诱导型血红素氧合酶在腹泻型肠易激综合征患者中的表达

摘    要:本研究旨在通过灌注导管测压仪和免疫组化技术,检测24例腹泻型肠易激综合征患者(diarrhea predomlnant irritable bowel syndrome,D-IRS)及15例对照组的直肠压力、直肠对容量刺激的感觉阈值和结肠粘膜中诱导型血红素氧合酶(inducible heme oxygenase,HO-1)表达,以探讨HO-1在D-IBS发病机制中的作用。肠道测压结果显示腹泻型IBS患者的静息压、最大自主收缩压和排便压与对照组比较差异无显著性(P〉0.05),但初始感觉阈值、排便阈值和最大耐受阈值明显低于对照组(P〈0.01);HO-1在腹泻组IBS患者结肠粘膜中的表达也明显强于对照组(P〈0.01)。因此HO-1在腹泻型IBS患者结肠粘膜中的表达异常,表明一氧化碳(carbon monoxide,CO)在腹泻型IBS的发病机制中可能起重要作用。

Abstract:To investigate the role of inducible heme oxygenase(HO-1) in the pathogenesis of diarrhea predominant irritable bowel syndrome(D-IBS).The rectal pressure,rectal visceral perception thresholds to balloon distension and the distribution of HO-1 proteins in the colonic tissues in 24 cases of D-IBS and 15 cases of normal control group were detected by SABC immunohistochemistry and perfusion catheter manometer.Rectal resting pressure,maximal squeezing pressure and defecation pressure in the patients between D-IBS and control group were similar.Thresholds of first perception,defecation and maximal tolerable in the patients with diarrhea predominant IBS were significantly lower than control group.H0-1 was expressed mainly in the colonic mucous membrane epithelia.The expression of HO-1 in patients with D-IBS was higher than that in normal control group.The abnormal expression of HO-1 in D-IBS suggests that CO may plays an important role in the pathogenesis of gastrointestinal motility disorders such as D-IBS.
