通辽地区志贺菌属菌群分布及耐药性研究

摘    要:目的:了解通辽地区志贺菌菌群分布及耐药情况,为临床合理用药提供科学依据.方法:从腹泻病人的粪便标本中分离到98株志贺菌,用生化反应和血清学方法鉴定分群,用Kirby—Bauer纸片扩散法进行抗菌药物敏感性试验.结果:98株志贺菌中,B群(福氏志贺菌)60株,D群(宋内氏志贺菌)38株,未分离出其他群的志贺菌;98株志贺菌对哌拉西林/他唑巴坦和头孢噻肟/舒巴坦合剂的耐药率明显低于其他β-内酰胺类抗生素,对碳青霉烯类的厄他培南敏感率为100%,对喹诺酮类的环丙沙星和左氧氟沙星耐药率分别为33.76%、25.18%,对氨基糖苷类的庆大霉素和大环内酯类的红霉素耐药率分别为26.27%、33.57%.结论:作者自通辽地区检出的志贺菌,对喹诺酮类、头孢类、氨基糖苷类和大环内酯类有较高的耐药率,治疗感染性疾病应该以药敏试验结果指导临床用药.

Abstract:Objective:Through investigating the distribution and drug resistance of Shigella in Tongliao to provide scientific basis for clinical rational drug use. Methods: 98 strains of Shigella were cultured from the stool samples of diarrhea patients. The groups were identified by biochemical and serologic tests. At the same time, the antibiotic sensitivity test was performed by Kirby-Bauer method. Results: There were 60 strains of S. Flexneri, 38 strains of S.Sonnei among 98 strains of Shigella. The resistance rates of Shigella to piperacillin/tazobactam and cefotaxime/sulbactam were remarkably lower than to other third-generation cephalosporins. The susceptible rates to ertapenem were 100%. The resistance rates to ciprofloxacin lactate and levofloxacin were 33.76% and 25.18%. The resistance rates to gentamicin and erythromycin were 26.27% and 33.57%. Conclusion: There are certain resistance rates of the Shigella to fluoroquinolones and the third-generation cephalosporins. To reduce the resistance, the antibiotic sensitivity test should be performed before clinical treatment.
