观察经皮热消融与肝动脉化疗栓塞同步治疗肝癌的临床疗效。选取兰州军区总医院安宁分院收治的肝癌患者94例,并随机将其均分为观察组与对照组(各47例)。对照组采取肝动脉化疗栓塞治疗,观察组联合应用经皮热消融与肝动脉化疗栓塞同步治疗。比较2组患者近期疗效、并发症及生存情况。观察组有效率较对照组高(P0.05);2组并发症发生率比较差异不明显(P0.05);观察组1年、2年及3年生存率均较对照组高(P0.05)。肝癌患者应用经皮热消融与肝动脉化疗栓塞同步治疗,可提高临床疗效,改善患者生存状况。  相似文献

目的:探讨肝癌肝动脉栓塞化疗术后发生上消化道出血的原因及处理方法。方法:回顾性分析152例肝癌介入治疗后11例上消化道出血的发生原因和治疗过程。全部病例均采用插导管至肝动脉内注入碘油化疗药物混悬剂(10ml~30ml)。结果:11例中出血发生在介入治疗后3d内有7例,3~7d4例。经内科治疗后8例出血停止,3例死亡。结论:碘油逆流入门静脉引起或加重原有门静脉高压是出血的主要原因,药物止血和降低门脉压力是治疗该症的主要方法,术前术中采取预防措施可有效降低出血的发生。  相似文献

肝癌是消化系统常见的恶性肿瘤,多数患者发现时已到中晚期,其死亡率排在恶性肿瘤死亡率的第3位,在我国每年有11万人死于肝癌。近年来,随着医疗技术的不断发展,肝动脉化疗栓塞治疗逐渐成为中晚期肝癌治疗的首选方案,其特点是准确性高、创伤小、出血少,可延缓肿瘤生长,延缓患者的生存期,并提高患者的生活质量。肝癌介入治疗的过程中,有效的心理护理,充分的术前准备,良好的术后护理可减少并发症的发生,使患者术前术后心理健康、平稳,对治疗积极配合,是肝动脉化疗栓塞治疗成功的重要因素之一。  相似文献

原发性肝癌化疗栓塞严重并发症原因分析   总被引:5,自引:0,他引:5  
目的:探讨肝动脉化疗栓塞(TACE)严重并发症原因。方法:选择原发性肝癌456例,行734次介入治疗,观察疗效并记录并发症。结果:发生严重并发症24例,发生率为5.4%,并发症包括上消化道大出血13例、肝癌破裂出血4例、肺梗死4例、截瘫2例、急性肝肾功能衰竭1例;死亡19例,死亡4.1%;总体1、2、3、4、5年生存率分别为47.1%、38.5%、9.7%、3.1%、1.6%,最长1例存活8年3个月。结论:TACE是中晚期肝癌有效的治疗方法,但在行介入时应严格掌握适应证,熟悉病理状态下肝血管短路走向,熟练应用介入操作技术,减少严重并发症发生。  相似文献

目的:观察原发性肝癌选择性肝动脉化疗栓塞术(TACE)后的并发症.方法:观察总结在我科住院56例肝癌病人行TACE术后的并发症及处理.结果:524例(占93%)有腹痛,48例(占86%)有发热,16例(占29%)有恶心呕吐,29例(占52%)AST升高,6例(占11%)血细胞减少.结论:肝动脉化疗栓塞术后的并发症少,化疗药物副作用小.  相似文献

评价肝动脉化疗栓塞治疗肝癌侵犯门静脉的疗效和安全性。方法;采用选择性肝动脉插管,造影证实靶血管存在后,先注入适量碘化油-抗癌药乳化剂栓塞肿瘤血和,再注入适量三联抗癌药。根据肝癌侵犯门静脉的大小,将其分为局限型和弥漫型两类,并对这两类肝癌经TACE治疗后的初期和长期疗效及安全性进行随访。  相似文献

原发性肝癌是世界上常见的恶性肿瘤之一,我国每年约有22万人死于原发性肝癌[1]。利用介入放射学进行肝动脉灌注化疗药物和血管栓塞,已达到临床治疗肝癌的目的,这是目前临床治疗中、晚期原发性肝癌的非手术方法,通过阻断肿瘤的血供,使瘤体严重缺血和坏死而缩小。从2004年8月至2006年6月,我科对42例中、晚期原发性肝癌病人采取了此疗法,取得了较好的疗效,现将护理体会介绍如下。  相似文献

理论推导与室内实验相结合,建立了低渗透非均质砂岩油藏启动压力梯度确定方法。首先借助油藏流场与电场相似的原理,推导了非均质砂岩油藏启动压力梯度计算公式。其次基于稳定流实验方法,建立了非均质砂岩油藏启动压力梯度测试方法。结果表明:低渗透非均质砂岩油藏的启动压力梯度确定遵循两个等效原则。平面非均质油藏的启动压力梯度等于各级渗透率段的启动压力梯度关于长度的加权平均;纵向非均质油藏的启动压力梯度等于各渗透率层的启动压力梯度关于渗透率与渗流面积乘积的加权平均。研究成果可用于有效指导低渗透非均质砂岩油藏的合理井距确定,促进该类油藏的高效开发。  相似文献
