糖尿病合并初治肺结核226例的临床对照分析

目的:探讨初治肺结核合并糖尿病患者的临床特点及预后.方法:对2001年1月~2007年12月收治的226例初治肺结核合并糖尿病患者进行回顾性分析并与单纯初治肺结核200例进行对照分析.结果:初治肺结核合并糖尿病患者的影像学特点与患结核病前血糖控制水平有关,血糖控制较好者肺结核影像多与单纯结核病组表现一致;血糖控制不好者,肺结核影像多不典型,痰菌阳性率高,治疗失败率高,抗结核治疗影像学改变慢于单纯肺结核者(P<0.05).结论:肺结核合并糖尿病患者影像学特点及结核病治疗效果与血糖控制水平密切相关,具有一定的临床特点,应积极控制血糖,早期抗结核治疗,且治疗疗程宜延长.

Contrast Analysis of 226 Cases of Diabetes Incorporated Initial Treating Pulmonary Tuberculosis

Objective:To investigate the clinical features and prognosis of initial pulmonary tuberculosis incorporated diabetes.Methods:The clinical features of 226 cases of diabetes incor-porated initial treatment pulmonary tuberculosis(A group) and 200 cases of pure pulmonary tub-erculosis(B group) were retrospectively and contrastedly investigated,all the cases were collectd from Jan.,2001 to Jul.,2007.Results:Radiography features of pulmonary tuberculosis incorporated diabetes are related with the level of blood sugar.If the blood sugar controlled well,the radiography features of A group are in accordance with B group.Otherwise,the radiography features of A group are worse than B group(P < 0.05);and the positive probability of sputum smear of A group is higher than B group;the treatment failure rate of A group is higher.Conclusion: Radiography features and therapeutic efficacy of the pulmonary tuberculosis incorporated diabetes are related to the blood sugar level intimately.It is important to control blood sugar level and start tuberculotherapy in early stage.The course of treatment should be extended reasonably.
