残胃236例临床内镜分析

本文报告全部经胃镜和病理检查证实的手术后胃２３６例．ＢｉｌｌｒｏｔｈⅠ式残胃（Ｂ－Ⅱ式）２４例，ＢｉｌｌｒｏｔｈⅡ式残胃（Ｂ－Ⅱ式）２１２侧．结果显示吻合口炎及残胃炎发生率为最多；病理以慢性炎症为最多，其次为不典型增生病变．检出残胃癌１２例（其中手术原因不明者３例）．残胃复发癌１２例．并对残胃癌、术式等问题进行讨论．认为术前术式选择宜从严掌握．术后必须内镜随访，尤其对胃癌术后、残胃癌前病变应定期胃镜及病理检查．     

消化内镜检查1142例

介绍内镜检查１１４２例并探讨其病理和病因。上消化道疾病以慢性浅表性胃炎最常风。慢性萎缩性胃炎，胃息肉，慢性胃溃疡，残胃为胃癌前期状态。应定期随访。     

幽门螺杆菌与胃肠病的关系及致病机理

幽门螺杆菌（ＨＰ）是慢性活动性胃炎、消化性溃疡致病菌，胃（ＭＡＬＴ）淋巴瘤的致病因子，亦可能是胃癌协同致癌因子，与功能性消化不良的关系有待明确，与其他疾病也有关联。ＨＰ致病主要依靠其在胃粘膜的定居能力和致病因子，很可能是胃粘膜免疫反应的结果。血浆和胃液的生长抑素和表皮生长因子含量下降，粘液结构的破坏、脱落，以及胃粘膜血流量下降等促使溃疡发生。长期慢性炎症导致肠化，异型增生直至胃癌，细胞的异常增殖、凋亡，以及癌基因的过度表达，抑癌基因的缺失等，增加了胃癌发生的危险性。     

视黄酸对胃癌细胞恶性表型的影响

研究ｔ－ＲＡ对胃癌细胞的作用和探讨其作用机理．结果表明：１）ｔ－ＲＡ能够有效地抑制胃癌细胞的生长．２）ｔ－ＲＡ能够降低胃癌细胞在软琼脂中形成集落的能力．３）ｔ－ＲＡ能够降低胃癌细胞的粘附能力．４）ｔ－ＲＡ能够抑制胃癌细胞在裸鼠中形成肿瘤的能力．４）扫描电镜观察显示,经ｔ－ＲＡ处理后,胃癌细胞表面的微绒毛消失．以上结果表明,ｔ－ＲＡ对胃癌细胞的恶性表型有显著的抑制作用．ｔ－ＲＡ可能作为治疗胃癌的有效药物．     

胃动力中药的理论与实践探讨

胃动力障碍是多种慢性胃肠道疾病常见的病理反应之一，亦可是某些疾病的并发症，或化疗、放疗的副作用。其特征是胃肠蠕动功能减缓、胃排空延迟而使消化功能降低。临床上如：慢性胃炎、胃溃疡、胃食管返流、空肠息室、慢性小肠假性梗阻等胃肠道疾病，以及糖尿病胃轻瘫综合...     

清除幽门螺杆菌可有效降低胃癌发病率

北京大学临床肿瘤学院游伟程研究小组历经10年研究证实,清除胃内幽门螺杆菌感染,可使胃癌癌前病变及胃癌的发病风险降低40％。这是目前世界上针对胃癌癌前病变开展的干预试验中样本量最大、时间最长的研究,具有系统性和原创性,在国内外肿瘤研究中尚属首次,为胃癌的预防奠定了理论基础,提供了重要科学依据。研究小组采用大样本和前瞻性研究方法．在山东临朐县胃癌高发区对3400名成人开展了研究。研究表明,幽门螺杆菌感染是胃癌发生的重要危险因素,清除幽门螺杆菌感染能有效降低慢性萎缩性胃炎、肠上皮化生、异型性增生等癌前病变及胃癌的发生。与安慰剂组比较。清除幽门螺杆菌组发生上述癌前病变和胃癌的危险度下降40％。研究还证实血清高水平VC能阻止癌前病变的进展,是降低胃癌危险性的保护性因素。     

无锡地区巨幼红细胞性贫血57例病因的临床分析

目的分析近年来无锡地区巨幼红细胞性贫血（MGA）患者的常见病因及临床意义．方法对无锡地区57例MGA患者的常见病因进行回顾性研究．结果导致MGA的常见病因如下：长期慢性萎缩性胃炎病史导致摄入不足、膳食异常、全胃及胃大部切除、慢性腹泻、小肠切除、长期酗酒、慢性肝病、甲亢及病因不明等．该病在无锡地区以中老年为多，病因多与器质性疾病有关；而膳食异常则为发病患者的另一重要原因，以年轻患者及女性患者为多．结论无锡地区导致MGA的病例以老年患者为多见，其中器质性疾病为主要病因；而膳食异常则是导致青年及女性患病的重要原因．     

