困难子宫摘除307例临床分析

目的：探讨困难子宫除的方法和手术原则。方法：对大理州妇幼保健院1982年至2001年困难子宫除307例进行临床分析。结果：下列为困难子宫除情况，即：宫颈肌瘤78例，巨大子宫肌瘤74例，严重粘连的子宫肌瘤67例，阔韧带肌瘤65例，侵蚀性葡萄胎15例，子宫肌瘤并妊娠8例。该组病例手术成功，末发生意外损伤。手术严重出血的病例经及时抢救康复。结论：术前诊断明确，术中处理方法得当，操作认真仔细是做好困难子宫除的三大要素。

Clinical Analysis on 307 Cases with Difficult Uterectomy

Objective:To explore the principles of diagnosis and operation for t he difficult uterectomy.Methods:To analysis all the clinical data of 307 cases wit h difficult uterectomy in Women and Children's Hospital of Dali prefecture from 1982 to 2001.Results:307 cases had been successfully cured.Among them,78 cases w ith myoma of cervix,74 cases with large uterine myoma,67 cases with seriously st icky myoma,65 cases with myoma of broad ligament,15 cases with invasive hydatidi form mole,8 cases with uterine myoma complicated by pregnancy.Conclusions:Correc t diagnosis,proper manipulate and suitable operation are very important factors for the difficult uterectomy.
