床旁血气分析仪与血液生化分析仪相同检测项目的比较分析

摘    要:目的探讨床旁快速血气分析仪与检验科大型血液生化分析仪在不同类别的样本中血钾、钠、血糖、红细胞比容以及血红蛋白5个相同检测项目的可比性。方法分别用GEMPremier3000血气分析仪与Bayer2400型全自动生化分析仪及雅培CD-1800全血细胞分析仪对123例体外循环术后入住ICU患者的动脉全血和静脉血清样本进行血钾、血钠、血糖、红细胞比容以及血红蛋白的检测,对检测结果进行相关分析和配对t检验。结果两类仪器测定血钾、血钠、血糖、红细胞比容和血红蛋白的结果均有显著相关性(r=0.795,r=0.200,r=0.324,r=0.540,r=0.708。全部P〈0.05),两类仪器测定血钾、血糖、血红蛋白的结果无统计学意义(P〉0.05)。GEMPremier3000血气分析仪测定的红细胞比容结果显著高于雅培CD-1800全血细胞分析仪(P〈0.05),血钠的结果显著低于Bayer2400型全自动生化分析仪(P〈0.05)。结论GEMPremier3000血气分析仪能替代传统生化分析仪测定血钾、血糖以及血红蛋白,但血纳和红细胞比容的结果有差异。

Comparative analysis of same test items by blood gas analyzer and biochemical analyzer

Abstract:Objective To explore the correlation and possible differences of blood potassium, sodium, glucose, hematocrit and hemoglobin by blood gas analyzer and biochemical analyzer. Methods The concentrations of artery or vein blood potassium, sodium, glucose, hematoerit and hemoglobin from 123 patients admitted to the ICU after cardiopulmonary bypass was examined by GEM Premier3000 blood gas analyzer and Bayer2400 Automated bio- chemical analyzer, Abbott CD - 1800 blood cell analyzer, and pearson correlation analysis and paired t - testwere employed to analyze the results. Results A significant correlation was found between results of blood potassium, sodium, glucose, hematocrit and hemoglobin of blood gas analyzer and biochemical analyzer( r = 0. 795, r = 0. 200, r = 0. 324, r = 0. 540, r = 0. 708, all P 〈 0.05 ). There was no significant difference between blood potassium, glucose and hemoglobin concentration detected by these two instruments, while hematocrit concentration by GEM Premier3000 blood gas analyzerwas significantly higher than that by Abbott CD - 1800 blood cell analyzer ( P 〈 0.05), and blood sodium concentration from GEM Premier3000 blood gas analyzerwas significantly lower than that by Bayer2400 Automated biochemical analyzer ( P 〈 0.05 ). Conclusion GEM Premier3000 blood gas analyzer can be used as a substitute for conventional biochemical analyzer in the detection of blood potassium, glucose and hemoglobin, but there are differences of blood sodium and hematocrit by twomethods above.
