消化内镜在上消化道出血治疗中的临床应用效果

分析消化内镜在上消化道出血治疗中的临床应用效果、并发症发生情况。选择2016年5月—2019年7月甘肃省徽县人民医院接治的上消化道出血患者78例,采取抽样法分组,即采纳常规治疗的对照组（n=39）和消化内镜治疗的观察组（n=39）,对比治疗效果,止血时间及再出血率。观察组有效率（97.44%）较对照组（82.05%）更高,P<0.05;观察组止血时间较对照组更短,P<0.05;观察组再出血率（5.12%）明显较对照组（20.51%）低,P<0.05;观察组并发症发生率（2.56%）明显较对照组（17.95%）更低,P<0.05;观察组不良反应发生率（2.56%）明显较对照组（20.51%）低,P<0.05;观察组住院时间（3.58±1.04） d明显较对照组（5.67±1.94） d更短,组间对比差异显著,P<0.05。对上消化道出血患者实施消化内镜治疗方式,有效地缩短了止血时间,具有安全性和可靠性,在临床中值得应用和推广。     

介入栓塞治疗术在消化道大出血当中的应用效果观察

探讨介入栓塞治疗术在消化道大出血治疗中的应用效果。选取我院收治的急性消化道大出血60例,按不同治疗方法分为对照组和观察组,各30例。对照组接受常规药物治疗,观察组在药物治疗基础上联合介入栓塞治疗,比较两组治疗效果、隐血转阴时间和住院时间,观察两组并发症发生情况。对照组有效止血20例,有效率为66.67%,成功止血时间为(3.98±1.12)d;观察组有效止血28例,有效率为93.33%,成功止血时间为(1.95±0.48)d。观察组有效止血率高于对照组,成功止血时间、隐血转阴时间和住院时间均短于对照组,差异均有统计学意义(P0.05)。术后随访1月两组均无再出血病例,未见因过度栓塞或误栓所致胃肠道坏死等严重并发症。在药物治疗基础上联合介入栓塞治疗能实现快速止血,提高消化性溃疡致急性消化道大出血患者的止血成功率,安全性较高,具有推广应用价值。     

23例 Dieulafoy 病的急诊内镜治疗

目的 :探讨Dirulafoy病的内镜诊断及治疗。方法 :分析我院1989年1月至2004年5月 ,经内镜检查治疗Dieulafoy病的临床资料。结果 :23例通过急诊内镜检查确诊 ,经内镜治疗23例 ,即时止血率100 %。2例经内镜治疗再出血转外科手术 ,再出血率8 7 % ,21例内镜治疗止血成功 ,总止血率91 3 %。结论 :急诊内镜是诊断Dieulafoy病的最佳手段 ,内镜治疗是Dieulafoy病的首选治疗方法     

非静脉曲张性上消化道出血内镜下的诊治

目的：探讨非静脉曲张性上消化道出血急诊内镜的诊治意义及价值．方法：对上消化道出血患者，急诊内镜检查明确病因及出血部位。然后根据不同情况选择适当止血方法，行内镜下止血治疗．结果：191例患者中，182例明确诊断。确诊率97．6％．并对其中63例内镜下可见活动性出血灶的患者行内镜下止血治疗，有效58例，再出血5例，止血有效率为92．1％。再出血率为7．9％．结论：对上消化道出血的患者行急诊内镜的诊治是安全可靠的。确诊率和止血率很高。再出血率相对较低，值得临床广泛应用．     

云南白药保险子在重度创伤性失血急救中的应用

创伤性失血是急救领域的重要挑战,可导致凝血-抗凝系统紊乱,云南白药保险子是中医经典急救药物,兼有止血和抗凝的功效。以2018年1月至2021年12月期间在呼和浩特市第一医院急诊中心收治的骨科创伤患者为研究对象,通过前瞻性平行对照临床研究,对保险子的止血疗效进行评价,指标包括术中出血量、术后引流量、血红蛋白下降值、凝血酶原时间（PT）、激活部分凝血酶原时间、纤维蛋白原（FBG）、输血率,预后出血/凝血不良反应等。结果显示,保险子组（n=27）的术中出血量、术后引流量、血红蛋白下降值、输血率、PT以及预后出血/凝血不良反应均显著小于对照组（n=24）,保险子组FBG显著高于对照组;说明保险子能够维持机体凝血-抗凝系统的平衡,在显著降低出血量的同时,兼具抗凝的功效,可预防创伤性凝血病,减少预后不良反应。     

