脑脊液改变不典型的化脓性脑膜炎10例临床分析

化脓性脑膜炎是临床常见的一组严重疾病，又称为细菌性脑膜炎，是指由多种细菌引起的蛛网膜、软脑膜的化脓性炎症。临床主要表现为发热、头痛及脑脊液的化脓性改变。脑脊液培养是诊断化脓性脑膜炎的金标准。近年来，随着我院临床检验的完善，现将2005年-2009年我科收治的脑脊液改变不典型的化脓性脑膜炎病人的临床资料进行整理分析。     

艾滋病并发新型隐球菌脑膜炎11例临床分析

目的:分析艾滋病(AIDS)并发新型隐球菌脑膜炎临床特征、诊断方法、治疗和预后.方法:对云南省德宏州医疗集团人民医院1998年12月-2005年12月收住院的AIDS并发新型隐球菌脑膜炎病人11例进行回顾性分析.结果:所有病例均有高热和头痛以及神经系统症状如呕吐、嗜睡、昏迷等,不同程度存在AIDS相关表现.所有病例AIDS病毒抗体(HIV-Ab)确诊试验阳性.所有病例脑脊液(CSF)墨汁染色法查见发新型隐球菌.结论:AIDS并发新型隐球菌脑膜炎临床表现无特异性,CSF墨汁染色仍不失为一种简单实用的诊断方法.本病例病情危重,预后差,病死率高.     

肝硬化腹水并发自发性腹膜炎42例临床分析

目的分析肝硬化并发自发性腹膜炎的临床特点及预后.方法对42例肝硬化并发SBP患者的临床资料进行回顾性分析.结果42例患者中,发热34例(80.9%),腹胀37例(88.4%),腹痛18例(42.1%),腹水PMN≥0.50 36例(85.5%).结论肝硬化并发SBP临床表现不典型,治愈、好转率低,腹水常规检查可早期诊断,腹水PMN比值是诊断SBP的可靠参数.

94例下消化道肿瘤内镜下黏膜切除术的临床分析

目的：探讨内镜下黏膜切除术（Endoscopic Mucosal Resection，EMR）在治疗下消化道肿瘤中的临床价值及安全性.方法：94例患者共175个病灶，以注射法行黏膜切除治疗，记录操作方法并按照性别年龄分布情况、并发症、切除标本病理类型等进行分析.结果：94例患者中20-29岁组，30-39岁组，40-49岁组，50-59岁组，60-69岁组，70-79岁组，80-89岁组所占比例分别为1.06%，4.26%，19.15%，40.43%，22.34%，10.64%，2.12%.175个病灶均完整切除，治愈率100%.并发症：术中渗血5例（5.32%），以氩离子凝固术（Argon Plasma Coagulation）成功止血.无一例发生延迟出血、穿孔、狭窄及败血症等严重并发症.术后病理诊断：炎性息肉3个，增生性息肉67个，腺瘤性息肉104个，类癌1个.结论：内镜下黏膜切除术在下消化道肿瘤治疗中，是一种安全、有效、微创的内镜治疗手段.     

肺炎支原体肺炎合并心肌损害30例临床分析

目的：探讨肺炎支原体（MP）肺炎患儿的心肌损害.方法：对30例合并有心肌损害的MP肺炎患儿的临床资料进行回顾性分析.结果：262例MP肺炎中的心肌受损11%,多发生在感染的急性期,大部分为一过性或症状轻微,少数表现为严重的心肌损害.结论：MP感染后可在呼吸道感染的基础上并发心肌损害.     

结核性脑膜炎53例早期辅助检查分析

目的：探讨相关辅助检查诊疗结核性脑膜炎的价值。方法：回顾性分析2002年6月-2006年8月在昆明医学院第一附属医院神经内科住院治疗的53例结核性脑膜炎患者的临床资料。结果：脑脊液测压、细胞学及生化检查异常率高,是重要的实验室检查方法。头颅CT和MRI增强扫描异常率也高,有特征性表现,也应作为常规检查;胸片可以为结核性脑膜炎寻找佐证。结论：结核性脑膜炎临床存在较多不典型之处,及时、合理运用相关辅助检查对正确诊治结核性脑膜炎意义重大。     

肺炎链球菌致化脓性脑膜炎1例

肺炎链球菌属链球菌属。主要寄生于人体上呼吸道,部分菌株有致病力,可引起大叶性肺炎,引起化脓性脑膜炎者临床极少见。我们于1993年5月从一例化脓性脑膜炎患儿硬膜外穿刺液中分离出一株肺炎链球菌,现报道如下。 患者,男,5个月,因不明原因发热二天伴呕吐,嗜睡继而昏迷于1993年5月4日采院就诊。实验室检查:硬膜外穿刺液外观呈柠檬包,微混,无凝块;白细胞总数18/mm~3,蛋白质定性呈强阳性,糖2.2mmol/L,氯化物106mmol/L。末梢血白细胞总数16.4×10~6/L,分类计数N 0.86,L0.14。经CT检查提示化脓性脑膜炎可能。临床诊断为化脓性脑膜炎。入院后因病情严重,持续昏迷七天后抢救无效于5月11日死亡。     

