手术治疗老年人桡骨远端骨折

探讨手术治疗复杂桡骨远端骨折的疗效。根据骨折类型及特点,对66例复杂桡骨远端骨折采用切开复位,取同侧腓骨或髂骨填充骨缺损,支撑桡骨远端关节面,恢复其高度及平整性,同时恢复掌倾角,尺偏角,然后采用微型T形锁定加压钢板(LCP)或克氏针联合外固定支架固定。通过随访观察骨折愈合时间,腕关节功能活动范围,术后掌倾角、尺偏角的平均角度及握力,应用Garland and Werley评分。本组随访时间6~24个月,平均15个月,所有骨折均获得满意效果。以Garland and Werley评分方法评定腕关节功能:优32例,良25例,可7例,差1例,优良率为87.69%。采用掌侧入路复位关节面,矫正掌倾角,尺偏角,并根据骨折类型及特点选择合适的固定方式或植骨,是治疗复杂桡骨远端骨折的有效方法。     

切开复位接骨板内固定治疗掌侧Barton骨折19例临床分析

目的探讨桡骨远端Barton骨折的手术方法及疗效观察.方法19例闭合掌侧Barton骨折,所有病例均为手法复位失败者.均采用掌侧人路,使用普通接骨板12例,国产锁定接骨板7例.结果19例在6～18月后得到随访,骨折全部愈合.X线片示桡骨远端的掌倾角和尺偏角均恢复至正常范围,桡骨远端关节面恢复程度好.综合评分优17例,占73%;良2例,占23%.优良率达100%.出现环指伸肌腱卡压1例,取出此螺钉后症状消失.结论桡骨远端Barton骨折采用掌侧接骨板内固定治疗,可以较好地维持骨折复位,允许术后早期功能锻炼,并可获得满意的腕关节功能恢复.     

尺骨茎突骨折对Colles骨折手法复位后疗效影响

评价尺骨茎突骨折对Colles骨折经手法复位石膏夹板固定治疗后腕关节功能的影响。回顾分析2014年12月-2017年12月在武威市中医医院治疗98例Colles骨折患者并发尺骨茎突骨折采用手法复位石膏夹板固定治疗的诊疗资料,选择同期未合并尺骨茎突骨折采用手法复位石膏夹板固定治疗的98例患者作为对照组。对比两组患者的相关诊疗资料,评价临床症状与体征积分,统计两组临床疗效及不良反应,所有数据均采用SPSS 17软件进行分析。两组均经保守治疗并随访6个月,复查X线片骨折均得到骨性愈合。观察组87例骨折解剖相对,腕关节运动正常,8例掌倾角6～9度,尺偏角15～20度,3例掌倾角小于6度,尺偏角小于15度;对照组90例骨折位于解剖位置,腕关节正常,6例掌倾角6～9度,尺偏角15～20度,2例掌倾角小于6度,尺偏角小于15度;临床症状与体征积分改善程度观察组相比对照组无显著性差异,P>0.05;治疗期间不良反应轻微。是否合并尺骨茎突骨折对Colles骨折经保守后腕部功能没有显著差异。     

掌侧入路锁定钢板内固定用于桡骨远端骨折的临床效果分析

分析掌侧入路锁定钢板内固定用于桡骨远端骨折的临床效果。将本院收治的桡骨远端骨折患者92例随机划分为对照组和对照组,对照组(n=46)给予背侧入路锁定钢板内固定治疗,观察组(n=46)采取掌侧入路锁定钢板内固定治疗。对比两组腕关节功能恢复情况、手术情况以及并发症发生率。观察组腕关节功能恢复优良率91.30%较对照组的71.74%高(P0.05;观察组手术用时、术后活动时间、住院天数均较对照组少(P0.05);观察组并发症发生率2.17%略较对照组的10.87%,但比较差异不明显(P0.05)。对桡骨远端骨折患者实施掌侧入路锁定钢板内固定治疗,可大大改善患者腕关节功能,且具有手术用时短、术后活动时间早、恢复快等优势。     

