MRI对脑转移瘤和多发性恶性胶质瘤的鉴别诊断

本研究的目的是探讨常规MRI对多发性恶性胶质瘤和转移瘤鉴别诊断的作用.病例包括2004年7月～2006年12月间收入脑外科在术前来我科做MRI检查的8例多发性恶性胶质瘤和15例多发脑转移瘤患者.8例恶性胶质瘤中,星形细胞瘤Ⅲ级即间变型(恶性)胶质细胞瘤(anaplastic astrocytomas)1例,星形细胞瘤Ⅳ级即多形性胶质母细胞瘤(multiforme glioblastoma)7例.15例脑转移瘤,除8例有明确肺癌病史其脑内多发弥漫结节状转移病灶未能手术外,其余7个病例均做了开颅手术,得到了病理证实.这7例转移瘤中原发瘤2例为乳腺癌,3例为肝细胞癌,3例为卵巢癌.结果15例脑转移瘤中,1例位于脑干外,余14例病灶位于皮质内或皮层下.呈环状强化,有大的壁结节,或整个结节均匀强化.8例多发性恶性胶质瘤中,病灶都位于同一大脑半球且各个肿块之间相距较近.病灶边界不清,5例病灶位于皮髓质交界处3例肿瘤位于髓质深部及基底节区,瘤内有大片状或多数小斑片状坏死无强化区,有完整或不完整的强化环.脑转移瘤和恶性胶质瘤MRI的鉴别诊断要点有①转移瘤患者年龄比胶质瘤大,常有原发瘤病史;②转移瘤易多发且位置表浅,瘤脑界面一般比胶质瘤清晰;③转移瘤单发时,整个肿瘤均匀强化比胶质瘤多见,环状强化时转移瘤环壁相对规则;④胶质瘤多发时病灶相对集中.但这些征象缺乏特异性.     

自体角膜缘移植联合羊膜移植治疗复发性翼状胬肉

探讨自体角膜缘上皮移植术联合羊膜移植在复发性翼状胬肉治疗中的作用.以68例(73只眼)复发性翼状胬肉患者采用自体角膜缘上皮移植术联合羊膜移植术治疗,术后观察6～24月,平均(13.1±2.3)月.植片全部存活,无排斥反应发生.随访期2例(2只眼)翼状胬肉发生复发.1例发生巩膜裸露区溶解.自体角膜缘上皮移植联合羊膜移植是治疗复发性翼状胬肉简单、安全、有效的手术方法.     

三种手术方法治疗翼状背肉的疗效比较

目的:进行自体角膜缘干细胞移植术治疗翼状胬肉与以往常规2种手术方法的疗效比较.方法:将翼状胬肉54例60只眼随机分为A,B,C三组,每组各20眼.A组采用传统胬肉切除及暴露角膜缘巩膜术;B组采用胬肉切除及结膜瓣移植术;C组采用新型胬肉切除及自体角膜缘干细胞移植术.对3组术后创面修复时间及2年内的复发率进行比较.结果:A组角巩膜创面修复时间平均8.0天,B组5.8天,C组4.6天;经2年以上随访,A组复发率20%,B组5%,C组无一例复发.结论:新型胬肉切除及自体角膜缘干细胞移植术治疗翼状胬肉,具有操作简便、创伤小、恢复快及复发率低等优点.     

直肠前壁低度恶性间质瘤3例临床分析

目的探讨直肠前壁低度恶性间质瘤的诊断及手术治疗效果.方法回顾性分析3例直肠前壁低度恶性间质瘤患者的临床资料.结果 3例患者均行手术治疗,经病理及免疫组化证实均为直肠低度恶性间质瘤.其中,2例完整切除,1例未完整切除.未完整切除者术后16个月死亡,另2例现无瘤存活.结论直肠前壁低度恶性间质瘤临床虽极少发生,但术前仍可作出初步诊断,病理及免疫组化可确诊,术中要尽可能完整切除肿瘤,这是治疗的关键,可明显改善预后.     

角膜缘干细胞移植术治疗翼状胬肉疗效观察

目的：观察角膜缘干细胞移植术冶疗翼状胬肉的疗效。方法：采用翼状胬肉切除术十角膜缘干细胞移植术对16例患者18眼进行治疗,术后随访6月～1年。结果：16例患者18眼未发现翼状胬肉复发及结膜组织异常增生。结论：角膜缘干细胞移植术冶疗翼状胬肉疗效确切。可做为门诊冶疗翼状胬肉理想的手术方式推广。     

47例恶性黑色素瘤的临床病理分析

为进一步提高恶性黑色素瘤的诊断水平,选取1980～2005年经病理诊断为恶性黑色素瘤病例47例,进行临床病理资料回顾性分析.恶性黑色素瘤临床表现、病理形态复杂多样,瘤细胞有上皮样、梭形、小痣细胞样、瘤巨细胞等多种类型.瘤细胞可呈巢状、腺泡状、束状、编织状或弥漫散在分布.分析结果表明,结合临床表现,以HE染色切片的细胞形态观察为基础,合理应用S-100蛋白、Vimentin、HMB-45等免疫组化染色,可以及时得出恶性黑色素瘤的明确诊断.     

