腹腔镜与开腹子宫肌瘤剔除术的疗效比较

目的 比较开腹与腹腔镜子宫肌瘤剔除术的临床效果.方法 选择我院2005年1月～2009年1月收治要求保留子宫的子宫肌瘤患者80例,其中40例经腹子宫肌瘤剔除（开腹组）,40例经腹腔镜子宫肌瘤剔除（腹腔镜组）,比较两组手术时间、术中出血量、住院时间、术后恢复情况及住院费用.结果 两组在手术时间、术中出血量比较差异无统计学意义（P〉0.05）,术后肛门排气时间、术后病率、术后镇痛药使用、住院时间腹腔镜组明显少于开腹组（P＜0.05）,而住院费用腹腔镜组较开腹组高（P＜0.05）.结论 腹腔镜子宫肌瘤剔除术具有创伤小,术后恢复快及住院时间短等优点,符合微创治疗原则,可优先选择.

Compare the clinical effectiveness of laparoscopic with abdominal myomectomy

Objective To compare the clinical effectiveness of laparoscoplc with al3dOmlnal myomectomy. Methods Eighty patients with hysteromyoma in demand of saving uterus were collected from January 2005 to January 2009. Forty patients of laparoscopic myomectomy and 40 patients of abdominal myomectomy were selected as study group and control group respectively. The operation parameters of the two groups were compared, including operative time, intro - operative blood loss, recovery duration of gastrointestinal function and hospital charges.Resultes There was little statistical significance in the mean operative time and blood loss between the two groups （ P 〉 O. 05 I. The recovery time of gastrointestinal function, post - operative morbidity, the usage rate of pain - killer and hospital days in study group was significantly shorter than that in control group. Whereas accessorial hospital charges was found in lap group（ P 〈 0.05）. Conclusion With less wound, shorter post 2 operative recovery dutation and hospital days laparoscopic myomectomy is minimally invasive surgery and should be a prior cure.