掌侧锁定钢板结合背侧经皮克氏针翘拨治疗桡骨远端前后壁骨折的疗效评估

摘要 为探讨掌侧锁定钢板结合背侧经皮克氏针翘拨治疗桡骨远端前后壁骨折的临床疗效,通过回顾性研究方法,纳入从2014年1月至2018年1月收入治疗的桡骨远端前后壁均骨折的患者。最终共收入43例患者。其中男性16例,女性27例。平均年龄为64.39±4.85岁（范围51岁至74岁）。其中32例患者为自行摔伤,11例患者为交通事故伤。记录患者手术时长,术中出血量,术后4周,12周,24周视觉模拟评分,以及患者并发症,测量患者术后24周X线尺偏角,掌倾角,双手握力,腕关节主动活动范围及上肢功能评定标准(disabilities of the arm, shoulder and hand,DASH)评分、Gartland-Werley腕关节评分。结果表明手术时长平均为82.63±9.2 min,术中出血量平均为30.47±15.42 ml。术后4周VAS评分为3.98±0.8分,术后12周VAS评分为3.07±0.88分,术后24周VAS评分为1.93±1.24分,X线尺偏角为17.23±3.54度,X线掌倾角为11.83±1.35度,DASH评分为7.6±2.32分,Gartland-Werley腕关节评分为5.22±1.73分。握力为健侧38.56±5.96 kg,患侧34.23±6.27度,掌屈患侧70.34±7.49度,健侧79.59±6.16度,背伸患侧65.50±7.78度,健侧66.36±8.02度,桡偏患侧16.84±3.92度,健侧20.60±4.28度,尺偏患侧22.22±6.60度,健侧23.47±6.64度,旋前患侧70.75±7.97度,健侧74.75±8.97度,旋后患侧63.35±9.39度,健侧64.57±8.54。没有患者出现伤口感染、骨折复位丢失,以及内固定物断裂等并发症。可见掌侧锁定钢板结合背侧经皮克氏针翘拨治疗桡骨远端前后壁骨折的疗效满意,值得临床推广。     

老年桡骨远端不稳定骨折手术及术后护理的临床观察

讨论老年桡骨远端不稳定骨折的手术治疗方案及术后相应的护理干预措施。筛选出2018年4月至2019年5月于我院实施桡骨远端不稳定骨折诊治处理的老年患者50例,给予全部患者切开复位掌侧锁定压缩板系统接骨板来进行治疗,并于术后对患者实施相应的护理干预措施。记录、观察患者的临床诊治结局。接受治疗及护理后,患者手术的相关指标评分、腕关节功能均得以明显改善(P0.05)。对老年桡骨远端不稳定骨折患者运用切开复位掌侧锁定压缩板系统接骨板实施诊治处理,并于术后实施相应的护理干预措施,临床诊治结局甚佳,对患者腕关节功能的快速恢复具有重大意义。     

可调式外固定架固定治疗老年人桡骨远端骨折的临床观察

为评价手法复位可调式外固定架固定治疗老年人桡骨远端骨折的疗效,以2003年1月～2006年1月应用可调式外固定架固定治疗老年人桡骨远端骨折32例,术后随访3个月～3年,以临床疗效和X线片表征为指标,按照Jakim评分系统进行疗效评分.结果评定为优21例,良9例,优良率为93.8%.说明手法复位可调式外固定架固定是一种治疗老年桡骨远端骨折的理想方法.     

克氏针撬拨复位经皮穿针 治疗大龄儿童桡骨远端骨折临床研究

目的探讨克氏针撬拨复位经皮穿针治疗大龄儿童桡骨远端骨折的疗效.方法 36例桡骨远端骨折患儿均采用克氏针撬拨复位经皮穿针固定,并对疗效进行分析评价.结果本组36例患儿均随访,平均随访时间为10个月,随访结果:伤肢外观无瘢痕及畸形,骨折骨性愈合,腕关节功能完全恢复.结论克氏针撬拨复位经皮穿针治疗大龄儿童桡骨远端骨折损伤小,复位佳,固定可靠,疗效显著.     

