超声引导下自动肾活检术的临床应用

目的 :评估超声引导自动肾活检术的临床应用价值。方法 :本组35例 ,其中男11例 ,女24例 ,年龄6～49岁 ,平均年龄29岁。用14G、16G自动活检针 ,选择右肾下极1/3,避开肾窦区 ,肾实质最厚处作为进针穿刺点进行穿刺活检。结果 :取材成功35例 ,成功率100%。取材次数2～5次 ,平均2.3次。取材病检肾小球数25～82个 ,平均32±12.5个。仅2例发生肉眼血尿 (占5.7% ) ,27例有镜下血尿 (占77 % ) ,无一例发生需要处理的严重并发症。结论 :超声引导下自动肾活检技术 ,取材高效、安全。     

介入性超声在甲状腺疾病诊断中的应用

目的：探讨介入性超声在甲状腺疾病诊断中的应用。方法：17例均系我院临床及影像检查(超声、CT)难以明确诊断或高度怀疑甲状腺癌而行超声引导下自动活检技术检查。其中女13例，男4例，年龄29～75岁，平均52岁。结果：17例甲状腺疾病超声引导下自动活检取材成功率100％，能作出明确病理诊断达94％，本组病例无严重并发症发生。结论：介入性超声在甲状腺疾病诊断中具有重要的临床应用价值。     

快速介入性活检技术在纵隔疾病诊断中的应用

目的探讨B超引导下自制穿刺导向器快速介入性活检技术在纵隔疾病诊断中的临床应用价值。方法对36例经X线、CT或MRI证实有纵膈占位性病变且不能确诊的患者行自制B超穿刺导向器引导下的快速自动活检。结果36例患者全部穿刺成功,取材满意,取材阳性率100%。34例有明确的病理结果,病理阳性率94.4%。结论B超引导下自制穿刺导向器的快速介入性活检技术所取组织能够充分满足病理切片需要,病理确诊阳性率高,无严重并发症,能够为临床提供可靠的诊断治疗依据。     

超声引导下外周型肺病变及胸腔肿瘤穿刺活检

目的评价超声引导下穿刺活检在外周型肺病变及胸腔肿瘤中的诊断作用和安全性。方法回顾性分‘析多种肺及胸腔病变共30例,在超声引导下用Bard弹射式自动活检枪和18G活检针、配合使用ALOKA9133小凸阵探头进行经皮穿刺活检。标本送组织病理检查,结果与手术病理及临床随访的最后诊断进行比较。结果30例患者,穿刺活检成功率为100％,组织取材满意率93．5％,诊断准确性为100％,穿刺后并发症共2例、咯血2例。结论超声引导下对外周型肺病变及胸腔肿瘤穿刺活检具有定位准确、操作简便、安全性好、并发症少及确诊率高等优点,使多数病变获得了明确诊断,有助于临床进一步治疗。     

骨穿一步法抽吸-活检双标本对全血细胞减少症诊断及鉴别诊断

探讨骨穿一步法,骨髓穿刺涂片与骨髓活检切片同步观察对诊断全血细胞减少症诊断及鉴别诊断应用意义.180例全血细胞减少症患者采用骨穿一步法取抽吸一活检双标本,同步观察其涂片和切片.取材合格的情况下,活检切片对骨髓增生程度的判断优于穿刺涂片.病态造血组织学的检出率,活检切片高于穿刺涂片;病态造血形态学的检出率,穿刺涂片高于活检切片.网状纤维及胶原纤维增生的观察,活检切片明显优于穿刺涂片.骨穿一步法证明先抽吸后环钻取材,并不影响活检结果;穿刺涂片和活检切片同步分析比单一穿刺涂片形态学观察更有利于提高全血细胞减少症诊断的准确性.     

48例肾脏病理分析

目的:总结48例经皮超声下肾穿刺活检术(简称肾活检)的方法及其临床与病理分析。方法:共48人采用美国RARD MAGNUM自动穿刺枪,在B超(配备穿刺探头)引导下进行肾活检。所有标本均作光镜、免疫病理及电镜检查。结果:48例患者肾活检均获成功,无严重并发症。术前临床病因不清者21例(44%),术后全部确诊。有9例对治疗方案进行了较大修改(19%)。结论:经皮超声引导下肾穿刺活检术并发症少,风险小。术后病理类型确诊,临床诊断、治疗方案修正率与文献相近〔1〕,使患者得到了及时正确的治疗。     

