不同胃疾病患者血清胃蛋白酶原亚群的变化

目的 探讨不同胃疾病患者胃黏膜病理改变过程中,血清胃蛋白酶原(PC)Ⅰ和Ⅱ的变化规律.方法 选择已行常规胃镜检查和病理活检确诊的245例患者.其中非萎缩性胃炎53例,慢性萎缩性胃炎20例,胃溃疡36例,十二指肠溃疡31例,进展期胃癌69例,早期胃癌13例,胃癌术后23例,并设正常对照组36例,用酶联免疫吸附实验(ELISA)测定其血清PGⅠ及PGⅡ,并计算PGⅠ/PGⅡ的比值(PGK).结果 与正常对照组比较,早期胃癌和进展期胃癌患者血清PG Ⅰ和PG Ⅰ/Ⅱ比值均显著降低(P<0.005),胃溃疡患者血清PGⅠ升高(P<0.05),十二指肠溃疡患者血清PG Ⅰ和PGⅡ升高,PGI/PGⅡ比值升高(P<0.05);进展期胃癌血清PG Ⅰ、早期与进展期胃癌PGR均显著低于萎缩性胃炎组(P<0.001).胃癌组中以PG Ⅰ≤70μg/L和PGR≤6作为阳性指标,同时符合这两项的确诊率为62.1%,特异性为94.2%.结论 血清PG Ⅰ、PG Ⅱ含量的变化及PGR值与胃黏膜病变有关,血清PG Ⅰ和PG Ⅰ/PGⅡ比值的降低,是胃癌发生的危险因素,可以作为人群筛查和辅助诊断胃癌的一项血清学指标.     

血清胃蛋白酶原与胃癌

综述了血清胃蛋白酶原(SPG)与胃癌的研究进展资料显示胃癌患者血清PG Ⅰ,PG Ⅰ/Ⅱ比值在统计学上有显著性下降,胃癌术后复发与转移患者血清PG Ⅰ,PGⅡ明显升高.研究认为血清PG含量可作为胃癌早期诊断、胃癌术后复发与转移的检测指标之一.     

阿莫西林联合叶酸对慢性萎缩性胃炎患者疗效及胃动素及胃蛋白酶原的影响

探讨阿莫西林联合叶酸对慢性萎缩性胃炎患者疗效及胃动素及胃蛋白酶原的影响。选取2013年6月-2016年3月在我院就诊的慢性萎缩性胃炎患者122例,随机分为观察组和对照组,对照组给予叶酸治疗,观察组给予阿莫西林联合叶酸治疗,观察两组患者临床疗效、胃动素(MOT)及胃蛋白酶原(PG)变化。观察组患者治疗有效率显著高于对照组(P0.05);观察组患者治疗后MOT、PGⅠ及PGⅡ明显高于对照组且差异有统计学意义(P0.05)。阿莫西林联合叶酸可有效提高慢性萎缩性胃炎患者治疗效果,改善MOT、PGⅠ及PGⅡ水平。     

胃溃疡性病变与血清胃蛋白酶原水平的关系

测定97例溃疡型胃癌(GC)和135例胃溃疡(GU)患者的血清PG水平,并与39例胃黏膜基本正常者(NOR)作对比分析.血清PGⅠ水平在GU和GC组明显低于NOR组(P均＜0.01),而GC组又明显低于GU组(P＜0.01);PGⅡ在GC组明显低于NOR组(P＜0.05);PGⅠ/PGⅡ值GC组明显低于NOR组和GU组(P均＜0.01).胃底胃体癌患者血清PGⅠ水平明显低于贲门癌患者(P＜0.05).提示血清PGⅠ水平和PGⅠ/PGⅡ值降低是溃疡型GC的危险因素,检测血清PG水平有助于临床对胃良恶性溃疡的鉴别诊断.     