尺骨茎突骨折是否愈合对桡骨远端骨折术后腕关节功能的影响对比

对比分析尺骨茎突骨折是否愈合对桡骨远端骨折内固定术后腕关节功能的影响。2013年1月至2016年3月收治136例单侧不稳定的桡骨远端骨折伴尺骨茎突骨折患者,根据尺骨茎突骨折愈合情况将患者分为尺骨茎突骨折愈合组(愈合组,43例)与尺骨茎突骨折未愈合组(未愈合组,93例)。末次随访时采用Gartlant-Werley评分和臂肩手功能障碍DASH评分评估两组腕关节功能,测定并记录腕关节的活动范围及握力。两组患者背伸、掌曲、桡偏、尺偏、握力和DASH评分比较差异均无统计学意义(P0.05);愈合组Gartlant-Werley评分的优良率为90.7%(39/43),未愈合组优良率为91.4%(85/93),两组优良率比较差异无统计学意义(X~2=0.294,P0.05)。尺骨茎突骨折是否愈合在短期内对桡骨远端骨折内固定术后腕关节功能的恢复无明显影响。     

左Colles骨折合并腕关节及尺桡下关节脱位1例

左桡骨远端骨折,即Colles骨折为骨折中常见病。但合并腕关节脱位较少见,而同时伴有尺桡下关节脱位则罕见。我院于1995年4月门诊治疗1例,现报告如下。 患者男,18岁,学生,1995年4月12日在学校要篮球争球时不慎扑倒,左腕部着地,当时腕部疼痛,手指麻木,不能活动,急来我院就诊。检查:左腕部畸形、肿胀、呈“锅铲状”,触痛,可触及骨擦感。手部皮肤感觉及末梢血运良好,腕关节功能丧失,手指活动受限。门诊拍X线片(05825);左桡骨远端骨折,远折段向背侧移位1/2,近捧腕骨向背侧移位与桡骨远端相错,尺桡骨下端骨间隙增宽,尺骨小头向掌侧移位。诊断:左Colles骨折合并腕关节及尺桡下关节脱位。 治疗方法:局麻下行手法复位。先整复腕关节脱位。助手双手握住前臂肘下方,术者双手握病人左手先行对抗牵引     

Markedness and its Interpretation in Language Use

Language markedness is a common phenomenon in languages, and is reflected from hearing, vision and sense, i.e. the variation in the three aspects such as phonology, morphology and semantics. This paper focuses on the interpretation of markedness in language use following the three perspectives, i.e. pragmatic interpretation, psychological interpretation and cognitive interpretation, with an aim to define the function of markedness.

Some differences are due to age, gender, statue and personality. Above all, gender is one of the obvious reasons. Lower Paleozoic oil relationships within Williston Basin, Canada

The Williston Basin is a significant petroleum province, containing oil production zones that include the Middle Cambrian to Lower Ordovician, Upper Ordovician, Middle Devonian, Upper Devonian and Mississippian and within the Jurassic and Cretaceous. The oils of the Williston Basin exhibit a wide range of geochemical characteristics defined as "oil families", although the geochemical signature of the Cambrian Deadwood Formation and Lower Ordovician Winnipeg reservoired oils does not match any "oil family". Despite their close stratigraphic proximity, it is evident that the oils of the Lower Palaeozoic within the Williston Basin are distinct. This suggests the presence of a new "oil family" within the Williston Basin. Diagnostic geochemical signatures occur in the gasoline range chromatograms, within saturate fraction gas chromatograms and biomarker fingerprints. However, some of the established criteria and cross-plots that are currently used to segregate oils into distinct genetic families within the basin do not always meet with success, particularly when applied to the Lower Palaeozoic oils of the Deadwood and Winnipeg Formation.     

低渗透非均质砂岩油藏启动压力梯度研究

理论推导与室内实验相结合,建立了低渗透非均质砂岩油藏启动压力梯度确定方法。首先借助油藏流场与电场相似的原理,推导了非均质砂岩油藏启动压力梯度计算公式。其次基于稳定流实验方法,建立了非均质砂岩油藏启动压力梯度测试方法。结果表明:低渗透非均质砂岩油藏的启动压力梯度确定遵循两个等效原则。平面非均质油藏的启动压力梯度等于各级渗透率段的启动压力梯度关于长度的加权平均;纵向非均质油藏的启动压力梯度等于各渗透率层的启动压力梯度关于渗透率与渗流面积乘积的加权平均。研究成果可用于有效指导低渗透非均质砂岩油藏的合理井距确定,促进该类油藏的高效开发。
