降低病死率。  相似文献

目的探讨主动脉内球囊反搏术辅助治疗急性ST段抬高心肌梗死(STEMI)合并左主干病变患者的近期和远期疗效.方法选取225例经冠脉造影证实为左主干病变STEMI患者,根据左主干病变和患者病情决定是否行主动脉内球囊反搏术,分为主动脉内球囊反搏术组89例、非主动脉内球囊反搏术组136例;每组再分为2个亚组,PCI组和药物治疗组.比较各组近期和远期的治疗效果、并发症发生情况.结果主动脉内球囊反搏术组患者的发病时间、左室舒张末期内径以及KillipⅢ~Ⅳ级、心源性休克、室性心律失常、急性肺水肿的比例明显高于非主动脉内球囊反搏术组患者,左室射血分数明显低于非主动脉内球囊反搏术组患者,以上差异均具有显著统计学意义(P0.01).主动脉内球囊反搏术组患者病变狭窄程度、中度以上钙化率、合并2支或3支血管病变率均明显高于非主动脉内球囊反搏术组患者(P0.05).PCI手术即刻成功率100%,平均每例患者植入支架(2.3±0.9)枚.主动脉内球囊反搏术组轻微出血发生率为14.6%,高于非主动脉内球囊反搏术组的6.6%(P0.05);大出血发生率两组间比较差异无统计学意义(P0.05).主动脉内球囊反搏术组住院期间死亡率明显低于非主动脉内球囊反搏术组,PCI治疗组死亡率明显低于药物治疗组(P0.01);主动脉内球囊反搏术联合PCI治疗后患者死亡率明显低于其他各组(P0.01).结论主动脉内球囊反搏术辅助治疗STEMI合并左主干病变可有效降低心脏不良事件发生率,提高患者近期和远期生存率.  相似文献

目的探讨优化介入流程对急性ST段抬高型心肌梗死(STEMI)患者PCI治疗血管再通时间及左室功能的影响.方法选择215例行急诊经皮冠状动脉介入(PCI)治疗并完成6个月随访的STEMI的患者,按照优化介入流程启动时间顺序分为两组.记录两组患者靶血管病变、心功能Killip分级,比较两组患者Dt B时间、平均住院日、住院总费用、入院和PCI术后6个月左室射血分数(LVEF)变化值及术后6个月死亡情况.结果两组患者基本情况、靶血管病变、心功能Killip分级等比较差异无统计学意义(P0.05);观察组入门-球囊扩张时间、平均住院日、住院总费用均低于对照组,差异具有统计学意义(P0.05);两组患者入院时LVEF相似,但在PCI术后6个月随访时左室射血分数变化值高于对照组(P0.05).结论单病种质量管理可缩短STEMI患者急诊PCI入门-球囊扩张时间,减少平均住院日及住院费用,改善术后6个月左心室功能.  相似文献

Some differences are due to age, gender, statue and personality. Above all, gender is one of the obvious reasons. The writer of this paper tries to describe the features of women's language from these perspectives: pronunciation, intonation, diction, subjects, grammar and discourse. From the discussion of the features of women's language, more attention should be paid to language use in social context. The discovery of the prolific Ordovician Red River reservoirs in 1995 in southeastern Saskatchewan was the catalyst for extensive exploration activity which resulted in the discovery of more than 15 new Red River pools. The best yields of Red River production to date have been from dolomite reservoirs. Understanding the processes of dolomitization is, therefore, crucial for the prediction of the connectivity, spatial distribution and heterogeneity of dolomite reservoirs.The Red River reservoirs in the Midale area consist of 3~4 thin dolomitized zones, with a total thickness of about 20 m, which occur at the top of the Yeoman Formation. Two types of replacement dolomite were recognized in the Red River reservoir: dolomitized burrow infills and dolomitized host matrix. The spatial distribution of dolomite suggests that burrowing organisms played an important role in facilitating the fluid flow in the backfilled sediments. This resulted in penecontemporaneous dolomitization of burrow infills by normal seawater. The dolomite in the host matrix is interpreted as having occurred at shallow burial by evaporitic seawater during precipitation of Lake Almar anhydrite that immediately overlies the Yeoman Formation. However, the low δ18O values of dolomited burrow infills (-5.9‰~ -7.8‰, PDB) and matrix dolomites (-6.6‰~ -8.1‰, avg. -7.4‰ PDB) compared to the estimated values for the late Ordovician marine dolomite could be attributed to modification and alteration of dolomite at higher temperatures during deeper burial, which could also be responsible for its 87Sr/86Sr ratios (0.7084~0.7088) that are higher than suggested for the late Ordovician seawaters (0.7078~0.7080). The trace amounts of saddle dolomite cement in the Red River carbonates are probably related to "cannibalization" of earlier replacement dolomite during the chemical compaction.  相似文献

理论推导与室内实验相结合,建立了低渗透非均质砂岩油藏启动压力梯度确定方法。首先借助油藏流场与电场相似的原理,推导了非均质砂岩油藏启动压力梯度计算公式。其次基于稳定流实验方法,建立了非均质砂岩油藏启动压力梯度测试方法。结果表明:低渗透非均质砂岩油藏的启动压力梯度确定遵循两个等效原则。平面非均质油藏的启动压力梯度等于各级渗透率段的启动压力梯度关于长度的加权平均;纵向非均质油藏的启动压力梯度等于各渗透率层的启动压力梯度关于渗透率与渗流面积乘积的加权平均。研究成果可用于有效指导低渗透非均质砂岩油藏的合理井距确定,促进该类油藏的高效开发。  相似文献

There are numerous geometric objects stored in the spatial databases. An importance function in a spatial database is that users can browse the geometric objects as a map efficiently. Thus the spatial database should display the geometric objects users concern about swiftly onto the display window. This process includes two operations:retrieve data from database and then draw them onto screen. Accordingly, to improve the efficiency, we should try to reduce time of both retrieving object and displaying them. The former can be achieved with the aid of spatial index such as R-tree, the latter require to simplify the objects. Simplification means that objects are shown with sufficient but not with unnecessary detail which depend on the scale of browse. So the major problem is how to retrieve data at different detail level efficiently. This paper introduces the implementation of a multi-scale index in the spatial database SISP (Spatial Information Shared Platform) which is generalized from R-tree. The difference between the generalization and the R-tree lies on two facets: One is that every node and geometric object in the generalization is assigned with a importance value which denote the importance of them, and every vertex in the objects are assigned with a importance value,too. The importance value can be use to decide which data should be retrieve from disk in a query. The other difference is that geometric objects in the generalization are divided into one or more sub-blocks, and vertexes are total ordered by their importance value. With the help of the generalized R-tree, one can easily retrieve data at different detail levels.Some experiments are performed on real-life data to evaluate the performance of solutions that separately use normal spatial index and multi-scale spatial index. The results show that the solution using multi-scale index in SISP is satisfying.  相似文献
