乙型肝炎患者血清中IgM类HBsAg免疫复合物的检测及意义

以羊IgG抗人IgM为包被抗体,以HRP-抗HBs为指示抗体,建立了一种较简便的检测IgM类HBsAg免疫复合物的捕捉法酶联免疫吸附试验.实验证明,该法特异性强,重复性好.用本法对乙型肝炎的6种临床类型检测表明,IgM类HBsAg免疫复合物的检出率与临床类型有关.其阳性率在无症状携带者、急性乙型肝炎、慢性迁延性乙型肝炎、慢性活动性乙型肝炎、重症乙型肝炎和乙型肝炎后肝硬化分别为3.2%、36.7%、28.6%、54.4%、55.6%和61.2%.同时还发现,慢性活动性乙型肝炎的IgM类HBsAg循环免疫复合物有较强的激活补体作用,提示乙型肝炎患者血清补体下降与IgM类HBsAg循环免疫复合物激活和消耗补体有关.

DETECTION OF CIRCULATING HBsAg IMMUNE COMPLEXES OF IgM CLASS IN PATIENTS WITH HEPATITIS B AND ITS SIGNIFICANCE

A capture antibody-ELISA method had been established to detect circulating HBsAg immune complexes of IgM class ( HBsAg/IgM-CIC ) in patients with hepatitis B. It was simple, specific, sensitive and reproducible.The positive rates of HBsAg/IgM-CIC in HBsAg carrier, acute hepatitis B, chronicpersistent hepatitis B, chronic active hepatitis B, severe hepatitis B and cirrhosis after hepatitis B were 3.2%, 36.7%, 28.6%, 54.4%, 55.6%, and 61.2% respectively.The results showed that the positive rates were related to clinical types of hepatitis B. It had also been found that the HBsAg/IgM-CIC in patients with chronic active hepatitis B had more strong effect on activating complement. it was suggested that the decreased level of serum complement could be associated with HBsAg/IgM in hepatitis B.
