血液稀释治疗急性缺血性肾功能衰竭的实验研究

摘    要:将一例肾切除.另侧肾夹闭肾动、静脉和输尿管2小时造成急性缺血性肾功能衰竭的18只家兔随机平分三组。第Ⅰ组不加治疗,用作对照;第Ⅱ组用低分子右旋糖酐作等容量血液稀释;第Ⅲ组用GBK 液作二倍容量血液稀释。三组动物在同等条件下饲养,观察、比较其血尿素氮含量,无尿时间,尿量变化,存活时间和死后肾组织学改变。结果显示血液稀释有降低急性缺血性肾功能衰竭动物血中尿素氮,缩短动物无尿时间,提高动物存活率和减轻肾缺血造成的肾损害等作用。

Abstract:Acute renal failure(ARF) was induced in 18 unilaterally nephrec- tomized rabbits by occluding the renal artery,vein,and ureter of the other kidney for 2 hours.The rabbits were then divided into 3 groups at random. Group 1,the control group(n=6),received no therapy while Group 2,the dextran treated grou(n=6),used isovolumetric hemodilution with dextran as diluent, and Group 3,the G B K treated group(n=6),used hemodilution of double amount. with G B K as diluent.All the animals were fed under identical conditions for observation and comparison of blood urea nitrogen(BUN) content,anuric time, survival rate,and histological changes in kidney after death.It was found that the control animals(Group 1)developed ARF characterized by progressive rise in BUN concentration,anuria,high death rate,and damage of renal tissue upon examination after deach,whereas the hemodiluted animals(Group 2 and 3)showed a relief in ARF characterized by significant and steady decrease in BUN coucent- ration,shortened anuria time,lower mortality rate,and less damage of renal tissue.Experimental results showed that hemodilution can reverse ARF induced by renal ischemia.