70例老年巨幼细胞贫血病因分析

1　临床资料70例患者中男 42例 ,女 2 8例 ,其临床表现和血液学改变均符合《血液病诊断及疗效标准》 ,在确诊巨幼细胞贫血后 ,所有病例均予胃镜或肠镜检查 .老年巨幼细胞贫血与消化道疾病及摄食密切相关 .由于摄入不足造成贫血的 2 5例 ,吸收不良 40例 ,其中胃大部分切除或全胃切除史 1 9例 ,8例有慢性萎缩性胃炎 ,4例伴有肠上皮化生 ,5例为小肠肿瘤 ,2例为急性非淋巴细胞白血病 (Ｍ2ｂ型 ) ,2例有慢性胰腺炎病史 .药物影响 5例 ,其中 4例因有风湿、类风湿疾病长期服用秋水仙大碱等抗风湿药物 ,1例有癫痫长期服用苯巴比妥类药物 .2　讨　　…     

胃癌及癌前病变患者外周血T淋巴细胞亚群的表达及意义

目的 探讨胃癌及癌前病变患者外周血T淋巴细胞亚群的表达情况.方法 收集20例慢性浅表性胃炎患者、20例胃癌前病变患者、38例胃癌患者血清标本,应用流式细胞仪检测外周血T淋巴细胞亚群表达水平,并进一步分析胃癌患者在不同分期、不同组织学分型及有或无淋巴结转移的T淋巴细胞亚群表达情况.结果 随着胃部疾病从浅表性胃炎-胃癌前病变-胃癌的逐渐进展,以及胃癌TNM分期的增加,胃癌发生淋巴结转移,外周血CD3+,CD4+T淋巴细胞表达逐渐降低,CD8+,CD25+T淋巴细胞表达逐渐升高.结论 外周血T淋巴细胞亚群的表达与胃癌发生、发展密切相关.     

胃癌的自体荧光特征谱

利用激光诱导荧光法（laser-induced auto fluorescence,LIAF)选择多种波长测试和比较了胃癌和胃正常组织标本，得到区分癌变组织与正常组织的最佳激发波长；选取光谱特征差异最显著的波段425－475nm，采用形状识别技术分析31例胃癌及胃正常组织的自体荧光光谱曲线，经去噪声，曲线拟合，求区间最大曲率，最终以区间最大曲率的理想平均值作为区分阈值标准，获得高达93．5％的敏感性和100％的特异性，为临床实用奠定了基础。     

Markedness and its Interpretation in Language Use

Language markedness is a common phenomenon in languages, and is reflected from hearing, vision and sense, i.e. the variation in the three aspects such as phonology, morphology and semantics. This paper focuses on the interpretation of markedness in language use following the three perspectives, i.e. pragmatic interpretation, psychological interpretation and cognitive interpretation, with an aim to define the function of markedness.     

Features of Women's language

Language is a means of verbal communication. People use language to communicate with each other. In the society, no two speakers are exactly alike in the way of speaking. Some differences are due to age, gender, statue and personality. Above all, gender is one of the obvious reasons. Lower Paleozoic oil relationships within Williston Basin, Canada

The Williston Basin is a significant petroleum province, containing oil production zones that include the Middle Cambrian to Lower Ordovician, Upper Ordovician, Middle Devonian, Upper Devonian and Mississippian and within the Jurassic and Cretaceous. The oils of the Williston Basin exhibit a wide range of geochemical characteristics defined as "oil families", although the geochemical signature of the Cambrian Deadwood Formation and Lower Ordovician Winnipeg reservoired oils does not match any "oil family". Despite their close stratigraphic proximity, it is evident that the oils of the Lower Palaeozoic within the Williston Basin are distinct. This suggests the presence of a new "oil family" within the Williston Basin. Diagnostic geochemical signatures occur in the gasoline range chromatograms, within saturate fraction gas chromatograms and biomarker fingerprints. However, some of the established criteria and cross-plots that are currently used to segregate oils into distinct genetic families within the basin do not always meet with success, particularly when applied to the Lower Palaeozoic oils of the Deadwood and Winnipeg Formation.     

低渗透非均质砂岩油藏启动压力梯度研究

理论推导与室内实验相结合,建立了低渗透非均质砂岩油藏启动压力梯度确定方法。首先借助油藏流场与电场相似的原理,推导了非均质砂岩油藏启动压力梯度计算公式。其次基于稳定流实验方法,建立了非均质砂岩油藏启动压力梯度测试方法。结果表明:低渗透非均质砂岩油藏的启动压力梯度确定遵循两个等效原则。平面非均质油藏的启动压力梯度等于各级渗透率段的启动压力梯度关于长度的加权平均;纵向非均质油藏的启动压力梯度等于各渗透率层的启动压力梯度关于渗透率与渗流面积乘积的加权平均。研究成果可用于有效指导低渗透非均质砂岩油藏的合理井距确定,促进该类油藏的高效开发。