57例新生儿败血症临床特点及检验结果分析

目的:分析新生儿败血症的临床特点,为新生儿败血症治疗提供支持。方法:回顾性分析2013年1月至2017年8月新生儿科收治的57例新生儿败血症的临床诊治资料,并对阳性检验结果进行分析。结果:早发型败血症53例(92.98%),母亲有致病危险因素36例,排前3位的分别是胎膜早破14例(24.6%)、孕期感染8例(14.0%)、羊水污染8例(14.0%);患儿有多种症状者27例(35.09%),排前3位的为呕吐17人次(29.8%)、皮肤黄疸16人次(28.1%)、纳差14人次(24.6%);降钙素原、C反应蛋白、中性粒细胞杆状核/中性粒细胞、白细胞及血小板5项检查结果阳性率为59.64%、56.14%、38.77%、38.60%及5.66%;C反应蛋白和降钙素原水平呈正向相关,r=0.7,p0.01;血培养阳性7例(12.28%),其中金黄色葡萄球菌2例,表皮葡萄球菌3例,溶血性葡萄球菌1例,大肠埃希菌1例。结论:综合参考实验检查的阳性指标,可提高新生儿败血症早期诊断。     

隐球菌性脑膜炎误诊为结核性脑膜炎11例临床分析

目的：探讨隐球菌性脑膜炎误诊为结核性脑膜炎的临床特点。为隐球菌性脑膜炎或结核性脑膜炎难以鉴别时进一步诊治提供参考．方法：回顾性分析11例误诊为结核性脑膜炎的临床资料．结果：本组患者均多次脑脊液涂片及培养后证实为隐球菌性脑膜炎，抗结核治疗均未愈．结论：隐球菌性脑膜炎误诊为结核性脑膜炎时多次脑脊液涂片和培养可证实为隐球菌性脑膜炎．     

Markedness and its Interpretation in Language Use

Language markedness is a common phenomenon in languages, and is reflected from hearing, vision and sense, i.e. the variation in the three aspects such as phonology, morphology and semantics. This paper focuses on the interpretation of markedness in language use following the three perspectives, i.e. pragmatic interpretation, psychological interpretation and cognitive interpretation, with an aim to define the function of markedness.     

Features of Women's language

Language is a means of verbal communication. People use language to communicate with each other. In the society, no two speakers are exactly alike in the way of speaking. Some differences are due to age, gender, statue and personality. Above all, gender is one of the obvious reasons. The writer of this paper tries to describe the features of women＇s language from these perspectives： pronunciation, intonation, diction, subjects, grammar and discourse. From the discussion of the features of women＇s language, more attention should be paid to language use in social context. What＇s more, the linguistic phenomena in a speaking community can be understood more thoroughly.

The book did not gain immediate success, but it is now thought one of the finest novels in the English language. Lower Paleozoic oil relationships within Williston Basin, Canada

The Williston Basin is a significant petroleum province, containing oil production zones that include the Middle Cambrian to Lower Ordovician, Upper Ordovician, Middle Devonian, Upper Devonian and Mississippian and within the Jurassic and Cretaceous. The oils of the Williston Basin exhibit a wide range of geochemical characteristics defined as "oil families", although the geochemical signature of the Cambrian Deadwood Formation and Lower Ordovician Winnipeg reservoired oils does not match any "oil family". Despite their close stratigraphic proximity, it is evident that the oils of the Lower Palaeozoic within the Williston Basin are distinct. This suggests the presence of a new "oil family" within the Williston Basin. Diagnostic geochemical signatures occur in the gasoline range chromatograms, within saturate fraction gas chromatograms and biomarker fingerprints. However, some of the established criteria and cross-plots that are currently used to segregate oils into distinct genetic families within the basin do not always meet with success, particularly when applied to the Lower Palaeozoic oils of the Deadwood and Winnipeg Formation.     

低渗透非均质砂岩油藏启动压力梯度研究

理论推导与室内实验相结合,建立了低渗透非均质砂岩油藏启动压力梯度确定方法。首先借助油藏流场与电场相似的原理,推导了非均质砂岩油藏启动压力梯度计算公式。其次基于稳定流实验方法,建立了非均质砂岩油藏启动压力梯度测试方法。结果表明:低渗透非均质砂岩油藏的启动压力梯度确定遵循两个等效原则。平面非均质油藏的启动压力梯度等于各级渗透率段的启动压力梯度关于长度的加权平均;纵向非均质油藏的启动压力梯度等于各渗透率层的启动压力梯度关于渗透率与渗流面积乘积的加权平均。研究成果可用于有效指导低渗透非均质砂岩油藏的合理井距确定,促进该类油藏的高效开发。

The analysis shows that this scheme is easy implemented, low cost and adapt to all kinds of mobile nework system.     

橡胶颗粒沥青混合料低温抗裂性试验研究

以AC-13级配为基础,将橡胶颗粒代替部分集料掺入混合料中,以低温弯曲试验为评价方法对不同橡胶颗粒掺量下沥青混合料的低温抗裂性进行研究,并引入应变能密度值对混合料的低温抗裂性进行综合评价.试验结果表明:橡胶颗粒沥青混合料试件的破坏微应变均超过2 300,满足冬寒区的技术指标;无论是否掺加橡胶颗粒,随着温度的下降,沥青混合料破坏时的最大弯拉强度增大,弯拉应变降低,劲度模量增大;弯曲应变能密度在胶粒掺量为1%左右时具有较大的弯曲应变能密度值,此时橡胶颗粒沥青混合料具有较好的低温抗裂性.