内镜下套扎治疗食管静脉曲张疗效分析

对40例肝硬化食管静脉曲张破裂出血的病人进行急诊或择期的内镜下食道曲张静脉套扎术,急诊止血率100％,再出血率5％,止血成功率95％,静脉曲张消失率45％,红色征阳性消失率70％,疗效优于内科药物和硬化剂治疗.     

ABC评分在肝硬化食管胃底静脉曲张出血患者预后评价中的应用

目的 评估ABC评分在预测肝硬化患者因消化道出血导致的早期高病死率风险以及再出血风险方面的表现,并将其与Child-Pugh评分、MELD评分进行比较。方法 选取2018年1月—2021年3月湖北省第三人民医院收治的肝硬化静脉曲张出血住院患者95例,记录了患者在临床住院后首次的ABC评分、CTP评分及MELD评分。用ROC曲线下面积比较3种评分方法 对早期高病死率风险以及再出血风险评估方面的表现。结果 在诊断食管胃底静脉曲张出血患者的30天死亡预后价值上,ABC评分（AUC=0.917）明显优于CTP评分（AUC=0.817）及MELD评分（AUC=0.768）。并且ABC评分对预测食管胃底静脉曲张出血患者出现再出血方面有一定的诊断价值,曲线下面积为0.720。CTP评分及MELD评分对静脉曲张出血患者出现再出血方面的预后无明显诊断价值,曲线下面积分别为0.562、0.578。结论 ABC评分预测肝硬化食管静脉曲张破裂出血患者住院30天内死亡有较佳的诊断价值,优于CTP评分与MELD评分。并且ABC评分对预测静脉曲张出血患者再出血方面有一定的诊断价值。     

血液病患者红细胞输血发生免疫溶血因素分析

目的：多因素Logistic回归分析血液病患者红细胞输血效果影响因素,为血液病患者精准输血提供实验依据。方法：回顾性调查2019年8月～2021年1月兰州大学第一医院血液科接受输血治疗的血液病患者834例,其中男性483例,女性351例。利用多变量Logistic回归分析输血效果的显著风险因素。结果：（1）本院血液病患者红细胞输注无效发生率为26.9%(224/834);男性输注无效率为28.4%(137/483),女性输注无效率为24.8%(77/351);(2)不同性别血液病患者红细胞输血效果影响因素结果：妊娠次数（OR=1.813）、本次住院输血袋数（OR=2.701）、既往红细胞输注袋数10～19袋（OR=3.632）、既往红细胞输注袋数≥20袋（OR=7.421）、临床诊断（骨髓增生异常综合征、多发性骨髓瘤,OR=6.923;白血病、淋巴瘤,OR=12.779）与女性血液病患者输注无效显著正相关。本次住院输血袋数（OR=2.824）、既往红细胞输注袋数10～19袋（OR=2.467）、既往红细胞输注袋数≥20袋（OR=2.859）、临床诊断（骨髓增生异常综合征、多发性骨髓瘤,...     

循证护理对急诊PCI患者术后并发症的影响

探究循证护理对急诊冠状动脉介入术(PCI)患者术后并发症的影响。选取2017年7月-2019年7月河西学院附属张掖人民医院收治的急性心肌梗死行PCI介入治疗的患者142例,采用随机数字表法分为2组,采用常规护理的患者设为对照组,采用循证护理的患者设为循证组,对比2组急救时间、住院时间、术后并发症发生率及护理满意度。循证组急救时间与住院时间均短于对照组(P0.05);循证组并发症总发生率5.63%低于对照组23.94%,差异有统计学意义(P0.05);循证组满意度95.77%高于对照组78.87%(P0.05)。循证护理可缩短急诊PCI患者急救时间与住院时间,降低术后并发症发生率,提高患者满意度。     