基于磁珠的胃蛋白酶原Ⅰ和Ⅱ双标记时间分辨荧光免疫分析试剂的研制

针对胃癌及胃病患者血清中胃蛋白酶原Ⅰ(pepsinogenⅠ,PGⅠ)和胃蛋白酶原Ⅱ(pepsinogenⅡ,PGⅡ),研制以磁珠为载体的基于镧系元素铕和钐双标记时间分辨荧光免疫分析技术的检测试剂,能够同时检测患者血清中的PGⅠ和PGⅡ.实验采用双抗体夹心一步法反应模式,铕和钐作为高灵敏的荧光示踪物分别标记抗PGⅠ及PGⅡ抗体,并利用磁珠作为抗原抗体反应的载体,建立双标记磁珠时间分辨荧光免疫分析试剂,并对试剂的各项指标进行评价.实验结果表明,该试剂检测PGⅠ和PGⅡ的分析灵敏度分别达到0.15 ng/m L和0.16 ng/m L,检测范围分别为0.15 500.00 ng/m L和0.16 50.00 ng/m L,准确度和精密性指标均达到临床检测要求,61份临床考核样本经本试剂和雅培化学发光试剂测定,结果一致性较高,无显著差异.实验阐明了磁珠作为固相载体结合经典的时间分辨荧光免疫分析技术在检测PGⅠ和PGⅡ方面的应用潜能,该研究将在多种胃病的诊断、监测及高通量血液筛查等领域发挥巨大的临床应用价值.     

PG与HP检测相关性的探讨

目的:分析HP感染与PG水平间存在的相关性.方法:对HP检测为阳性的标本采用ELISA法检测PG Ⅰ、PG Ⅱ和PG Ⅰ/PG Ⅱ比值;对于PG Ⅰ、PG Ⅱ和PG Ⅰ/PG Ⅱ比值异常的标本分别采用金标法检测HP抗体.结果:HP阳性组与对照组相比较,PG Ⅰ、PG Ⅱ和PG Ⅰ/PG Ⅱ比值均有显著差异(P＜0.01);PG Ⅰ、PG Ⅱ和PG Ⅰ/PG Ⅱ异常组与对照组相比较,HP抗体阳性率均无显著差异(P＞0.01).结论:我国是胃癌发病率较高的国家之一,因此建议检出HP阳性的病例均要进行PG检测,PG的优先检测对胃病或胃癌的防治和诊断有重要意义.     

胃液成分多参数逐步判别在胃病判别中的意义

根据胃液成分的6项参数,用POMS程序,就6种(510例)慢性胃病,按3种组合进行Bayes准则逐步判别。结果显示:1.胃癌、胃溃疡和萎缩性胃炎之间具有十分显著的差异,有判别作用的参数依次为DNA、胆汁酸以及蛋白酶和游离酸;其判别距阵示萎缩性胃炎的符合率达78％。2.胃癌、胃溃疡和十二指肠溃疡3种病之间,胃液参数判别的差异亦十分显著;粘液、DNA、胆汁酸及pH值的判别效果有显著性;胃癌和十二指肠溃疡的判别符合率分别为63.3％及92％。3.浅表性、浅表-萎缩性和萎缩性胃炎进行判别时差异有显著意义,本组有判别作用的参数则依次为粘液、胆汁酸以及蛋白酶和pH值4项;判别距阵显示的符合率,萎缩性胃炎为51％,浅表性胃炎为75.8％,而浅表-萎缩性仅13％,表明后者具有很高的重叠性。     

萎缩性胃炎白细胞介素-1β水平和幽门螺杆菌感染的意义

目的探讨白细胞介素-1β(IL-1β)在萎缩性胃炎发病机制中的作用,以及IL-1β和幽门螺杆菌(HP)感染的关系.方法选择浅表性胃炎患者52例,萎缩性胃炎患者50例.试验分4组:A组:浅表性胃炎并HP阴性;B组:浅表性胃炎并HP阳性;C组:萎缩性胃炎并HP阴性;D组:萎缩性胃炎并HP阳性.采用酶联免疫吸附实验法(ELISA法)检测患者血清IL-1β水平,采用14C-尿素呼吸实验方法(14C-uBT)判定HP感染情况.结果萎缩性胃炎患者血清IL-1β水平均高于浅表性胃炎,尤其萎缩性胃炎伴HP感染阳性患者血清IL-1β水平升高明显,显著高于浅表性胃炎患者(P<0.01).与无HP感染的萎缩性胃炎患者相比,其血清IL-1β水平也明显升高(P<0.05).血清IL-1β水平在浅表性胃炎患者中有HP感染比无HP感染的患者也有升高趋势,但无显著性差异(P>0.05).结论在萎缩性胃炎的发病机制中IL-1β是关键的功能细胞因子,并可能是HP感染导致萎缩性胃炎的相关发病机制之一.     

