彩色室壁运动技术检测PTCA术对左室舒张功能的改善

为探讨应用彩色室壁运动技术检测经皮穿刺冠状动脉内成形术(PTCA)对左心室功能的影响,38例陈旧性心梗(OMI)患者,术前记录常规12导联心电图,二尖瓣血流频谱的1/3充盈分数(1/3EF)主进行彩色室外壁运动技术(COLOR、KINESIS、CK)参数测量,包括:左室壁各节段全舒张期及前1/3舒张期心内膜位移(DEM、1/3DEM),前1/3舒张期位移分数(1/3DEMF),全舒张期,前1/3及后2/3舒张期心内膜位移平均速度(VDEM、1/3VDEM、2/3VEM)以上根据二维定性方法对运动正常,运动减弱和运动消失三种室壁运动状态分别进行测量,然后进行彩色室壁运动检测继之行PTCA和血管内支架置入术,于术后1月重复上述测量.结果:术前CK测量结果显示,三种室壁运动状态间的DEM,VDEM均存在显著性差异(P＜0.01)且运动节段的1/3VDEM显著低于2/3VDEM,运动减弱节段的1/3VDEM略快于2/3VDEM,运动正常节段的1/3VDEM显著快于2/3VDEM,且1/3FF和1/3DEMF呈高度相关(P＜0.01),PTCA术后1个月左室舒张功能较术前有明显改善,表现为DEM、VDEM的增加(P＜0.05).

On Improvement the Effect of PTCA on Left Ventricular Diastolic Function Using Color Kinesis

To evaluate the effect of PTCA on left ventricular diastilic function using Color Kinesis 38 patients with OMI underwent CK imaging as following the transmittal 1/3 filling fraction (1/3EF) was calculated.The diastoic segmental endocardial motion(DEM).DEM of the first third diastolic period(1/3DEM) and 1/3 diastolic segmental endocardial motion fraction (1/3DEMF) .We compared CK measurements among 3 wall motion grades (i.e.normal hypokinetic and akinetic) assessed by quantitative wall motion scoring. All these measurements were one month after.There was a signifcant overall difference in DEM and VDEM(P<0.01)among wall motion grades,though 1/3 VDEM was slower than 2/3 VDEM in akinetic segments(P<0.01).There was also a close correlation between 1/3FF and 1/3 DMEF(P<0.01).The left ventricular diastolic function got a significant improvement (P<0.05) during one month after PTCA.