重症肺炎合并2型心肌梗死患者行序贯通气治疗的临床

目的：探讨重症肺炎合并2型心肌梗死患者行序贯通气治疗的临床应用价值。方法：收集我院ICU收治重症肺炎合并2型心肌梗死患者58例,随机分为无创呼吸机通气组（NV,n=29）和无创通气+高流量吸氧（NV+HFNC,n=29）,分别比较两组在患者拔管后的4h、12h、24h及48 h时心率、呼吸、平均动脉压、氧分压、二氧化碳分压以及48 h内再插管率、48 h后再插管率、ICU住院时间、30 d及90 d死亡率。结果：两组患者在拔管后4h、12h、24h及48h的心率、平均动脉压、氧分压以及48h后发生气管插管再插率、入院30d和90d、死亡率比较无统计学差异（P＞0.05）;NV+HFNC组各时间点的呼吸频率均小于NV组,且在拔出气管插管后的12h的二氧化碳分压小于NV组,有统计学意义（P＜0.05）;NV+HFNC组在拔出气管后的48h内发生气管插管再插率及ICU住院时间均低于NV组,有统计学意义（P＜0.05）。结论：对重症肺炎合并2型心肌梗死患者采用NV+HFNC序贯治疗通气策略可减低拔管后患者呼吸频率及48h内气管插管再插率,减少ICU住院时间,有较高的临床应用价值。     

Markedness and its Interpretation in Language Use

Language markedness is a common phenomenon in languages, and is reflected from hearing, vision and sense, i.e. the variation in the three aspects such as phonology, morphology and semantics. This paper focuses on the interpretation of markedness in language use following the three perspectives, i.e. pragmatic interpretation, psychological interpretation and cognitive interpretation, with an aim to define the function of markedness.

Language is a means of verbal communication. People use language to communicate with each other. In the society, no two speakers are exactly alike in the way of speaking. Some differences are due to age, gender, statue and personality. Above all, gender is one of the obvious reasons. Lower Paleozoic oil relationships within Williston Basin, Canada

The Williston Basin is a significant petroleum province, containing oil production zones that include the Middle Cambrian to Lower Ordovician, Upper Ordovician, Middle Devonian, Upper Devonian and Mississippian and within the Jurassic and Cretaceous. The oils of the Williston Basin exhibit a wide range of geochemical characteristics defined as "oil families", although the geochemical signature of the Cambrian Deadwood Formation and Lower Ordovician Winnipeg reservoired oils does not match any "oil family". Despite their close stratigraphic proximity, it is evident that the oils of the Lower Palaeozoic within the Williston Basin are distinct. This suggests the presence of a new "oil family" within the Williston Basin. Diagnostic geochemical signatures occur in the gasoline range chromatograms, within saturate fraction gas chromatograms and biomarker fingerprints. However, some of the established criteria and cross-plots that are currently used to segregate oils into distinct genetic families within the basin do not always meet with success, particularly when applied to the Lower Palaeozoic oils of the Deadwood and Winnipeg Formation.     

低渗透非均质砂岩油藏启动压力梯度研究

理论推导与室内实验相结合,建立了低渗透非均质砂岩油藏启动压力梯度确定方法。首先借助油藏流场与电场相似的原理,推导了非均质砂岩油藏启动压力梯度计算公式。其次基于稳定流实验方法,建立了非均质砂岩油藏启动压力梯度测试方法。结果表明:低渗透非均质砂岩油藏的启动压力梯度确定遵循两个等效原则。平面非均质油藏的启动压力梯度等于各级渗透率段的启动压力梯度关于长度的加权平均;纵向非均质油藏的启动压力梯度等于各渗透率层的启动压力梯度关于渗透率与渗流面积乘积的加权平均。研究成果可用于有效指导低渗透非均质砂岩油藏的合理井距确定,促进该类油藏的高效开发。