用酶标凝集素检测胃液中的胃蛋白酶

采用辣根过氧化物酶（ＨＲＰ）标记ＡＬＬ，通过夹心酶联法检测各种胃病患者胃液中的胃蛋白酶。结果：胃蛋白酶在正常组（１５例）、胃炎组（２８例）、十二指肠溃疡组（２５例）、胃溃疡组（２５例）及胃癌组（１３例）中的含量无显著性差异。但胃液ｐＨ值≥５的胃蛋白酶含量明显高于ｐＨ值＜４的酶含量。表明胃粘膜屏障的局部缺陷使之对胃蛋白酶抵抗能力下降，可能与消化性溃疡的发生关系更为密切。     

胃癌与胃幽门螺杆菌关系的探讨

本文分析了胃癌与胃幽门螺杆菌(HP)的关系,结果表明胃癌组与胃癌相关性胃炎组(包括萎缩性胃炎伴肠化、萎缩性胃炎伴异型增生)之间具有显著性差异(P<0.05).提示HP的感染是胃癌的危险因素.     

Markedness and its Interpretation in Language Use

Language markedness is a common phenomenon in languages, and is reflected from hearing, vision and sense, i.e. the variation in the three aspects such as phonology, morphology and semantics. This paper focuses on the interpretation of markedness in language use following the three perspectives, i.e. pragmatic interpretation, psychological interpretation and cognitive interpretation, with an aim to define the function of markedness.

Some differences are due to age, gender, statue and personality. Above all, gender is one of the obvious reasons. Lower Paleozoic oil relationships within Williston Basin, Canada

The Williston Basin is a significant petroleum province, containing oil production zones that include the Middle Cambrian to Lower Ordovician, Upper Ordovician, Middle Devonian, Upper Devonian and Mississippian and within the Jurassic and Cretaceous. The oils of the Williston Basin exhibit a wide range of geochemical characteristics defined as "oil families", although the geochemical signature of the Cambrian Deadwood Formation and Lower Ordovician Winnipeg reservoired oils does not match any "oil family". Despite their close stratigraphic proximity, it is evident that the oils of the Lower Palaeozoic within the Williston Basin are distinct. This suggests the presence of a new "oil family" within the Williston Basin. Diagnostic geochemical signatures occur in the gasoline range chromatograms, within saturate fraction gas chromatograms and biomarker fingerprints. However, some of the established criteria and cross-plots that are currently used to segregate oils into distinct genetic families within the basin do not always meet with success, particularly when applied to the Lower Palaeozoic oils of the Deadwood and Winnipeg Formation.     

低渗透非均质砂岩油藏启动压力梯度研究

理论推导与室内实验相结合,建立了低渗透非均质砂岩油藏启动压力梯度确定方法。首先借助油藏流场与电场相似的原理,推导了非均质砂岩油藏启动压力梯度计算公式。其次基于稳定流实验方法,建立了非均质砂岩油藏启动压力梯度测试方法。结果表明:低渗透非均质砂岩油藏的启动压力梯度确定遵循两个等效原则。平面非均质油藏的启动压力梯度等于各级渗透率段的启动压力梯度关于长度的加权平均;纵向非均质油藏的启动压力梯度等于各渗透率层的启动压力梯度关于渗透率与渗流面积乘积的加权平均。研究成果可用于有效指导低渗透非均质砂岩油藏的合理井距确定,促进该类油藏的高效开发。