自体角膜缘干细胞移植治疗复发性翼状胬肉

目的:评价自体角膜缘干细胞移植术治疗复发性翼状胬肉的效果。方法:采用自体角膜缘干细胞移植术治疗复发性翼状胬肉50例56眼,术后随访6～48个月,平均26个月。结果:50例56眼术后治愈53眼,被胬肉组织侵袭的角膜均透明或仅遗留薄翳,无新生血管长入。复发3眼,复发率5.36%。结论:自体角膜缘干细胞移植术可为受损的角膜缘提供新的健康的干细胞,可有效防止翼状胬肉的复发,是治疗翼状胬肉的理想方法。     

探讨中西医结合治疗翼状胬肉的临床疗效

探讨翼状胬肉逆行切除联合自体角膜缘干细胞移植，术后口服中药治疗翼状胬肉的临床疗效。原发性翼状胬肉40例50眼，男10例，女30例，年龄40-72岁，平均56岁。10例双眼。分别行翼状胬肉逆行切除及自体角膜缘干细胞移植术，术后应用裂隙灯观察植片成活情况。非接触式眼压测眼压，术后口服中药，随访6个月～2年。术后植片均成活。48眼治愈，1眼复发，1眼出现复视。翼状胬肉逆行切除联合自体角膜缘干细胞移植可重建角膜缘的正常结构，有效地阻止异常结膜源性组织的再侵入，术后口服中药调理以后，大大降低了翼状胬肉的复发率。     

角膜缘干细胞移植治疗翼状胬肉46例分析

目的：观察角膜缘干细胞移植治疗翼状胬肉的疗效，探讨角膜缘干细胞移植术后复发率是否比传统单纯行翼状胬肉切除术复发率低。方法：对46例46眼翼状胬肉行角膜缘干细胞移植，24例24眼行单纯翼状胬肉切除术，术后定期随访，观察疗效和眼表情况。结果：角膜缘干细胞移植组平均随访11月，手术成功率97．83％，复发率2．17％；传统单纯翼状胬肉切除术组平均随访10月，手术成功率75％，复发率25％。结论：角膜缘干细胞移植手术复发率明显低于单纯翼状胬肉切除术，可替代传统手术方式治疗翼状胬肉。

Markedness and its Interpretation in Language Use

Language markedness is a common phenomenon in languages, and is reflected from hearing, vision and sense, i.e. the variation in the three aspects such as phonology, morphology and semantics. This paper focuses on the interpretation of markedness in language use following the three perspectives, i.e. pragmatic interpretation, psychological interpretation and cognitive interpretation, with an aim to define the function of markedness.

Language is a means of verbal communication. People use language to communicate with each other. In the society, no two speakers are exactly alike in the way of speaking. Some differences are due to age, gender, statue and personality. Above all, gender is one of the obvious reasons. Lower Paleozoic oil relationships within Williston Basin, Canada

The Williston Basin is a significant petroleum province, containing oil production zones that include the Middle Cambrian to Lower Ordovician, Upper Ordovician, Middle Devonian, Upper Devonian and Mississippian and within the Jurassic and Cretaceous. The oils of the Williston Basin exhibit a wide range of geochemical characteristics defined as "oil families", although the geochemical signature of the Cambrian Deadwood Formation and Lower Ordovician Winnipeg reservoired oils does not match any "oil family". Despite their close stratigraphic proximity, it is evident that the oils of the Lower Palaeozoic within the Williston Basin are distinct. This suggests the presence of a new "oil family" within the Williston Basin. Diagnostic geochemical signatures occur in the gasoline range chromatograms, within saturate fraction gas chromatograms and biomarker fingerprints. However, some of the established criteria and cross-plots that are currently used to segregate oils into distinct genetic families within the basin do not always meet with success, particularly when applied to the Lower Palaeozoic oils of the Deadwood and Winnipeg Formation.     

低渗透非均质砂岩油藏启动压力梯度研究

理论推导与室内实验相结合,建立了低渗透非均质砂岩油藏启动压力梯度确定方法。首先借助油藏流场与电场相似的原理,推导了非均质砂岩油藏启动压力梯度计算公式。其次基于稳定流实验方法,建立了非均质砂岩油藏启动压力梯度测试方法。结果表明:低渗透非均质砂岩油藏的启动压力梯度确定遵循两个等效原则。平面非均质油藏的启动压力梯度等于各级渗透率段的启动压力梯度关于长度的加权平均;纵向非均质油藏的启动压力梯度等于各渗透率层的启动压力梯度关于渗透率与渗流面积乘积的加权平均。研究成果可用于有效指导低渗透非均质砂岩油藏的合理井距确定,促进该类油藏的高效开发。