CT引导下经皮肺穿刺活检诊断周围性肺部病变

目的 探讨CT引导下经皮肺穿刺活检在周围性肺部病变中的诊断价值。方法对2002年6月～2005年1月周围性肺部病变行CT引导下经皮肺穿刺活检的64例临床病例进行回顾性分析结果64例中穿刺成功率100％，活检诊断准确率969％，3例发生气胸、1例发生痰中带血。结论 周围性肺部病变行CT引导下经皮肺穿刺活检是一种创伤小、准确率高、并发症少的诊断方法。     

CT引导下经皮穿刺活检在肺癌临床病理类型诊断中的应用

探讨CT引导下经皮穿刺活检在肺癌临床病理类型诊断中的应用价值。回顾性分析高台县人民医院2013年10月～2018年7月接受CT引导下经皮穿刺活检的78例病例资料,将获得的活检结果与后期治疗及随访诊断结果进行对比。78例病例均获得满意的组织标本,穿刺成功率为100%;CT引导下经皮穿刺活检结果与后期治疗及随访诊断结果基本吻合,诊断准确率高达96.15%(P0.05),其中良性病灶31例(39.74%),恶性病灶47例(60.26%),误诊3例,穿刺为炎性细胞浸润,而经后期治疗及随访发现为腺癌;穿刺后并发气胸4例,并发咯血7例,经对症治疗均好转。将CT引导下经皮穿刺活检用于肺癌临床病理类型诊断中,可有效提高诊断准确率,且并发症相对较少,不失为一种安全有效的诊断方法。     

超声引导下颈部淋巴结自动活检的临床应用价值

超声引导下自动活检已广泛应用于临床，它是在超声显像的基础上为进一步满足临床需要而发展起来的一门新技术。我们自１９９７年以来对１２例１２个淋巴结行穿刺活检。１资料与方法１２例均为在我院就诊时发现颈部淋巴结肿大，其中男７例，女５例，年龄１５—６５岁，平均４４岁。所用ＡＢＤ为国产及进口的可调式自动活检枪，取材长度为１５—２２ｍｍ连续可调，穿刺针用１８Ｇ内槽式切割针。所用超声仪为东芝ＳＳＡ—２４０Ａ及东芝ＳＳＨ—１４０Ａ，针具引导用自制可拆卸角度可调式导向器，探头．引导架穿刺针经福尔马林熏蒸消毒。受检者充分暴露颈部，仰卧于检查床上，在颈后双肩垫一枕头，呈头低颈高位，用Ｂ超选择穿刺点，尽量选择远离颈总动脉和颈内静脉的淋巴结。常规皮肤消毒及局麻后，用消毒刀片在穿刺点做一小切口。由于颈部组织松弛，进针时要求助手压紧颈部皮肤，固定淋巴结，根据淋巴结的大小调节活检枪使切取组织深度在０．５—１．２ｃｍ的范围。在超声引导下将切割针刺向淋巴结表面，开枪切取组织，迅速拔针，将槽式针内切割的组织置于１０％甲醛液中固定送病理检查。２结果１２例患者均为颈部多发淋巴结肿大。１２例１１个淋巴结一次性获得明确的组织学诊断，１例取材较浅，没有获得明...

Markedness and its Interpretation in Language Use

Language markedness is a common phenomenon in languages, and is reflected from hearing, vision and sense, i.e. the variation in the three aspects such as phonology, morphology and semantics. This paper focuses on the interpretation of markedness in language use following the three perspectives, i.e. pragmatic interpretation, psychological interpretation and cognitive interpretation, with an aim to define the function of markedness.     

Features of Women's language

Language is a means of verbal communication. People use language to communicate with each other. In the society, no two speakers are exactly alike in the way of speaking. Some differences are due to age, gender, statue and personality. Above all, gender is one of the obvious reasons. The writer of this paper tries to describe the features of women＇s language from these perspectives： pronunciation, intonation, diction, subjects, grammar and discourse. From the discussion of the features of women＇s language, more attention should be paid to language use in social context. What＇s more, the linguistic phenomena in a speaking community can be understood more thoroughly.     

低渗透非均质砂岩油藏启动压力梯度研究

理论推导与室内实验相结合,建立了低渗透非均质砂岩油藏启动压力梯度确定方法。首先借助油藏流场与电场相似的原理,推导了非均质砂岩油藏启动压力梯度计算公式。其次基于稳定流实验方法,建立了非均质砂岩油藏启动压力梯度测试方法。结果表明:低渗透非均质砂岩油藏的启动压力梯度确定遵循两个等效原则。平面非均质油藏的启动压力梯度等于各级渗透率段的启动压力梯度关于长度的加权平均;纵向非均质油藏的启动压力梯度等于各渗透率层的启动压力梯度关于渗透率与渗流面积乘积的加权平均。研究成果可用于有效指导低渗透非均质砂岩油藏的合理井距确定,促进该类油藏的高效开发。
